CHILDREN'S.IMPACT STATEMENT•
The Project will support three of the five community outcomes established in the Children's Report Card,
2) "Children and Youth Healthy and Preparing for Productive Adulthood", 3) "Families that are Economically
Self Sufficient", 5)"Communities that are Safe and Provide a High Quality of Life for Children and Families"
by providing interactive youth focused training to child social workers to understand the needs of foster youth
and families as they move toward self-sufficiency.

The Y.O.U.T.H. Training Project will address the problem of foster youth who are beginning to transition out of
foster care by training 400 child welfare workers using an innovative, interactive youth focused curriculum and
recruiting and developing teams of current and foster youth ages 16—22 years to deliver youth lead trainings.
Using a youth development perspective and team building techniques, these youth will develop the skills and be
empowered to deliver a training focused on changing social worker knowledge, skills and values concerning
communication with foster youth. The Project will provide child welfare staff development and training
services to improve the quality and outcome of services to children and families in the child welfare system.
The Employment and Human Services Department, as an active leader in the Bay Area Social Services
Consortium,will act as the lead agency for the project, receiving and distributing funding and will provide
leadership to Child Welfare Redesign Cohort One counties in developing this project and ensuring that training
is provided to staff from all Cohort One counties.
The original contract amount was erroneously calculated and the new amount-is basad upon project costs and
funding source amounts.

RECON MENDATION(S):
Approve and authorize the Health Services Director,or his designee(Jeff Smith,M.D.),to execute on behalf of
the County, Contract Amendment Agreement #26.203.22 with Nor-Cal Medical Temps, a corporation,
effective November 1, 2004, to amend Contract #26.203.21., to increase the Contract Payment Limit by
$60,000 from$260,000 to a new total Payment Limit of$320,000,with no change in the original term of the
Contract through March 31, 2005.
FISCAL IMPACT:
This Contract Amendment is included in the Health Services Department's Enterprise 1 budget.
BACKGROUND/REASON(S) FOR.RECOMI MENDATION(S):
On March 9, 2004,the Board of Supervisors approved Contract#26.203-21 with Nor-Cal Medical Temps,for
the period from April 1, 2004 through March 31, 2005,for temporary help during vacations,sick leave,and
temporary absences of County pharmacists and technicians, as well as during periods when permanent
positions were vacant.
At the time of negotiations, the payment limit was based upon target levels of utilization. However, due to
staff turnover and the national pharmacist shortage, the utilization during the term of the agreement was
actually higher than originally anticipated..
Approval of Contract Amendment Agreement #26.203.22 will allow the Contractor to provide additional
pharmacy and technicians temporary help services, through March 31, 2005.

Approve and authorize the Health Services Director, or his designee, (Jeff Smith, M.D.) to execute on
behalf of the County, Contract Amendment Agreement #26-115-9 with Martha E. Corcoran, M.D., a
self-employed individual, (Specialty: Otolaryngology), effective November 1, 2004, to amend Contract
#26-915-8, to increase the Contract Payment Limit by $55,000 to a new total Payment Limit of
$490,000 with no change in the original term of the Contract through September 30, 2006.
FISCAL IMPACT:
This Amendment is funded 104% by Enterprise I Trust fund. Cost to the County depends upon
utilization. As appropriate,patients and/or third party payors will be billed for services.

For a number of years the County has contracted with Medical and Dental Specialists to provide
specialized professional services which are not otherwise available in its hospital and clinics.
On September 23, 2003, the Board of Supervisors approved Contract #26-115-8 with Martha E.
Corcoran, M.D., for the period from October 1, 2043 through September 30, 2006, to provide
professional Otolaryngology services including consultation, training, on-call and medical and/or
surgical procedures.
Approval of Contract Amendment Agreement #26-115-9 will allow the Contractor to provide an
additional level of services including acting as Otolaryngology Section Leader, through September 30,
2006.

Transportations Study for Contra Costa County, for the period from October 1, 2004 through
September 30, 2005.

Contra Costa County is one of the most heavily industrialized Counties in California, with both
chemical and petroleum industrial facilities. To date there has been no hazmat commodity flow
surveys conducted in this County. Based on information from the U.S. Department of
transportation, this proposed study would enhance the emergency planning capabilities of local and
state agencies by providing data on the types of amounts of hazmat material traveling in and through
the County. This Study is designed specifically for Contra Costa County to address hazardous
materials commodity flow by commercial rail, pipeline and truck shipments, to understand the
potential of accidents and the impact of accidents in Contra Costa County
Under Contract #22912, the California Department of Health Services will review existing railroad
hazmat transportation databases; design a roadside survey for commercial truck transporting
hazardous materials; and develop methods to integrate sensitive receptor locations for schools,
hospitals and other locations, through September 30, 2005.

behalf of the County, Contract Amendment Agreement #23-090-6 with
Pacific Medical Care, Inc. , a corporation, effective December 1,
2004, to amend Novation Contract #23-090-1 (as amended by Contract
Amendment Agreements #23-090-2, #23-090•-3, #23-090-4 and #23-090-
5) , to increase the County' s monthly payment to the Contractor,
with no change in the original term which is automatically renewed
from year to year unless Sooner terminated as provided in the
Contract.

The Contract cost will .increase by $5, 510 monthly, which is, fully
revenue offset through a combination of Medi-Cal, targeted Case
Management, and Health Plan revenues. The Contract is budgeted in
Enterprise Fund I . No County General Funds are utilized.

Pacific Medical Care has provided cost effective and efficient
financial management services for the County' s Health Services
Department since early 1987 . The firms specialized knowledge in
the field, of public health care economics provides the Health
Department with the expertise necessary to deal with the
complexities of health care funding and financing.
on December 6, 1988, the Board of Supervisors approved Novation
Contract #23-090-1 with Pacific Medical Care, Inc. , for the period
from December 1, 1988 through November 30, 1989 (and automatically
renewed each year unless sooner terminated as provided in the
agreement) .
Approval of Contract Amendment Agreement #23-090-6 will increase
the monthly payment, which the County has been paying to the
Contractor under the prior agreement (as amended by , Contract
Amendment Agreements #23-090-2, #23-090-3, #23-090-4 and #23-090--
5) , to allow the Contractor to continue the provision of
technical/administrative services.
