Approve and authorize the Health Services Director, or his designee
(Jeff Smith, M. D. ) , to execute on behalf of the County, Contract #26-
345-10, Agreement #V261S-0290 with the U. S. Department of Veterans
Affairs Northern. California Health Care System (VANCHCS) , for the
provision of Neurology services, in an amount not to exceed $35,000.
in accordance with the term of the agreement, County agrees to
indemnify and hold harmless Contractor from claims, losses, damages,
liabilities, costs, expenses or obligations arising out of, or
resulting from CCRMC's wrongful or negligent conduct in the
performance of this Contract, for the period from October 1, 2004
through September 30, 2005

This Contract is funded in the Health Services Department' s Enterprise
I Budget. The services provided for the County's patients under this
Contract are billable to patients and third party payors.

For many years the County and U.S. Department of Veterans Affairs has
maintained a mutual sharing agreement, which has made available to the
County specialized medical services not otherwise available due to
lack of resources, equipment, and personnel. These services included
specialized laboratory testing, radiology services, nuclear medicine
studies, CT scans, MRI 's, dermatology, gastroenterology, urology,
audiology and speech, 24-hour ambulatory EEC monitoring, outpatient
Multiple Sleep Latency Studies, and ophthalmology services. The
County provided Emergency Roam treatment and inpatient care, including
certain ancillary services, for VA-referred patients.
Approval of Contract #26-345-10, Agreement #V261S-0290 will allow
VANCHCS to continue providing Neurology services to patients referred
for treatment by Centra Costa Regional Medical Center physicians
through September 30, 2005.
The Contract documents have always been prepared by the Veterans
Administration.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director,or his designee(Richard Harrison),to execute on behalf of
the County, Contract #27-458-5 with. Beverly Jacobs, PHN, a self-employed individual, in an amount not to
exceed$93,471,for provision of consultation and technical assistance to the Department with regard to the local
Initiative and other Contra Costa Health Plan programs,for the period from February 1,2005 through January 31,
2006.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums. Costs depend upon utilization. As
appropriate,patients and/or third party payors will be billed for services.
BACKGROUND/REASOMS3 FOR RECOMMENDATIONN:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County.
On January 27,2004,the Board of Supervisors approved Contract#27-458-4 with Beverly Jacobs,PHN,for the
period from February 1,2004 through January 31, 2005,to provide consultation and technical assistance to the
Health Plan with regard to the Local Initiative and other Health Plan programs including evaluation of the local
Initiative,attending meetings with the State Department of Health Services,and making presentations to Health
Plan participating providers to maintain compliance with State regulations and requirements.
Approval of Contract#27-4585 will allow the Contractor to continue to provide services,through January 31,
2006.

the County, Contract#26-507, with BAART (DBA California Detoxification Programs, Inc.), a Corporation, in
an amount not to exceed $50,000, to provide a Methadone Opiate Treatment Program for inmates housed at the
Main Detention and West County Detention facilities, for the period from September 1, 2004 through June 30,
2005.
F.ISCAL IMPAC:
This Contract is 100%funded.by County funds.
REASON(S)FOR RECOMMENDAIMN(SURACKGRO1 IND:
Due to Budget reductions, the Detention Health Services (DHS) has been unable to meet the staffing
minimums to complete the NCCHC accreditation, and as a result of this action, the DHS has had to relinquish
its Federal Licenses to operate a Methadone Opiate Treatment Program for inmates housed at the Main
Detention and West County :Detention Facilities. The existing licensing waiver with the DBA expired on
August 31, 2004. BA-ART currently provides these services for the Mental Health Division, and has agreed to
provide these services for inmates in custody of the Contra Costa County Sheriff
The Contract was delayed due to a need to clarify the specific deliverables and programmatic obligations of
both County and Contractor.
Under Contract #26-507, Contractor will provide Methadone Opiate treatment and maintenance services, at the
request of the County, for inmates housed at the Main Detention and West County Detention Facilities, through
June 30,2005.

RECOMMENDATION
1. APPROVE an Amendment in the amount of $5,874 and authorize extra work under the
Consulting Services Agreement dated October 19, 1999 with Don Dommer Associates for
additional architectural services for the New Fire Station No. 79, 1423 Lillian Street, Crockett.
The Amendment increases the Payment Limit from $124,345 to $130,219.
2. AUTHORIZE the Director of General Services, or designee, to execute the Amendment to the
Consulting Services Agreement and to issue written authorizations for extra work, in addition to
the authorization in the Amendment, provided that the cumulative total cost for all such
authorizations shall not exceed $5,000.
FINANCIAL IMPACT
All expenses are funded by the Crockett-Carquinez Fire District.
BACKGROUND
On October 19, 1999 the Board of Supervisors approved and authorized the Director of General
Services to execute the consulting services agreement with the Consultant. The Consultant provided
architectural services for the demolition and replacement of Fire Station No. 79 in Crockett.
The Amendment increases the scope of services to provide for extended construction administration
services because of the extended project duration caused by the termination of the original general
contractor and use of a replacement contractor.

TENTH AMENDMENT TO CONSULTING SERVICES AGREEMENT
FOR NEW FIRE STATION NO.79,1423 LILLIAN STREET,CROCKETT

I. Effective Date and Parties: Effective January 4, 2005, Don Dommer Associates(herein called "Consultant"),a California Corporation, and the
County of Contra Costa(herein called"County") a political subdivision of the State of California,mutually agree as follows:
2. Purpose: On October 19, 1999,the parties entered into a contract entitled"Consulting Services Agreement,"referred to as the"Agreement,"which
covers architectural services for the demolition and replacement of Fire Station No.79 in Crockett. The Agreement has previously been amended as
follows: First Amendment dated December 8, 1999, Second Amendment dated May 8, 2000, Third Amendment dated October 9, 2000, Fourth
Amendment dated February 6,2001,Fifth Amendment dated August 7,2001,Sixth Amendment dated January 1,2002,Seventh Amendment dated
June 4,2002,Eighth Amendment dated September 6,2002 and Ninth Amendment dated January 9,2003. The parties desire to further amend the
Agreement to expand the scope of service and to increase the payment limit accordingly.
3. Amendments to Agreement:
A. In the Agreement, Section 1(c), change the Payment Limit from $124,345.00 to$130,219.00, an increase of$5,874.00. This change shall
apply to charges from and after August 12,2003.
B. In accordance with Section 14 of the Agreement provide the following:
Assist the Fire District in completing the project by providing professional architectural services after termination of the original contractor's
rights under their contract.
4. Effect:
Subject to the revisions made by this and any prior amendments,the Agreement shall remain in full force and effect.

BACKGROUND-
Employment and Human Services (EHSD)received security guard services from the Guardian Security Agency
through an interdepartmental agreement with the Health Services Department(HSD). The contract and
interdepartmental agreement have terminated, as HSD is no longer using contract security personnel at HSD
locations. Guardian Security Agency continued to provide services to EHSD locations under a contract with the
Department. The rates established under the HSD interdepartmental agreement and contract remain in place.
The increase in total contract payment limit is requested for services at additional locations and unanticipated
guard overtime required at EHSD locations. During this period of coverage, EHSD will engage in the
competitive bid process for security guard services for EHSD site locations.
