SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
ADOPT Position Adjustment Resolution No. 19784 to establish a Workers' Compensation Claims
Supervisor (AJHB) class and allocate the salary of Workers' Compensation Claims Supervisor to
13851 1717 ($5,622 — $6,833), and add two (2) positions in the Risk Management Division of the
County Administrator's Office as recommended by the Human Resources Director.
ANNUAL & FISCAL IMPACT:
There are no Net County Costs associated with this action. According to the Risk Management
Division, the total annual cost of these position will be $204,990. The total FY 2004/2005 cost will be
$102,494. The position will be funded by the Workers' Compensation Trust Fund.
BACKGROUND:
In 1986,-the Workers' Compensations Unit transitioned from a third party administrator and brought
claims in-house. In response this unit change, the Assistant Risk Manager was created in December
1998, and throughout the years has had responsibility for the entire Workers' Compensation
Program, which has included supervising claims adjusters and clerical staff, and managing and
administering the Workers' Compensation Program. Under the supervision of the Assistant Risk
Manager, Workers' Compensation Claims Adjusters have been responsible for a claims caseload
well above the industry standard of 150. The vision of department management is to break the
Workers' Compensation Unit into two (2) pieces having each Workers' Compensation Claims
Supervisor responsible for about eight (8) Workers' Compensation Claims Adjusters and some
clerical staff. This will provide the program staff with adequate line supervision for quality control,
training, and the day-to-day supervision, as well as implementing substantial legislative changes that
have been enacted in the past two years. In addition, this will allow the Assistant Risk Manager to
focus on managing and administering the Workers' Compensation Claims Program.
CONSEQUENCE OF NEGATIVE ACTION:
These positions are critical to the functioning of the rkers' Compensation Clai Program.
Positions in this class will be responsible for supervisin W kers' Compen ion Clai s Adjusters
which is vital to efficient program operations.

RECOMMENDATION:
Adopt Position Adjustment Resolution Number 19771, reallocating the salary of Computer Operations
Analyst (LKVA) from B85-1312 to B85-1427.
FISCAL IMPACT:
This action will result in an annual cost of $26,490.24. Funding for this position is within the
Department's budget and the cost is recovered through user fees. _
BACKGROUND:
An Information Technology salary study done in 1998 did not include the Computer Operations
Analyst classification, creating an inequity in salary with comparable classifications in Contra Costa
County. Before this study, the classifications of Computer Operations Analyst and Information
Systems Technician II were equally compensated. The Computer Operations Analyst is expected to
and must be able to perform the computer operator, shift supervisor, and technician duties while
covering the swing and graveyard shifts for Computer Operations Supervisor. If the reallocation of
salary is approved, the Department has agreed to eliminate the temporary upgrade pay that
Computer Operations Analyst receive when filling in for a Computer Operations Supervisor.
CONSEQUENCE OF NEGATIVE ACTION:
The consequence of negative action will result in the inability of the Department to staff this complex
area and correctly compensate the incumbents.

RECOMMENDATION : Adopt Position Adjustment Resolution # 19823 adding one Clerk — Senior
level position and-canceling one vacant Secretary—Advanced level position.
FISCAL IMPACT: This action results in a cost savings of approximately$10,000 annually.
BACKGROUND: The Secretary—Advanced level position is vacant and is no longer needed. The
previous incumbent was promoted to Executive Secretary and took many of the duties .of the position
with her. The remaining duties no longer warrant aSecretary—Advanced Level designation.
The Clerk— Senior level position is needed due to heavy workload. Currently, many of the county's
health clinics are staffed with temporary clerks even though there is a need for permanent staff. The
addition of a permanent position will make it easier for the department to recruit and retain qualified
staff.
RESULT OF NEGATIVE ACTION: The Health Services Department will have to continue to rely on
temporary staff which makes it more difficult to recruit and retain qualified employees.

