SUBJECT: Approval of Contract Amendment/Extension Agreement

SPECIFIC REQUEST(S)OR RECOMMENDATIONS)&BACKGROUND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute
on behalf of the County, Contract Amendment/Extension Agreement #74-049-8, with Oleg
Andreev, M.D., a self-employed individual, effective November 30, 2006, to amend Contract
#74-049-7, to increase the payment limit by $37,850, from $37,850, to a new payment limit of
$75,700, and extend the term from November 30, 2006 through May 31, 2007.
FISCAL IMPACT:
This Contract is funded 25% by Mental Health Services Administration (MHSA — Prop 63) and
75% by Mental Health Realignment.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
On June 6, 2006, the Board of Supervisors approved Contract #74-049-7 with Oleg Andreev, M.D.,
for the period from June 1, 2006 through November 30, 2006, for the provision of consultation and
technical assistance to the Department with regard to the MHSA — Prop 63, including data analysis
and report generation with regard to compliance, quality assurance, eligibility, billing, and claims
reporting, as required by the California Department of Mental Health, and development and
implementation of database systems to track Mental Health services, contractors, and employees.
Approval of Contract Amendment/Extension Agreement #74-049-7 will allow the Contractor to
continue providing services through May 31, 2007.
