Access to Care Summit sponsored by Health Services and its community
partners
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
Recommendation:
AUTHORIZE the Health Services Director, or his designee, Wendel Brunner, M.D., to sign a hold
harmless agreement indemnifying Pacheco Community Center located at 5800 Pacheco Blvd.,
Pacheco, California, on January 13, 2004.
Background:
The facility will be used for an all day meeting on Tuesday, January 13, 2004. This Access to Care
Summit will develop a strategic plan for increasing access to health care for families and children in
Contra Costa County.
Fiscal' Impact:
Funds in the amount of$850 to cover the rental fees are being provided by Blue Cross of California and
Kaiser Permanente, two of our partners in the Access to Care coalition.

RESERVATIONS/CANCELLATIONS
1. R.eaffvations for use of the facility may be made by contacting the Pacheco
Community Center at 925-676-8877 or in person at the community Center Office w
5800 Pacheco Blvd.
2. No date is held until a contract is signed and proper fees are paid. Your$100.00
deposit holds your date and is non-refire debte.
3. No rental is taken for more than one(1)year in advance and no rentals are taken on
a cstinual year-to-year basis.
4. Rental fees must be pail iafull no less than 30 days prior to rental. If fees am not
paid 30 days but,the sesertvation will be yii
