RECOMMENDATION
ADOPT Resolution No. 20041 628 for the Fuel Tank Replacement at Fire Station No. 1, 1330
Civic Drive, Walnut Creek and Fire Station No. 2, 2012 Geary Road, Pleasant Hill, accepting the
contract as complete between Glacier Environmental Services, Inc. and Contra Costa County Fire
Protection District.
FINANCIAL IMPACT
There is no impact to the General Fund associated with adoption of the Resolution of Acceptance
and Notice of Completion.
BACKGROUND
On May 11, 2004, the Contra Costa County Fire Protection District entered into a contract with
Glacier Environmental Services, Inc. for the Fuel Tank Replacement at Fire Station No. 1, 1330 Civic
Drive, Walnut Creek and Fire Station No. 2, 2012 Geary Road, Pleasant Hill. The project has been
completed in accordance with the approved plans and specifications.

Notice of Completion of Contract with and NOTICE OF COMPLETION
Glacier Environmental Services, Inc. (C.C. §3086, 3093)
Budget Line Item No. 7300-4701 & 4706 RESOLUTION NO. 004/62

The Board of Supervisors RESOLVES THAT:
The Contra Costa County Fire Protection District on May 11, 2004, contracted with
Glacier Environmental Services, Inc. for the Fuel Tank Replacement at Fire Station
No. 1, 1330 Civic Drive, Walnut Creek and Fire Station No. 2, 2012 Geary Road,
Walnut Creek for the Contra Costa County Fire Protection District, Budget Line Item
No. 7300-4701 & 4706, Authorization No. 0929-WH70IB & WH706B, with CB &
MS of WA, Inc. as surety, for work to be performed on the grounds of the County;
and
The Director of General Services reports that said work has been inspected and
complies with the approved plans and specifications, and recommends its acceptance
as complete as of November 30, 2004.
Therefore, said work is accepted as recommended above, and the Clerk shall file with
the County Recorder a copy of this Resolution and Notice as a Notice of Completion
for said contract.
Time extension to the date of acceptance is granted as the work was delayed due to
unforeseeable causes beyond the control and without the fault or negligence of the
Contractor.
