BACKGROUND/REASON(S) FOR RECO),=NDATION(S) : On December 18, 2001, the Board of Supervisors approved Contract #26- 360-3 (as amended by Contract Amendment Agreements #26--360-4, #26-360-5 and #26-360--6) with DELIVEREX, for the period from November 1, 2001 through October 31, 2004, to provide off-site medical records storage and retrieval services for Contra Costa Regional Medical Center (CCRMC) and various Divisions within the Health Services Department . Approval of Contract Amendment/Extension Agreement #26-360-7 will allow the Contractor to provide additional services through April 30, 2005, as requested by the Department, due to a continuous increase in utilization during the term of the agreement, which was actually higher than originally anticipated. Funding for this Contract is included in the Department's Enterprise I Budget. On April 20, 2004, the Board of Supervisors approved Contract #26.362-4 (as amended by Contract Amendment Agreement #26-362-5) with Dobri Kiprov, M.D., Inc. (formerly Bay Area Mobile Apheresis Program), for provision of therapeutic apheresis services, for the period from April 1, 2004 through March 31, 2405. At the time Contract #26.362.4 (as amended by Contract Amendment Agreement #26.362-5) was negotiated, the payment limit was based upon targeted levels of utilization. However, the utilization of services continues to exceed the Department's projections. BACKGROUND/REASONS) FOR RECOMMENDATION(S): For many years the County and U.S. Department of Veterans Affairs has maintained a mutual sharing agreement,which has made available to the County specialized medical services not otherwise available due to lack of resources, equipment, and personnel. These services included specialized laboratory testing, radiology services, nuclear medicine studies, CT scans, MRI's, dermatology, gastroenterology, urology, audiology and speech,and ophthalmology services. The County provided Emergency Room treatment and inpatient care, including certain ancillary services, for VA-referred patients. Under the terms of this Sharing Agreement #26424-6, VANCHCS will continue to provide Oncology services,including physician services with 24 hours of on-call coverage,Tumor Board,and clinic support,to County's patients at the VA Outpatient Clinic in Martinez, for the period from October 1, 2004 through September 30, 2005. BACKGROUND/REASON(S) FOR R.ECOMt► MATION(S) : In February 2003, the County Administrator approved and the Purchasing Services Manager executed Contract #26-464-2 with Angelica Matamoros, to assist in the coordination of the promotora (Promote) program for the Bay Point Family Health Center, to train community residents to become community health educators to increase access to health education or Spanish-speaking residents for the period from January 4, 2004 through January 31, 2005. The Department has received more grant funding from private organizations, therefore allowing a greater expansion of the promotora program. Contract extension will be paid for with stormwater utility assessment revenue from the unincorporated areas (Fund 2517) designated to the County's Watershed Program. Iii. Reasons for Recommendations and Background: Routine street sweeping helps satisfy the goal of the County's Watershed Program to reduce,eliminate,and prevent stormwater pollution and is mandated by the California Regional Water Quality Control Boards through their joint Municipal Stormwater National Pollutant Discharge Elimination System Permit with the County and cities of Contra Costa. This activity removes sediment, debris, and other contaminants that might normally enter the storm drain system and flow untreated to creeks and other natural waterways. The one-year Central County contract expires on October 31, 2004. It includes an option to extend the contract for one year by mutual agreement of the contractor and the County.In our current climate of budget cuts and increased regulatory requirements,the Watershed Program is attempting to reduce costs while still satisfying our permit requirements.Amending the contract to a sweeping schedule of once per month instead of twice per month should meet the California Regional Water Quality Control Board's minimum requirement. The contractor has agreed to the extension at the current rate of pay per curb mile and the reduced frequency of sweeping. These changes have the potential to save a considerable amount of stormwater utility assessment revenue,since putting the contract out to bid may result in raised rates caused by increased fuel,insurance and sanitary landfill tipping costs currently being experienced by contractors. Amending the contract to include sweeping the curbed section of Bixler Road in Discovery Bay will meet the California Regional Water Quality Control Board's minimum requirement that all curbed streets be swept. Consequences of Negative Action: No routine street sweeping will be provided to Central County until the contract can be put out to bid.