RECOMMENDATION Adopt Position Adjustment Resolution # 19812 increasing the hours of
Medical Record Technician position # 9511 from 20/40 to 40/40.
FISCAL IMPACT: The cost associated with this request is approximately $32,748 per year and will
be offset by reductions in overtime and registry costs.
BACKGROUND: This position is assigned to the Release of Information Unit at the North Richmond
Center for Health. The increase in hours is needed to address increased workload and to provide
efficient workflow in the Medical Records Department. This position is currently vacant and the
department has had difficulty recruiting for apart-time position. In order to provide adequate staff
coverage, the department has had to rely on overtime and registry staff at a significantly higher cost
to the county.
RESULT OF NEGATIVE ACTION: Without the increase in hours, the department will continue to
have inadequate staff coverage and will be forced to continue to use overtime and registry staff.

ADOPT Resolution #2005/60 to adjust the Phase 112004/2005 Budget Rebalancing Plan
approved by the Board of Supervisors on August 10, 2004, as recommended by the
County Administrator.
FISCAL IMPACT:
None. Increased position costs will be absorbed within the current Department Budget.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
On August 17, 2004 the Board of Supervisors approved Position Adjustment Resolution
#2004/435 which reduced the hours of twelve positions in the Department of Health
Services Detention Medical program. These position reductions were requested as part of
the 2004/2005 Phase II Budget Rebalancing Plan and were made possible due to the
anticipated implementation of a robotic pharmaceutical distribution system to be
implemented in the County jails.
Due to unanticipated issues involving the purchase, delivery and installation of the new
system, it will not be operational by the end of January as originally planned. Currently, it
is anticipated that the system will be fully functional by the end of March 2005. Therefore,
the Health Services Department is requesting that the position reductions be delayed with
a new effective date of"on or before June 30, 2005." This will allow the Department
sufficient time for installation of the equipment and complete testing. Position hours will
be reduced as soon as feasible, but no later than the end of the fiscal year. Impacted
positions are shown on Attachment A. This new resolution supercedes and replaces
Resolution 2004/435.
CONSEQUENCES OF NEGATIVE ACTION:
Reduction of the hours for the impacted positions, prior to the implementation of the
robotic pharmaceutical distribution system, will result in the inability to distribute the
needed pharmaceuticals in the County jails.

SUBJECT: Reduction of Hours for Certain Employees in the Health Services Department
THE CONTRA COSTA COUNTY BOARD OF SUPERVISORS in its capacity as governing Board of the
County of Contra Costa and all districts of which it is ex-officio governing Board RESOLVES THAT:
1. The Board has considered the financial impact on the Health Services Department due to reduced funding
and has considered the position and staff reduction/retention plans submitted by the Department.
2. The Board has considered the impact of limited funding and the staffing necessary for the efficiency of
County government operations and has considered the staff retention plans submitted by the Health
Services Department for the Detention Medical Program.
3. This resolution supersedes and replaces Resolution 2004/435.
4. In order to keep expenditures within available funding, it is necessary to reduce the hours of positions
described in Attachment A on the dates indicated for reasons of economy, lack of funds or because the
necessity for the position(s) involved no longer exists and to lay off employees accordingly. Said list is
incorporated herein by reference, and the hours of said positions are hereby reduced effective on or before
June 30, 2004. 0
5. The Health Services Department shall issue layoff or displacement notices, as the case may be, and give
notice to the affected employees of the Board's action.
6. The Employee Relations Officer shall give notice of this Resolution to recognized employee organizations
representing employees impacted by this action.
7. To the extent that the subjects of this Resolution are within the scope of representation pursuant to the
Meyers-Milias-Brown Act (Government Code Section 3500 et seq.), this Board offers to meet with
recognized employee organizations upon request concerning this Resolution.
8. Recognized employee organizations may submit to the Employee Relations Officer written requests to
meet and confer on specific proposals with respect to the Resolution or resulting layoffs. This
authorization and direction is given without prejudice to the Board's right to reduce or terminate the
operations and services of the County and of Districts governed by this Board, and to eliminate classes of
employees or positions as involving the merits, necessity, or organization of services or activities of the
County or districts governed by the Board and.not within the scope of representation.
9. This action is taken without prejudice to pending consulting, meeting, and meeting and conferring with
recognized employee organizations.
