RECOMMENDATION:
APPROVE and AUTHORIZE the Animal Services Director, or designee, to execute a contract
amendment to change the name of the contractor for Veterinary Services from Richard
Bachman, DVM dba Northtown Animal Hospital to Richard Bachman, Shelter Medicine
Support, with no change in the payment limit of $1,200,000 and no change in the term of
January 1, 2004 through December 31, 2007.
FISCAL IMPACT
No impact to the general fund.
BACKGROUND
Contract amendment changes the name of the contractor for veterinary services from Richard
Bachman, DVM dba Northtown Animal Hospital to Richard Bachman, Shelter Medicine
Support, sole proprietor.

with Public Health Foundation Enterprises, Inc. (dba PHFE Management Solutions)
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD) to
execute on behalf of the County, Contract Amendment/Extension Agreement#22-806-7 with Public
Health Foundation Enterprises, Inc. (dba PHFE Management Solutions), anon-profit corporation,
effective November 1, 2005, to amend Contract #22-806-5 (as amended by Contract Amend-
ment/Extension
Agreement #22-806-6), to increase the Contract payment limit by $460,680, from
$570,144 to a new total of$1,030,824 and extend the Contract term through June 30,2006.
FISCAL IMPACT:
This Contract is funded 70% by a Federal Bioterrorism Grant, 3% by a California Nutrition Grant,
and 27%by budgeted County funds.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On April 20,2004,the Board of Supervisors approved Contract#22-806-5 (as amended by Contract
Amendment/Extension Agreement #22-806-6) for the period from April 1, 2004 through March
3 1, 2006, for the provision of consultation and technical assistance with regard to community health
promotion for the Partnership for Public Health (PPH) Program including organization services for
the West County Health Collaborative, Monument Community Partnership and Bay Point
Community Partnership Programs, leadership and public health advocacy training and participation
with PPH Pro-gram's Action Plans.
Approval of Contract Amendment/Extension Agreement #22-806-7 will allow the Contractor to
provide additional hours of service, and continue providing services through June 3 0,2006.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract #74-108-20(1) with A Better Way, Inc., aNon-profit corporation, in
an amount not to exceed $50,,000, to provide Medi-Cal mental health specialty services, for the
period from December 1,2005 through June 30,2007.
FISCAL IMPACT:
This Contract is funded by 49% State and 51% Federal FFP Medi-Cal Funds.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On January 14, 1997, the Board of Supervisors adopted Resolution#97/17, authorizing the Health
Services Director to contract with the State Department of Mental Health to assume responsibility
for Medi-Cal mental health specialty services.. Responsibility for outpatient mental health
specialty services involves contracts with individual, group and organizational providers to
deliver these services.
Under Contract#74-108-20(1),the Contractor will provide Medi-Cal mental health specialty services
through June 30,2007.

behalf of the County, Contract #74-033-7 with Kimberly Mayer, aself-employed individual, in an
amount not to exceed$100,620, to provide coordination services to the Department's Mental Health
Division with regard to the Mental Health Services Act for Proposition 63 for the period from
January 1,,2006 through December 31,2006.

BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
On December 11, 2004, the Board of Supervisors approved Contract#74-033-5 (as amended by 74-
033-6) with Kimberly Mayer for the period from January 1, 2005 through December 31, 2005, for
professional., administrative and management analysis, consultation and technical assistance to the
Department's Mental Health Division.
Approval of Contract #74-033-7 will allow the Contractor to provide coordination and oversight
services, including planning and implementing internal and public planning processes, providing
staff training and technical assistance to the Mental Health Director with regard to hiring and
supervising County Designated staff and consultants, and responding to inquiries from and ensuring
compliance with the State Department of Mental Health,through December 31,,2006.

SUBJECT: Approval of Novation Contract#74--05 1-11 with Family Stress Center

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on behalf of
the County, Novation Contract #74-051-11 with Family Stress Center, anon-profit corporation, in an amount
not to exceed $750,000, to provide Therapeutic Behavioral Services, (TBS) for children age 0-21 who
participate in the Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Supplemental Specialty
Mental Health Services, for the period from July 1, 2005 through June 30, 2006. This Contract includes asix-
month automatic extension through December 31,2006,in an amount not to exceed$375,000.
FISCAL IMPACT:
This Contract is funded 50%by Federal FFP Medi-Cal funds, 47%by State EPSDT funds, and 3%by Mental
Health Realignment.
CHILDREN'S IMPACT STATEMENT:
This TBS Program supports the following Board of Supervisors community outcomes: "Children ready for and
succeeding in school"; "Families that are safe, stable,nurturing"; and"Communities that are safe and provide a
high quality of life for children and families". Expected outcomes include all goals identified by Children's
Statewide System of Care guidelines including an increase in school attendance as measured by school records,
increase in positive social and emotional development as measured by the Child and Adolescent Functional
Assessment Scale (CAFAS), increase in family satisfaction - as measured by the Parent-Satisfaction Survey,
decreased use of acute care system, and placement at discharge to a lower level of care.
BACKGROUND/REASON(S)FOR R.ECONEMENDATION(S).-
The
State Department of Mental Health has been working in collaboration with the County's Mental Health
Division to establish a mandated program to provide Therapeutic Behavioral Services (TBS) to children ages
0-21 years of age, who participate in the Early and Periodic, Diagnosis, and Treatment (EPSDT) Supplemental
Specialty Mental Health services. This program will provide services to eligible Medi-Cal beneficiaries under
21 years of age who meet the Mental Health Plan medical necessity criteria, who are members of the certified
class and who meet the criteria for needing these services.
On November 2, 20041, the Board of Supervisors approved Novation Contract #74-051-9 (as amended by
Contract Amendment Agreement #74-051-10) with Family Stress Center for the period from July 1, 2004
through June 30, 2005, (which included asix-month automatic extension through December 31, 2005) for
the provision of Therapeutic Behavioral Services.
Approval of Novation Contract #74-U5 1-11 replaces the automatic extension under the prior Contract and
allows the Contractor to continue providing services through June 30,2006.
