Belden-Burk, a self-employed individual, effective October 1, 2004, to amend Contract #23-344-1, to increase the total Contract Payment Limit by $12,675 from$49,672 to a new total payment limit of$62,347 with no change in the original term of the contract through January 31,2005. FISCAL IMPACT: This Contract is funded 100%by{CSA-EMI}Measure H Funds. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): On January 6, 2004, the Board of Supervisors approved Contract #23-344-1 with Ruth C. Belden- Burk for the period from February 1, 2004 through January 31, 2005, for the provision of consultation and technical assistance to County's Emergency Medical Services for the First Responder Quality Improvement Project. FtscAL IMPACT: This Contract is handed by 90.3% SAPT (Substance Abuse Prevention & Treatment) and 9.7% AGAP(Alternative to General Assistant Program). BACKGRQ?l ND/RF,AsoN(s)Foo RwomMENDATmf sl: This Contract meets the social needs of County's population in that addicted residents, 18 years of age and older, are provided the opportunity to recover from the effects of alcohol and other drug use, become self sufficient, and to contribute to their families and communities as clean and sober individuals. On December 9, 2003, the Board of Supervisors approved Novation Contract #24-723-56 with Neighborhood House of North Richmond for the period from July 1, 2003 through June 30, 2004, (which included a six-month automatic extension through December 31, 2004) for the provision of substance abuse detoxification and residential treatment program services. FISCAL IMPACT: This Contract is funded 100% by a Federal "Safe and Drug Free Schools and Communities" Grant Award. Approval of this Contract will result in a 12-month payment limit over$25,000; therefore, Board of Supervisors approval is required. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): In Tune 2004, the County Administrator approved, and the Purchasing Services Manager executed Contract #24-855-10 with Melinda Moore (dba M. K. Associates) for the period from March 6, 2404 through September 30, 2004, for the provision of grant writing, technical assistance, and research services for the Division's 'Youth, Family and Community System of Care, including evaluation and measurement of outcomes in order to meet the requirements of the "Safe and Drug Free Schools and Communities"grant award. Approval of Contract #24-855-11 will allow the Contractor to continue providing services through June 30,2005. June 30, 2005, for the provision of emergency shelter services. Approval of Contract Amendment/Extension Agreement #25-042-5 will allow the Contractor to provide additional services, including acting as fiscal agent for youth emergency shelters through September 30,2005.
