The Employment and Human Services Department is requesting authority to extend the grant from the U. S.
Department of Health and Human Services, Administration of Aging to support the Providing Assistance to
Caregivers in Transition(PACT)Program. This program,which is a partnership between the Employment ent and

Human Services Department and Health Services Department, has been designed to support, educate and
empower Fam
• ppily Care Givers (FCGs)with family members, aged 60+, who have recently been placed in skilled
nursing facilities. With referrals from hospital discharge planners, PACT provides various levels of care
coordination for FCGs whose care recipient chooses to transition back to the community, as well as for those
who will remain in the skilled nursing facility.
The PACT Program:
• Assists the family caregiver in understanding the care recipient's current medical and psycho-social

needs, situation and available alternatives,
• Provides information, education and referrals,
• Conducts assessments to determine the care recipient's ability to transition into the community,
• Designs and implements a care plan which supports the family caregiver's needs if the care recipient
remains in the skilled nursing facility or chooses to transition to the community).
Staffed by a social worker and a public health nurse, and housed in the County's Aging and Adult Services

Bureau, PACT contracts with local organizations to assist family caregivers with necessary support ort and
transition services. PACT provides a continuum of services to a care-giving population that has traditionally
been isolated during the difficult time period of the first weeks/months after placement of a loved one. PACT
enables family caregivers to maximize their ability to keep their family me in the"most integrated and
appropriate setting"possible.

RECOMMENDATION(S),:
Accept a contribution of$63,450 from Meals on Wheels of Contra Costa, Inc., on behalf of the County Home
Delivered Meals Program (Meals on Wheels), administered by the Public Health Division of Contra Costa
Health Services.
FISCAL IMPACT:
This contribution of$63,450 subsidizes approximately 31,700 additional nutritious meals delivered to nearly
500 additional homebound elders at their residences each day this calendar quarter through the countywide
Meals on Wheels network. Substantial voluntary contributions from program clients provide sufficient financial
leverage to allow one additional meal to be delivered to a homebound elder for each$2.00 received in private
sector contributions or grants. A portion of this contribution, $3,450, reimburses the county for fundraising
development costs incurred on behalf of the program
BACKGROUND:
Meals on Wheels of Contra Costa, Inc., is a charitable private non-profit coalition of public and private
organizations founded in 1990, by two County managers from the Health Services and Employment and
Human Services Departments, to develop on-going private sector funding resources to bridge the gap
between stagnant program funding, levels and an ever-growing demand for Meals on Wheels services for the
county's homebound elders. With this contribution, Meals on Wheels of Contra Costa, Inc. has donated
$1,287,300 since its inception in 1990,to subsidize approximately 736,400 meals delivered to the residences
of homebound elders throughout Contra Costa County. Meals on Wheels of Contra Costa, Inc., is the only
organization in the county that raises funds specifically for meals for homebound elders served through the
countywide Meals on Wheels network.
Organizations represented on the Meals on Wheels of Contra Costa, Inc. Board of Directors include: Aging
and Adult Services Bureau (Employment& Human Services Department); County Senior Nutrition Program
(Health Services-Public Health); County Advisory Council on Aging; Senior Nutrition Program Advisory
Council; West County Meals on Wheels, Inc; Family & Community Services, Inc. (dba Senior Outreach
Services); and Martinez Shell Oil Refinery.
The County Home Delivered Meals Program is the primary provider of Meals on Wheels services in Contra
Costa County, delivering over 1,000 meals each day to the residences of homebound elders throughout the
county via its network of 70 delivery routes almost entirely staffed by volunteers. Service delivery is
coordinated regionally in much of the county through contracts with private non-profit community-based
agencies.

On April 20, 2004, the Board of Supervisors approved Contract #29-771-8 with Partnership Health
Plan of California for the period February 1, 2004 through February 28, 2005 to provide the
Partnership's Health Plan members with telephone services from Contract Costa Health Plan's
Advice Nurses including: information about how to access urgent care services; authorization for
emergency services; and clinical advice. the Partnership Health Plan of California is the entity which
oversees Solano County's Medi-Cal Managed Care Program.
Approval of this Contract #29-771-9 will allow Contra Costa Health Plan to continue providing after
hours Advice Nurse services to members of the Partnership Health Plan of California, through

BACKGROUND/REASONS FOR RECOMMENDATIONS
A. The City of Walnut Creek, the City of Walnut Creek Redevelopment Agency and the
County executed an agreement dated July 1, 2004, for the 2004-05 fiscal year.
B. The City has approved and authorized the expenditure of an additional One Hundred
One Thousand, Five Hundred Dollars ($101,500.00) in redevelopment funds. The City
of Walnut Creek Redevelopment Agency desires to increase the funding it provides
pursuant to the Agreement by $101,500.00 for a total of $173,500.00.
C. The County has demonstrated the experience and the qualifications to operate a
housing rehabilitation program which includes the following: screening and approving
loan applicants; preparing and presenting loan documents; developing a list of repairs
to be performed; inspecting repair work for compliance; and authorizing contractor
payments for work completed.
D. This is the second year the County has contracted with the City of Walnut Creek for
these services. The Amendment will result in an additional $20,300 in revenue to the
County for costs related to the implementation of the Program.
E. The term of the amendment is January 25, 2005 to June 30, 2005.

