SUBJECT: Approval of Contract Amendment Agreement#24--97$-7 with Susan Waters
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract Amendment Agreement #24-978-7 with Susan Waters, a self-
employed
elfemployed individual, effective September 21, 2004, to amend Contract #24.978-6, to decrease the
Contract Payment Limit by $1,300 from $51,354 to a new total of$50,054 with no change in the
original team,through June 30,2005.
FISCAL IMPACT:
This Contract is funded 12.5% by Federal FFP, 12.5% State EPSDT and 75% State AB3015 Grant
funds.
This Contract Amendment Agreement reduction was included in the Department's Phase 2 Fiscal
Year 2004-2005 Budget Reduction Plan, approved by the Board on September 21,2004.
BACKGROUND/REASON(S)FOR RECOMMENDATION{:
On June 8, 2004, the Board of Supervisors approved Contract#24-978-6 with Susan Waters for the
period from July 1, 2004 through June 30, 2005, to provide Mental Health Intensive Case
Management for children and their families.
Approval of Contract Amendment Agreement #24-978-7 will decrease the Contract Payment Limit
and allow the Contractor to continue providing services, through June 30,2005.

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION():
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract Amendment Agreement #74-076--7 with Maricela Cisneros, a self-
employed individual, effective September 21, 2004, to amend Contract #74-076-6, to decrease the
Contract payment limit by $1,000, from $40,006 to a new total of$39,006 with no change in the
Contract term,through June 30, 2005.
FISCAL IMPACT:
This Contract is funded 75% by Federal SAMHSA Grant, 12.5% Federal FFP and 12.5% State
EPSDT.
This Contract was included in the Department's Phase 2 Fiscal Year 2004-2005 Budget Reduction
Plan, approved by the Board on September 21, 2004.
BACKGROUND/REASON(S)FOR.RECOMMENDATION(S):
On June $, 2004, the Board of Supervisors approved Contract #74-076-6 with Maricela Cisneros,
for the period from July 1, 2004 through June 30, 2045, for the provision of consultation and
technical assistance to the Division as a Family Partner Wrap-Around Team Facilitator, including
parent support services, training of County-selected staff, liaison services, increasing parent
involvement in planning, service design and evaluation of Children Programs, and gathering
information with regard to community resources.
Approval of Contract Amendment Agreement#74-076-7 will decrease the Contract Payment Limit
and allow the Contractor to continue providing services through June 30,2445.

BACKGROUND/REASON(S)FOR.RECOMMENDATION(S):
On June 8, 2004, the Board of Supervisors approved Contract #74-104-5 with Elizabeth Lucas for
the period from July 1, 2004 through June 30, 2005, to provide consultation and technical assistance
with regard to coordination of family involvement for the Children's Mental Health Program.
