APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to EXECUTE a
revenue agreement with the Contra Costa Community College District to claim up to $172,000 of the District's
FY 2.003/2004 CalWORKs State funds allocation for off campus childcare. (#29-074)
EINANC.rAL IMPACT:
No County cost. The agreement allows up to $172,000 of childcare costs incurred by the County to be
reimbursed with CalWORKs funds allocated to the Contra Costa Community College District.
The Contra Costa Community College District receives a State allocation of CalWORKs funds for services to
CalWORKs participants enrolled as students at the Contra Costa Community College District colleges. This
revenue agreement will allow Employment and Human Services to identify and receive reimbursement for up to
$172,000 in off campus childcare costs of the CalWORKs participants enrolled at the District colleges for FY
2003/2004.

amount up to $112,269.
FINANC,IA IMPACT•
Per California Department of Aging Standard Agreement(TV-0304-07), County to receive an increase of$142
Title V, Senior Employment Program Federal funding for FY 2003/2004.
BACKGROUND!
Since 1976 the Area Agency on Aging has been providing services for older residents of the County under the
Older Americans and Older Californians Act(s). The California Department of AgingStandard Agreement,
Title V, Senior Community Services Employment Program funds provide for temporary part-time employment
of low-income individuals age 55 years and over.
The FY 2003/2004 funding increase and budget adjustments are resulting from an increase in State funds for
this program.

County to receive$107,351 California Department of Aging(CDA) additional funds for FY 2003/2004. (CDA.
Agreement#IVIS-0304-27). No County match required.
The MSSP program provides case management on a countywide basis for Contra Costa County residents age 65
and over,Medi-Cal eligible, and certifiable for skilled nursing facility care. Services include case management,
assessment, care planning, service arrangement, and client monitoring through a team of health and social
service providers. The provision of case management services to this senior population allows individuals to
remain safely in their own residences thereby avoiding institutionalization.
The FY 2003/2004 funding increase and budget adjustments are resulting from an increase in state funds for this
program.

Recommendation:
APPROVE and AUTHORIZE the Health Services Director, or designee, to submit a funding
Application in the amount of$25, 856 to the Community Development Block Grant program
for services overcoming language barriers and providing Literacy assistance to clients of the
Bay Point Family Health Center. This grant is for fiscal year 2004-2005. The term is 1 year.
Background:
The Bay Point Family Health Center currently provides 422 non-English speaking and low

RECOMMENDED ACTION
A. Approve the Child Health and Disability Prevention Program
(CHDP) Annual Plan and Budget for FY 2003-2004 (County #29-
338-15) for submission to the State Department of Health
Services; and
B. Authorize the Health Services Director or his designee
(Wendel Brunner) to execute, on behalf of the County, the ,
certification page which certifies compliance with certain
State requirements .
FINANCIAL IMPACT:
This funding is included in the Department ' s current budget . The
funding source breakdown for FY 2003-2004, Child Health Disability
Prevention Program (CHDP) is as follows :

BACKGROMM/REASON(S) FOR RECOMMENDATION(S) :
The Child Health and Disability Prevention (CHDP) Program carries out
State mandates regarding early and periodic screening,' diagnosis and
treatment and case coordination of health and dental services for
children on Medi-Cal or within the 200%' poverty level. These services
are federally required and are consistent with approved standards of
medical practice. . The CHDP Program is responsible for provider
certification, network and resource development, training, outreach,
case coordinating, follow-up and communications with medical and dental
care providers. This program works closely with community providers,
other health related services and facilities, County Department ' s
including Employment and Human Services, Probation and. Community
Services Departments and other Health Services Department ' s Divisions
to provide a wide variety of health related consultation services .
Four sealed/certified copies of this Board Order should be returned to
the Contracts and Grants Unit for submission to the State.