FIRST AMENDMENT TO AGREEMENT BETWEEN CONTRA COSTA
COUNTY, CITY OF WALNUT CREEK AND CITY OF WALNUT CREEK
REDEVELOPMENT AGENCY FOR HOUSING REHABILITATION LOAN
PROGRAM SERVICES
This Amendment is entered into on January 25, 2005, by and among Contra Costa
County("County"), the City of Walnut Creek("City"), and the Redevelopment Agency
of the City of Walnut Creek ("Agency").
Recitals
A. County, City and Agency are parties to an Agreement for Housing
Rehabilitation Loan Program Services dated July 1, 2004 (the "Agreement"). The
Agreement provides for the County to administer a housing rehabilitation loan program
for City and Agency involving loans of City and Agency funds, including up to $72,000
in Agency funds (less administrative expenses). Agency desires to increase the funding it
provides pursuant to the Agreement by$101,500.00 for a total of$173,500.00.
Now, therefore, in consideration of the terms and conditions of the Agreement
and this Amendment,the parties hereto agree as follows:
Agreements
1. Recital E of the Agreement is hereby amended to read as follows:
"E. The Agency and City have a housing rehabilitation loan program. The
program is funded by redevelopment funds, city general funds, and Community
Development Block Grant funds. For fiscal year 2004-2005, the Agency has approved
and authorized the expenditure of$173,500 in redevelopment funds for the housing
rehabilitation loan program. For fiscal year 2004-2005, the City has approved and
authorized the expenditure of$60,000 in City general funds for the housing rehabilitation
loan program."
2. Section 4 of the Agreement is hereby amended to read as follows:
"4. Compensation. The Agency and City will pay the County a total of$46,700
as compensation for the services provided by the County to the Agency and City under
this Agreement. This amount includes all administrative and technical services provided
by the County, including the Rehabilitation Specialist's salary, administrative oversight,
clerical assistance and overhead costs. The Agency and City will make payments to the
County of$6,600 on September 30, 2004; $6,600 on December 31, 2004; $16,750 on
March 31, 2005; and$16,750 on June 30, 2005."

3. All other terms and conditions of the Agreement shall remain in full force and
effect.

FISCAL IMPACT
The commercial property owners at Pleasant Hill BART have voted to approve the formation of a County
Service Area to fund mandated Transportation Demand Management services in the area. The County
Service Area assessment generates approximately$190,000/year(Fund 247600). In addition$25,000
will be funded by the County from general revenues generated at Contra Costa Centre. An additional
$25,000 from fund 111200—Pleasant Hill BART Transit Planning is incorporated into this contract for a
special project related to establishment of a city car share program.

BACKGROUND/REASONS FOR RECOMMENDATIONS
Contra Costa County has required a Transportation Demand Management(TDM)program
for the Pleasant Hill BART Station Area since 1986 to encourage the use of alternative
modes of transportation by station area employees in order to mitigate the traffic impacts
resulting from the development of the area.The County has entered into an agreement with
the Contra Costa Centre Association to manage the TDM program for the Pleasant Hill
BART Station Area each year since 1990. The property owners in the Pleasant Hill BART
Station Area voted to approve a County Service Area with an annual assessment to fund
TDM. In addition the County retains funds from a prior assessment proceedings for special
projects related to transit planning implementation. These funds are incorporated into the
contract for vanpool replacement, transit fairs, and initiation of a City Car Share Program.
The program offers commute alternative services to station area employees. While there is
no mandate requiring participation by employers or any penalty for non participation,the area
employers and their employees have been highly participatory in the program. The TDM
program goals of achieving over 30% of the area employees travel to work via alternative
modes(transit,carpools,vanpools,bicycle,walk,telecommute,etc.)have consistently been
met.
The project agreement for 2004-2005 covers a 12-month period from July 1, 2004 to June
30, 2005. It continues funding for operating vanpools, for operation a noontime shuttle
service to nearby commercial areas, for on-site transit ticket sales, and for marketing and
information programs.
