Approve and authorize the Health Services Director or his Designee
(Wendel Brunner) , to execute on behalf of the County, Interagency
Agreement #28--641-6 with the Contra Costa County Office of Education
(Superintendent of Schools) , in an amount not to exceed $15, 000, for
the period from July 1, 2004 through June 30, 2005, for the Junior
Health Facilitator Project in West County.
FINANCIAL_ IMPACT:
Approval of this Interagency Agreement will result in $1.5, 000 to
support the Junior Health Facilitator Project through June 30, 2005.
No County funds are required.
BEA&M.S.FOR C IONS/RM&G ►
This project joins Contra Costa Health Services, the Contra Costa
County Office of Education and the Neighborhood Youth Corps in a
cooperative effort to reduce the number of children who grow up in
fatherless homes as the result of teen pregnancies . Under the Junior
Health Facilitator Project, junior health facilitator and Teenage
Program (TAP) health facilitator conduct class presentations,
organize health--related projects and create teen support groups in
West Contra Costa middle and high schools.
Approval of Interagency Agreement #28-641-6 will allow Contra Costa
County Office of Education (Superintendent of Schools) to continue to
pay the County a total of $15, 000, to continue to implement the
Junior Health Facilitator Project, through June 30, 2005.

RECOMMEENDATION{S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD) to
execute on behalf of the County, Interagency Agreement #28-682-3 with Mt. Diablo Unified School
District, to pay County an amount not to exceed$4,000, for the Outreach TB Testing Program for ML
Diablo Unified School District School District (MDUSD) Employees, for the period from October 5,
2004 through June 30, 2005.
FISCAL IMPACT:
Approval of this Interagency Agreement will result in $4,000 to support the Outreach TB Testing
Program. No County funds are required.
BACKGRO T�TD/l MQN(S) FOR RECOMMENDATIMS):
In the past, Mt. Diablo Unified School District employees received vouchers to be used at Public
Health Clinics for TB testing. Employees experienced extremely long waits to redeem their
vouchers for testing and subsequent reading. In an effort to provide better services, Mt. Diablo
Unified School District, Contra Costa Health Services, Public health Clinic Services, and Vista Oaks
Occupational Medicine have coordinated to provide TB testing and readings services at two
MDUSD facilities.
On October 7, 2003, the Board of Supervisors approved Interagency Agreement#28-682-2 with the
Mt. Diablo Unified School District, for the period from September 16, 2003 through June 30, 2004,
to provide the Outreach TB Testing Program for MDUSD Employees.
Approval of Interagency Agreement#28-6823 will allow Mt. Diablo Unified School District to pay the
County a total of$4,000, to continue the Outreach TB Testing for its employees, through June 30,
2005.

Approve and authorize the Health Services Director or his designee (Wendel Brunner, M.U.) to execute on
behalf of the County, Grant Agreement #28727 (1009018) with the Public Health Institute, to pay County
an amount not to exceed $8,332, for the period from September 30, 2004 through September 29, 2005, for
the Youth Empowerment Strategies (YES) Program. In accordance with the term of the agreement, County
agrees to indemnify and hold harmless Contractor from claims, losses, damages, liabilities, costs, expenses
or liability arising out of,or resulting from County's wrongful or negligent conduct in the performance of this
Grant Agreement.

This Grant Agreement funded by Public Health Institute will result in an amount not to exceed$8,332 for the
Youth Empowerment Strategies(YES)Project. No County funds are required.

The Youth Empowerment Strategies (YES)Project, the Healthy Neighborhoods Project, and Pubic Health
Outreach, Education, and. Collaboration units are currently working, primarily with youth, adults, schools
groups, and existing communities to engage strategic methods for health advocacy. The YES Project
builds on the youth based community mobilization and advocacy strategies currently being pursued in
Forth Richmond. The goal of the YES Project is to train and guide youth co-facilitators, and youth on
how to conduct a successful neighborhood community forums and how to build and carry out an advocacy
project.
Approval of Grant Agreement#28-727 will allow the County to provide training and guidance to the Public
Health Institute with regard to planning neighborhood forums for impoverished children in Contra Costa
County including, but not limited to two %day training sessions,through September 29,2005.
Three certified copies of the Board Order should be returned to the Contracts and Grants Unit.

RECOMMENDATION(S) :
Approve and authorize the Health Services Director or his designee (Wendel Brunner,
M.D. ) , to accept Notice of Award #28-726, in an amount not to exceed $35,720 from the
Hospital Council of Northern & Central California, for the Hospital Council Report for
2004, for the period from September 1, 2004 through November 30, 2004.
FISCAL IMPACT:
Acceptance of this Award will result in an amount not to exceed $35,720 from the Hospital
Council of Northern & Central California. (No County funds are required)
BACKGROLINDZREASON(S) FOR RECOMt=NDATION t S) :
The County's Public Health Division, in collaboration with The Hospital Council of
Northern & Central California, will conduct a Hospital Council Report for 2004 on
selected health indicators for the following Contra Costa County Hospitals, such as John
Muir Medical Center, Kaiser Permanente-Richmond and Walnut Creek, Mt. Diablo Medical
Center, and Sutter Delta Medical Center. The Public Health's Community Health
Assessment, Planning, and Evaluation (CHAPS) project will be responsible for retrieval of
data from different sources, processing of new data, data analysis, calculation of rates,
production of tables and graphs, report writing, and report formatting.
Three sealed and certified copies of the Board should be returned to the Contracts and
Grants Unit.

Correct the Board Order which was approved by the Board of supervisors on July 13, 2004, (C.87) with
Pittsburg Unified. School District, to reflect the intent of the parties in which the Contractor pay the County
in an amount not to exceed $210,000 instead of $115,000, for the County's provision of professional
mental health intervention services for certain Special Education students, for the period from July 1, 2004
through Juice 30, 2005.
FISCAL IMPACT:
Approval of Agreement will result in a total payment to the County in an amount not to exceed
$210,000. No County match is required.
REASONS FOR RFCC MMENDA11 NI SI 3 CKGROUN '
On July 13, 2004, the Board of Supervisors approved Contract #29-520-3 with Pittsburg Unified School
District, for the period from July 1, 2004 through June 30, 2005 for the provision of professional mental
health intervention services for certain special Education students.
Due to lengthy negotiations, the Interagency Agreement #29-520-3 was renewed using the prior year
fees. The County and Contractor have now finalized negotiations and the Contractor has agreed to pay
the County at a higher monthly rate of$17,500 instead of$9,583.33, for the period from July 1, 2004
through June 30, 2005.
Approval of this Board Order will correct the total amount paid to County from $115,000 to a new total
of$210,040 to reflect the intent of the parties and allow Pittsburg Unified School District to continue
paying the County for the provision of professional mental health intervention services to Agency-
designated severely emotionally disturbed Special Education students, through June 30, 2005.
