FISCAL IMPACT: This Contract is funded 12.5% by Federal FFP, 12.5% State EPSDT and 75% State AB3015 Grant funds. This Contract Amendment Agreement reduction was included in the Department's Phase 2 Fiscal Year 2004-2005 Budget Reduction Plan, approved by the Board on September 21,2004. BACKGROUND/REASON(S)FOR RECOMMENDATION{: On June 8, 2004, the Board of Supervisors approved Contract#24-978-6 with Susan Waters for the period from July 1, 2004 through June 30, 2005, to provide Mental Health Intensive Case Management for children and their families. Approval of Contract Amendment Agreement #24-978-7 will decrease the Contract Payment Limit and allow the Contractor to continue providing services, through June 30,2005. BACKGROUND/REASON(S)FOR.RECOMMENDATION(S): On June $, 2004, the Board of Supervisors approved Contract #74-076-6 with Maricela Cisneros, for the period from July 1, 2004 through June 30, 2045, for the provision of consultation and technical assistance to the Division as a Family Partner Wrap-Around Team Facilitator, including parent support services, training of County-selected staff, liaison services, increasing parent involvement in planning, service design and evaluation of Children Programs, and gathering information with regard to community resources. Approval of Contract Amendment Agreement#74-076-7 will decrease the Contract Payment Limit and allow the Contractor to continue providing services through June 30,2445.
