RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna)X igand) to execute on
behalf of the County, Contract #74-225-94 with Cutina Maddox, MFT, aself-employed individual,
in an amount not to exceed $50,000, to provide Medi-Cal mental health specialty services, for the
period from December 1,2005 through June 30,2007.
FISCAL IMPACT:
This Contract is funded by 49% State and 51% Federal FFP Medi-Cal Funds.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On January 14, 1997, the Board of Supervisors adopted Resolution#97/17, authorizing the Health
Services Director to contract with the State Department of Mental Health to assume responsibility
for Medi-Cal mental health specialty services. Responsibility for outpatient mental health
specialty services involves contracts with individual, group and organizational providers to
deliver these services.
Under Contract #74-225-94, the Contractor will provide Medi-Cal mental health specialty services
through June 30, 2007.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee(Donna Wigand)to execute
on behalf of the County, Contract Amendment Agreement#74-253-2 with Nancy J. Frank, aself-
employed individual, effective November 15, 2005, to amend Contract#74-253 (as amended by
Amendment Agreement#74-253-1)to increase the Payment Limit by$39,900 from $49,500 to a
new total Payment Limit of$89,400 with no change in the original term of the Contract from May
1, 2005 through April 30, 2006.
FISCAL IMPACT:
This Contract is 100% funded by State Prop 63 Funds.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
In May 2005,the County Administrator approved and the Purchasing Services Manager executed
Contract#74-2 5 3 (as amended by#74-2 5 3-1)with Nancy.J- Frank to provide program
development services for the Department's implementation of the Mental Health Services Act
(Prop 63), for the period from May I, 2005 through April 30, 2006.
Approval of Contract Amendment Agreement#74-253-2 will allow the Contractor to provide
additional hours of planning and program development services for Prop 63,through April 30,,
2006.

Young Men's Christian Association of the East Bay,Inc.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on behalf of
the County, Novation Contract #24-315-36 with Young Men's Christian Association of the East Bay, Inc., a
non-profit corporation, in an amount not to exceed $300,897, to provide on-school-site counseling services for
emotional and behavioral disturbed students in selected schools in West County for the period from July 1,
2005 through June 3 0, 2006. This Contract includes asix-month automatic extension through December 3 1,
2006.,in an amount not to exceed$150,449.
FISCAL IMPACT:
This Contract is funded by 44% Federal Medi-Cal (FFP), 44% State EPSDT, and 12% Mental Health
Realignment.
CHILDREN'S IMPACT STATEMENT:
This Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) Program supports the following Board
of Supervisors' community outcomes: "Children ready for and succeeding in school"; "Families that are safe,
stable, and nurturing"; and "Communities that are safe and provide a high quality of life for children and
families". Expected program outcomes include an increase in positive social and emotional development as
measured by the Child and Adolescent Functional Assessment Scale(CAFAS).
BACKGROUND/REASON(S)FOR RECOMIVIENDATION(S):
This Contract meets the social needs of County's population in that it provides counseling and therapy services
for emotionally and behavioral disturbed students in selected junior high schools in the West Contra Costa
Unified School District that improves their high school attendance records and decreases the use of acute
'Mental Health system of care services.
)n December 14,, 2004,, the Board of Supervisors approved Novation Contract #24-315-35 with Young Men's
Iristian Association of the East Bay, Inc., for the period from July 1, 2004 through June 30, 2005 (which

County, Novation Contract #24-409 22 with West County Youth Services Bureau, anon-profit corporation, in
an amount not to exceed $530,738, to provide wraparound services to Severely Emotionally Disturbed (SED)
children, for the period from July 1, 2005 through June 30,2006. This Contract includes asix-month automatic
extension through December 31,2006,in an amount not to exceed$265,369.
FISCAL IMPACT:
This Contract is funded 50% by Federal Medi-Cal,, 49% State Early and Periodic, Screening, Diagnostic and
Treatment Program(EPSDT), and I%Mental Health Realignment.
CHILDREN'S IMPACT STATEMENT:
This EPSDT program supports the following Board of Supervisors community outcomes: Children ready for
and succeeding in school; Families that are safe, stable, nurturing; and Communities that are safe and provide a
high quality of life for children and families. Expected outcomes include all goals identified by Children's
Statewide System of Care guidelines including an increase in positive social and emotional development'
as
measured by the Child and Adolescent Functional Assessment Scale (CAFAS) and decreased use of acute care
system.

This Contract meets the social needs of County's population in that it provides child-family team facilitators and

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on behalf of
the County, Novation Contract #24^705-50 with We Care Services for Children (formerly We Care Society,
Inc.), anon-profit corporation, in an amount not to exceed $693,200, to provide day treatment services for
high risk, delayed or emotionally disturbed children for the period from July 1, 2005 through June 30, 2006.
This Contract includes asix-month automatic extension through December 31, 2006, in an amount not to
exceed$346,600.
FISCAL IMPACT:
This Contract is 50%Federal Medi-Cal,,48% State EPSDT,, and 2%Mental Health Realignment funded.
CHILDREN'S IMPACT STATEMENT:

preschoolers with measurable delays in interpersonal, social/emotional, language and cognitive
development or children who are at risk for such delays, including abused, developmentally delayed,
emotionally disturbed and environmentally .deprived children who do not meet the criteria for any
categorical funding source for services.
On November 30, 2004,, the Board of Supervisors approved Novation Contract #24-705-49 with We Care
Society, Inc. for the period from July 1, 2004 through June 30, 2005, (which included asix-month automatic
extension through December 31, 2005) for the provision of day treatment services for high risk, delayed or
emotionally disturbed children. The Contractor has changed its name to We Care Services for Children.
Approval of Novation Contract #24-705-50 replaces the automatic extension under the prior Contract and
allows the Contractor to continue providing services through June 30,2006.
