' County J �tq c0 N't`= SUBJECT: Approval of Contract Amendment Agreement#74-261-1 with Family Stress Center SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Haven Fearn) to execute on behalf of the County, Contract Amendment Agreement#74-261-1 with Family Stress Center, a non- profit corporation, effective October 1, 2005, to amend Contract #74-261, to increase the Contract payment limit by $27,511, from $88,124 to a new total of$115,635 and change the Contract term from October 31,2006 to lune 30, 2006. FISCAL IMPACT: This Contract is funded 100%by State Prop 10(First 5 Contra Costa). BACKGROUND/REASON(S) FOR RECOMMENDATION(S): On December 20, 2005, the Board of Supervisors approved Contract #74-261 with Family Stress Center, for the period from October 1, 2005 through October 31, 2006, for the provision of Early Intervention Mental Health services to children of pregnant and parenting women involved in the Child Welfare System. Approval of Contract Amendment Agreement#74-261-1 will reform the Contract to reflect the intent of the parties, and First 5 Contra Costa,which was for the Contractor to provide an additional level of services, including Continuing Care Services to women and children at Rosemary Corbin House, and correct the term of the Contract of October 1, 2005 through June 30, 2006. Because of late changes in the requirements of the funding agency,the Contra Costa Children and Families Commission,specification of the contractors'services to the graduates and their children could not be completed in time for these contracts. However,to make sure that the graduating residents and their children received continuity of care,Ujima and Family Stress have been providing basic follow-up and planning full continuing care under the original contracts. Therefore retroactive approvals of these amendments to the contracts are hereby requested to assure continuation of services. BENEFIT TO COUNTY Approval of these contract amendments will allow for Ujima to continue to provide Continuing Care services to clients graduating Rosemary Corbin House, and for Family Stress to continue to provide Continuing Care services to these clients'children, for up to six months post-graduation. AUTHORIZATION OF SERVICES These services were requested by the Contra Costa Children and Families(Furst Five) Commission and fimded through Contract#47-774 to the Alcohol and Other Drugs Services Division of the Health Services Department and authorized by the Alcohol and Other Drugs Services Division Director. FUNDING SOURCES These contract amendments are 100 percent paid by First Five Commission dollars.
