November 15, 2005 co, c- County SUBJECT: Accept Contributions to County Home Delivered Meals (Meals on Wheels) Program SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): ACCEPT a contribution of$75,125 from Meals on Wheels of Contra Costa, Inc.,on behalf of the County Home Delivered Meals (Meals on Wheels) Program, administered by the Public Health Division of Contra Costa Health Services. FISCAL IMPACT: This contribution of$75,125 subsidizes approximately 37,600 additional nutritious daily meals delivered to nearly 550 additional homebound elders at their residences this calendar quarter through the countywide Meals on Wheels Program. Substantial voluntary contributions from program clients provide sufficient financial leverage to allow one additional meal to be delivered to a homebound elder for each $2 received in private sector contributions or grants. A portion of this contribution, $5,125, reimburses the county for fundraising development costs incurred on behalf of the Meals on Wheels of Contra Costa. BACKGROUND: Meals on Wheels of Contra Costa, Inc., is a charitable non-profit coalition of public and private organizations founded in 1990, by two County managers from the Health Services and Employment and Human Services Departments,to develop on-going private sector funding to bridge the gap between stagnant program funding levels and an ever-growing demand for Meals on Wheels services for the county's homebound elders. With this contribution, Meals on Wheels of Contra Costa, Inc. has donated$1,500,700 since its inception in 1990,to subsidize approximately 843,300 meals delivered to the residences of homebound elders throughout Contra Costa County. Meals on Wheels of Contra Costa, Inc., is the only organization in our county raising funds specifically for meals delivered to homebound elders through the countywide Meals on Wheels Program. Organizations represented on the Meals on Wheels of Contra Costa, Inc. Board of Directors include: Aging and Adult Services Bureau (Employment& Human Services Department); County Senior Nutrition Program (Health Services-Public Health); County Advisory Council on Aging; Senior Nutrition Program Advisory Council;West County Meals on Wheels, Inc.; and Meals onWheels/Family&Community Services, Inc. (dba Senior Outreach Services). The County Home Delivered Meals Program is the primary provider of Meals on Wheels services in Contra Costa County, delivering over 1,020 daily meals to the residences of homebound elders throughout the county via its network of 70 delivery routes almost entirely staffed by volunteers. County has been the recipient of Federal Health Resources and Services Administration (HRSA) Bioterrorism Hospital Preparedness, local assistance grants funded, through the California Department of Health Services for the past three years. Funding from these grants is intended to enhance the preparedness of hospitals and community clinics to respond to potential terrorist events and other disasters. Under Contract#23-187-5, Contractor will assist the EMS Division in developing and preparing a Pro ject Work Plan to identify proposed expenditures and acquisitions of equipments and supplies needed in order to meet critical benchmarks of the HRSA grant, including, but not limited to, analyzing and reviewing the steps required to advance County's preparedness level, coordinate and prepare grant applications, act as a liaison between County and the Department of Health Services, attend scheduled meetings, and, prepare reports to the Department of Health Services and provide additional material as required by DHS,through August 31, 2006. BACKGROUND/REASON(S) FOR RECONIlENDATION(S) On November 30, 2004, the Board of Supervisors approved Contract #24-238-39 with Mental Health Consumer Concerns,, Inc. , for the period from July 1, 2004 through June 30, 2005, (which included a six-month automatic extension through December 31, 2005) for the provision of a State-mandated Patients ' Rights Advocacy Program which includes general advocacy and training services, Certification Review Hearing services, and Reise Capacity Hearing services. Approval of Novation Contract #24-238-40 will replace the automatic extension under the prior Contract and allows the Contractor to continue providing services, through June 30, 2006. This Contract includes asix-month automatic extension through December 31, 2006, in an amount not to exceed$487,500. FISCAL IMPACT: This Contract is 50% Federal Medi-Cal Revenue, 45% State EPSDT, and 5% Mental Health Realignment funded. CHILDREN'S IMPACT STATEMENT: This EPSDT program supports the following Board of Supervisors' community outcomes: "Children Ready For and Succeeding in School"; "Families that are Safe, Stable, and Nurturing"; and "Communities that are Safe and Provide a High Quality of Life for Children and Families'", Expected program outcomes include an increase in positive social and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS) and a decrease in out-of-home placements as measured by mental health and social services statistics. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): This Contract meets the social needs of County's population in that it provides mental health services in West County to keep clients with their families in the community and out of higher levels of care. On November 30, 2004, the Board of Supervisors approved Novation Contract #24-308-3 0 (as amended by Contract Amendment Agreement #24-308-3 1) with Early Childhood Mental Health Program for the period from July 1, 2004 through June 30, 2005, (which included asix-month automatic extension through December 31, 2005) for the provision of specialized mental health services to children and their families. Approval of Novation Contract #24-308-32 replaces the automatic extension under the prior Contract and allows the Contractor to continue providing services through June 3.0,2006.
