Diablo Health System) SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION JIF(''nMMF.Nn ATTnN(�1: Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.), to execute on behalf of the County, Contract #22-780-5 with John Muir Health (formerly: John Muir/Mt. Diablo Health System), in an amount not to exceed$3,000, for County's use of a mobile van in the provision of health services in Central and East Contra Costa.County,for the period from January 1,2006 through December 31,2006. John Muir Health (formerly: John Muir/Mt Diablo Health System) has requested to be held harmless and indemnified as part of their standard for approval of the Agreement with the County. In the view of County Risk Management this indemnification will create a minimal potential loss. The Health Services Department desires to expedite approval of the Agreement as written. FI&CAL IMPACT: This Contract is funded 100%by a Federal Health Care for the Homeless Grant. Approval of Contract #22-780-5 will allow the County to use the Contractor's mobile van to conduct regularly scheduled clinics which offer much needed health care services to low-income families and disadvantaged individuals in Central and East Contra Costa County, particularly along the Monument Corridor, through December 31,2006. FISCAL IMPACT: This Contract is 100%funded by the California Department of Health Services—On the Move with FoodWise Program. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): The California Nutrition Network is the primary funding source of food and nutrition initiatives in the Public Health Division. The food and nutrition program recently had significant leadership change. This contract will allow program continuity while new leadership is recruited and hired. Under Contract #22-934, the Contractor will provide consultation and technical assistance to the Department with regard to County's Nutrition Programs, including ensuring compliance with funding requirements, managing focus groups and pilot testing, supervising workshops and other nutrition education services, collecting data, and developing documentation procedures, through January 31,2007. FISCAL IMPACT: This Contract is funded 100%by the Department Enterprise I budget. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): On January 4, 2005, the Board of Supervisors approved Contract #23-278-6 with Robert Asher for the period from February 1, 2005 through January 31, 2006, for the provision of consultation and technical assistance with regard to the Call Center Communications and the Homeless Management Information Systems including, but not limited to, system administration, troubleshooting, capacity planning and monitoring, and coordinating of system-wide maintenance, settings and updates. Approval of Contract Amendment/Extension Agreement #23-278-7 will allow the Contractor to provide additional hours of services through April 30,2006. Under Contract #23-225-1, the Contractor will conduct search activities throughout the healthcare industry, focusing on hospitals, integrated health organizations, managed care organizations, and healthcare information consulting firms and refer qualified candidates to the Department for consideration. This Contract includes asix-month automatic extension through December 31, 2006,in an amount not to exceed$24,700. FISCAL IMPACT: This Contract is funded 100% by State Funds through the Employment and Human Services Department. CHILDREN'S IMPACT STATEMENT: This CalWORKs program supports the following Board of Supervisors' community outcomes: "Children Ready For and Succeeding in School"and"Families that are Safe, Stable, and Nurturing". Expected program outcomes include an increase in positive social and emotional development, as measured by the Child and Adolescent Functional Assessment Scale (CAFAS) and a decreased use of acute mental health care. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): MMNNWNEWNP� nwmmww� This Contract meets the social needs of County's population in that it provides individual, group and family collateral counseling, case management, and medication management services for CalWORKs participants to reduce barriers to employment. On January 18, 2004, the Board of Supervisors approved Novation Contract #74-035-6 with Early Childhood Mental Health Program for the period from July 1, 2004 through June 30, 2005, (which included asix-month automatic extension through December 31; 2005) for the provision of mental health services to recipients of the CalWORKs Program and their children. Approval of Novation Contract#74-035-7 replaces the automatic extension under the prior Contract and allows the Contractor to continue providing services through June 30,2006.
