Approve and authorize the Health Services Director or his designee(Wendel Brunner,M.D.),to accept on behalf of the
County Grant Award #28-743 from Kaiser Foundation Health Plan, Inc. (dba Kaiser Permanente), in the amount of
$5,000,for the Asthma Management Education Project,for the period from September 1,2005 through June 30,2006.

Acceptance of this Grant Award will result in an amount of$5,000 from Kaiser Permanente,for the Asthma.Management
Education Project. No County funds are required.
RAC'KGRnTTNn/RF.ASnN(S)FOR RECOMMENDATION(S):
Asthma is a chronic respiratory disease of enormous public health significance and is associated with significant
disability and costs,both monetarily and its impact on the quality of life. Asthma is the most common chronic disease of
childhood and one of the leading causes of childhood hospitalizations and school absences. A recent summary of
hospital discharge data. compiled by the Regional Asthma Management and Prevention (RAMP) Program provides
specific information about areas of Contra Costa County where asthma is disproportionately affecting children. It is the
schools serving these communities and the families living in these areas that are the focus of this project. In collaboration
with the Regional Asthma Management and Prevention (RAMP) Initiative, Richmond Health Center, John Muir/MT.
Diablo Health System,Contra Costa Health Plan and Public Health Clinic Services,this project will be integrated into the
pediatric asthma program currently underway in Public Health Clinics Services. The goal of the project will be to
perform educational home visits to address the home environment and to enhance asthma self-management skills by
conducting in-school asthma management education for children with asthma. This Project will also reduce school
absences, emergency room visits and hospitalizations for these children and assist in relieving some of the burdens that
families experience in dealing with this chronic disease.
Approval of this Grant Award will provide additional funding from Kaiser Foundation Health Plan, Inc. (dba Kaiser
Permanente)for the Asthma Management Education Project,through June 30,2006.
Three sealed and certified copies of the Board should be returned to the Contracts and Grants Unit.
