HomeMy WebLinkAboutMINUTES - 10252005 - C51 To: BOARD OF SUPERVISORS FROM: William Walker, M.D., Health Services Director Contra By: Jacqueline Pigg, Contracts Administrator Costa DATE: October 12, 2005 County SUBJECT: Approve Standard Agreement(Amendment) #29-25045 with the State Department of Health Services for the Dental Disease Prevention Program SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.), to execute on behalf of the County, Standard Agreement(Amendment) #29-25045 with the State Department of Health Services, effective June 30, 2005, to make technical adjustments to the scope of work to identify the correct number of targeted students the program is to serve for the Children's Dental Disease Prevention Program,with no change in the original amount of$589,500 paid to the County and no change in the term of agreement from July 1 ,2004 through June 30, 2007. FISCAL IMPACT: Approval of this Standard Agreement(Amendment)will result in no change in the original State funding of $589,500 for the second year of the Dental Disease Prevention Program, ending June 30, 2007. No County match is required. BACKGROUND/REASON(S) FOR RECOMMENDATIONN. The Dental Disease Prevention Program is aState-mandated program, is designed to prevent and control dental disease found in children at selected school districts and to provide a comprehensive community-supported and school-based dental disease prevention program. The program serves approximately 21,156 children in elementary schools and preschool sites in Contra Costa County. On December 7, 2004, the Board of Supervisors approved Standard Agreement#29-25044 with the State Department of Health Services for the Dental Disease Prevention Program, for the period from July 1, 2004 through June 30, 2007. Approval of Standard Agreement.(Amendment) #29-250- 44 will revise the Exhibit A. Scope of Work to identify the correct number of targeted students in the program is to serve in the affected program components and to include a Head Start Objective for Plaque Control that was originally left out of the Contract. Approval of Standard Agreement (Amendment) #29-25045 will allow the County to continue to provide services for the Dental Disease Prevention Program, through June 30, 2007. Three certified/sealed copies of this Board Order should be returned to the Contracts and Grants Unit for submission to the State Department of Health Services.
