FISCAL IMPACT: This Contract is funded by Contra Costa Health Plan member premiums. Costs depend upon utilization. As appropriate,patients and/or third party payors will be billed for services. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): The Health Plan has an obligation to provide certain specialized professional health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County. In February 2004,the County Administrator approved and the Purchasing Services Manager executed Contract #27-185-3 with Jorge Salazar-Suero,M.D.,for the period from February 1,2004 through January 31,2005,for professional primary care services for Contra Costa Health Plan members. Approval of Contract 927-185-4 will allow the Contractor to continue to provide professional primary care services to Health Plan members, through January 31, 2007. Approval of Contract#27-291-7 will allow the Contractor to continue to provide professional ophthalmology services to Health Plan members,through January 31, 2007. (#29-187-0) FINANCIAL IMPAM, None. All expenses are 100%claimable under the State Travel Policy. Ms. Karen Cordes has been assigned since June 2002, to work out-of-county as the Contra Costa representative during development and implementation phase of the CaIWORKS Information Network(CalWIN)Program.in Sacramento County. Her housing,travel,meals, and some utility costs incurred in connection with this assignment are reimbursable under the State Travel Policy and is being handled as a monthly supplemental amount through the Payroll System as recommended by the Auditor's Office. She receives a Flat Monthly Allowance of approximately$3,608/month. On January 6, 2004 (Item 78), the Board approved the extension of Ms. Cordes' out-of-county assignment through December 31, 2004, as a result of delays in CaIWIN implementation. There are further delays with implementation and it is anticipated that Ms. Cordes will now be working on CalWIN project through.June 2005. This Beard Order will amend accordingly the terra of her out- of-county assignment and increase the total payment amount through that term.
