REcomMLNDATION(S}:
Approve and authorize the Health Services Director, or his designee (Haven Fearn) to execute on
behalf of the County, Novation Contract #14-135--3 with Center Point, Inc., a non-profit
corporation, in an amount not to exceed $111,580 to provide substance abuse intervention and
treatment services,for the period from July 1,2004 through June 30, 2005.
FISCAL IMPACT:
This Contract is funded 100% by State Substance Abuse and Crime Prevention Act (SACPA) -
Prop 36 funds. No County funds are required.
BACKGROUND/REAs- N( )Foo RECOMMENDATION(S):
The Substance Abuse and Crime Prevention Act of 2000 (Prop 36) has mandated and funded
Counties to establish Alcohol and Other Drug (AOD) treatment programs for non-violent drug
offenders in lieu of incarceration.
On February 6, 2001, the Board of Supervisors designated County's Health Services Department,
through its Alcohol and Other Drugs Services Division, as the lead agency in Contra Costa
County responsible for the implementation of Prop 36 treatment efforts.
On December 2, 2003, the Board of Supervisors approved Novation Contract #14-135--2 Frith
Center Point, Inc. for the period from July 1, 2003 through June 30, 2004, (which included a six-
month automatic extension through December 31, 2004) for the provision of substance abuse
intervention and treatment services.
Approval of Novation Contract#74-135-3 replaces the automatic extension under the prior contract
and allows the Contractor to continue providing services through June 30,2005.

RmOMMENDATION{S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County,Novation Contract#24-360-26 with Mental Health Consumer Concerns, Inc.,
a non-profit corporation, in an amount not to exceed $323,526, to provide support services and
training programs for County's Mental Health clients for the period from July 1, 2004 through June
30, 2005. This Contract includes a six-month automatic extension through December 31, 2005, in
an amount not to exceed$161,763.
FISCAL IMPACT:
This Contract is funded 100%by Mental Health Realignment.
BACKGROUND/REASON{S)FOR RECOMMENDATION(S):
This Contract meets the social needs of County's population in that it provides self-help and peer
support programs for County's mental health clients, operation of Community Centers in Pittsburg,
Concord, and Richmond to provide social and recreational activities; and organizes and conducts
County's annual Mental Health Service Provider Individualized Recovery Intensive Training
(SPIRIT).
On December 9, 2003, the Board of Supervisors approved Novation Contract #24-360-25 with
Mental Health Consumer Concerns, Inc. for the period from July 1, 2003 through June 30, 2004,
(which included a six-month automatic extension through December 31, 2004) for the provision of
support services for County's Mental Health clients.
Approval of Novation Contract #24-360-26 replaces the automatic extension under the prior
Contract and allows the Contractor to continue providing services through June 30,2005.

BACKGROUND/REASON(S)FOR.RECOMMENDATION(S):
This Contract meets the social needs of County's population in that it provides ongoing operational
funding for mental health homeless outreach facilities in West,Central, and East County
On July 23, 2002, the Beard of Supervisors approved Novation Contract #24-385-28 With Phoenix
Programs, Inc, for the period from July 1,20103 through June 30,2004, (which included a six-month
automatic extension through December 31, 20104) for the provision of mental health outreach
services for the homeless mentally ill.
Approval of Novation Contract #24-385-29 replaces the automatic extension under the prior
Contract and allows the Contractor to continue providing services through June 30,2005.

December 31, 2004) for the provision of Subacute skilled nursing care services for County's
Seriously and Persistently Mentally III (SMPI) and Neurobehavioral clients.

Approve and authorize the Health Services Director, or his designee (Jeff
Smith, M. D. ) , to execute on behalf of the County, Contract #26-398-4 (V261Sd
0426) with the U.S . Department of Veterans Affairs Northern California Health
Care System (VANCHCS) , in an amount not to exceed $43, 249 for Fiscal Year
2004-2005 to provide Dermatology services, for the period from October 1,
2004 through September 30, 2005.
In accordance with the term of the agreement, County agrees to indemnify and
hold harmless Contractor from claims, losses, damages, liabilities, costs,
expenses or obligations arising out of, or resulting from CCRMC' s wrongful or
negligent conduct in the performance of this Contract.
The Agreement is funded in the Health Services Department' s Enterprise I
Budget. Under the term of the agreement, VANCHCS is paid a fee for service
in accordance with the fee schedule which is incorporated in the Contract.
The services provided for the County' s patients under this Contract are
billable to patients and third party payors .

For many years the County and U. S. Department of Veterans Affairs has
maintained a mutual sharing agreement, which has made available to the County
specialized medical services not otherwise available due to lack of
resources, equipment, and personnel. These services included specialized
laboratory testing, radiology services, nuclear medicine studies, CT scans,
MRI ' s, dermatology, gastroenterology, urology, audiology and speech, and
ophthalmology services. The County provided Emergency Room treatment and
inpatient care, including certain ancillary services for VA--referred
patients.
Approval of Contract #26--398-4, will allow Contractor to continue to provide
specialized Dermatology services to County' s patients referred for treatment
by Contra Costa Regional Medical Center physicians through September 30,
2005.
The Contract documents have always been prepared by the Veterans
Administration.
