SUBJECT: Issue a Public Health Emergency due to the Influenza Vaccine Shortage
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMMENDATIONS:
• ACCEPT the Report from the Health Services Department on Influenza Vaccine Shortage.
• ADOPT the Resolution Declaring a State and Local Emergency.
• AUTHORIZE the Chair of the Board of Supervisors, or his designee, William Walker, MD, Health
Services Director and Health Officer, to declare a Public Health Emergency.
• DIRECT all Health Care Providers in Contra Costa County to Limit Influenza Vaccination to
Persons in High Risk Categories. Health care providers should refrain from vaccinating healthy
individuals not at high risk of serious illness or death from influenza.
• DIRECT the Health Services Department to work with health providers and the community to
ensure the limited supply of influenza vaccine is utilized to protect the most high risk individuals in
ContraCosta.
FISCAL IMPACT:
None.
BACKGROUND:
The declaration of a Public Health Emergency is necessary due to the extreme shortage of influenza
vaccine in the country and in Contra Costa County.
Chiron Corporation, which produces approximately half of the flu vaccine for the United States is unable to
provide the 44 million doses of flu vaccine they were contracted to provide for distribution in the United
States. Contra Costa County Public Health will not receive any flu vaccine and has had to cancel all
Senior Drop-in flu clinics in the county.
The flu vaccine produced by Aventis-Pasteur, about 54 million doses is available in the United States.
This amount represents about one half of the doses needed for the flu season 2004-20305.
This declaration is necessary to ensure that the very limited amount of flu vaccine available is provided to
the highest risk individuals.

On this day the Board received background information from Dr.
Walker to declare a Public Health Emergency due to the extreme
shortage of influenza vaccine in the country and in Contra Costa
County.
Supervisor Gioia questioned the Health Services Department on
the efforts they would make to distribute vaccinations, and the
current status of vaccinations available to the private medical
community and to public agencies that serve low-income families.
Supervisor Glover requested the Health Services Department to
ensure that the limitations on the vaccine and the County's
priorities on who may receive the vaccine are communicated
effectively to Senior Centers and Child Care Providers.
After further discussions, the .Board made the following
recommendations:
• ACCEPTED the report from the Health Services Department
on Influenza Vaccine Shortage;
• ADOPTED Resolution No. 570 Declaring a State and Local
Emergency;
• AUTHORIZED Dr. William Walker, M.D., Health Services
Director and Health Officer, to declare a Public Health
Emergency;
• DIRECTED all Health Care Providers in Contra Costa
County to limit Influenza. Vaccination to persons in high-risk.
categories.
• DIRECTED Health Care Providers to refrain from
vaccinating healthy individuals not at high risk of serious
illness or death from influenza;
• DIRECTED the Health Services Department to work with
health care providers and the community to ensure the
limited supply of influenza vaccine is utilized to protect the
most high-risk individuals in Contra Costa.
Supervisor Gioia made the motion to approve the
recommendations, and Supervisor Uilkema seconded the motion.
The recommendations were approved by unanimous decision.
RESOLUTION NO. 2004/570
Contra Costa County OCTOBER 19 , 2004
Resolution Declaring a State of Local Emergency in
Contra Costa County Resulting from Influenza Vaccine Shortage
WHEREAS, approximately one half of the expected supply of influenza vaccine for the
United States will not be available for the current influenza season; and
WHEREAS, Contra Costa County Health Services received most of its supply of
influenza vaccine from the California Department of Health Services; and
WHEREAS, California does not have any State purchased adult vaccine available to
distribute of health jurisdictions; and
WHEREAS, in Contra Costa County persons at highest risk for complications of
influenza include approximately 100,000 adults over 65 years of age, children ages 6 to
23 months, and infants under 6 months of age who are too young to receive influenza
vaccine; and;
WHEREAS, in the United States, more than 36, 000 people die annually of
complications related to influenza, approximately 100 of these in Contra Costa; and
WHEREAS, the California State Public Health Officer, Dr. Richard Jackson issued a
Public Health Order on October 8, 2004, directing all healthcare providers in California
to limit provision of flu vaccine only to persons in high-risk categories and to refrain
from immunizing health person; and
WHEREAS, there exists a critical local public health crisis in that there is extreme peril
to the health and safety of vulnerable populations including the medically fragile, the
elderly and young children caused by the influenza vaccine shortage; and
WHEREAS, immediate action is needed to protect the limited supply of vaccine and
ensure that vaccine is provided to the most at risk individuals;
NOW,THEREFORE, BE IT RESOLVED that the Contra Costa County Board of
Supervisors hereby proclaims the existence of a state of local emergency within the
county due to the shortage of influenza vaccine

ORDER TO CONTROL INFLUENZA VACCINATION
IT IS HEREBY ORDERED BY THE STATE PUBLIC HEALTH OFFICER, pursuant to sections 100180 and
120140 of the California Health and Safety Cade THAT ALL.HEALTH CARE PROVIDERS IN CALIFORNIA
SHALL LIMIT INFLUENZA VACCINATION TO PERSONS IN HIGH RISK CATEGORIES. Health care
providers shall refrain from vaccinating healthy individualsnot at high risk of serious illness or death from
influenza.
For purposes of this order:
"Health care provider"means any person authorized under the laws of California to administer influenza
vaccinations.
'High risk categories"means:
(1) all children aged 6--23 months;
(2) adults aged 65 years and older;
(3) persons aged 2-64 years with underlying chronic medical conditions;
(4) all women who will be pregnant during the influenza season;
(5) residents of nursing homes and long-term care facilities;
(6) children aged 6 months-18 years on chronic aspirin therapy;
(7) health care workers involved in direct patient rare; and
(8) out-of-home taregtvefs and household contacts of children aged <6 months.
This order is issued due to a shortage of vaccine within the State of California. Chiron Corporation has notified
the Centers for Disease Control and Prevention that none of its influenza vaccine will be available for distribution
in the United States for the 2004-05 influenza season. This development will reduce by approximately one half
the expected supply of influenza vaccine available in the United States for the 2004-05 influenza season.
It is further ordered that health care providers and others that possess influenza vaccine cooperate with local
health officers to assess vaccine supply and coordinate vaccination of persons in high risk categories.
It is further ordered that local health officers provide: this Order to Control Influenza Vaccination to health care
providers in their jurisdiction, and that this order be prominently posted in locations where influenza vaccination
is administered.
This order shall take effect immediately.
