Approve and authorize the Health Services Director, or his designee (Wendel Brunner, ; to
execute on behalf of the County, Contract 22--401--23 with Pittsburg Preschool Community
Council, Inc,, a non.-profit corpooration, in an amount not to exceed $142,505, to provide support
services for County residents diagnosed with MV disease for the period from March 1, 2004 through
February 28, 2005.
FISCAL I 'ACT:
This Contract is funded by federal funds through the Ryan White CARE Act, "Title I, through an.
inter-governrn ental agreement with Alameda County, who is the Grantee of these funds. No County
fznds are required.
BACKGROU` IREA ON OR I C I AjIQI_ :
This Contract meets the social needs of County's population in that it provides health and support
services to County residents living with IRV to improve arid/or maintain their health status and
promote stabiity in their lives.
On march 4, 2003,the Board of Supervisors approved Contract#22-401-21 (as amended by Contract
Amendment Agreement #22-401--22) with Pittsburg Preschool Community Council, Inc., for the
period from larch 1, 2003 through February 29, 2004, to provide support services for County
residents diagnosed with. HIV disease, including case management services, benefits assurance
counseling, and non-emergency transportation.
Approval of Contract 22-401-23 will allow the Contractor to continue providing services through
February 28, 2005.

BACKGROUND/REASON(S)FOR REC(IIb.IMENDATIQN S :
This Contract meets the social needs of County's papulation in that it provides health and support
service case manager.ent and referrals to County residents living with HIS/AIDS to improve and/or
maintain their health status and promote stability in their lives.
On March 4, 2003, the Board of Supervisors approved Contract #22--403-1 (as amended by
Contract Axnend:nent Agreement #22-403-19) with Desarrollo Familiar, for the period /March 1,
2003 through February 29, 2004, to provide support services to West Contra Costa County residents
with HILI disease.
Approval of Contract #22-403-20 will allow the Contractor to continue providing services through
February 28,2005.

related counseling, advocacy, referral, and placement services to County residents living with

Cr March 4, 2003, the Board of Supervisors approved Contract -611®10 (as amended by
Contract Amendment Agreement#22-611-11.)with New Connections, for the period from March 1,
2003 through February 29, 2004, for the provision of support services to County residents with
HIV disease.
Approval of Contract #22-611-12 will allow the Contractor to continue providing services through
February 25, 2005.

the City of Oakland. No County funds are required,
In 1930, Congress passu.the National Affordable Housing Act,including Housing Opportunities for
People with ASS ( OP A). The City of Oakland acts as the federal grantee. for HOPWA funds
for Alameda and Contra Costa Counties and has entered.-into a contract with.Contra Costa County to
provide supportive services for County residents with HIV/AIDS and their families.

BACKGROUND/REASON(S)FOR ONI}A'.�.ION(5):
In 1990, Congress passed.the National Affordable Housing Act, including Housing Opportunities for
People with AID (HOPWA). The City of Oakland acts as the federal grantee for HOPWA funds
for Alameda and Contra Costa Counties and has entered into a contract with.Contra Costa County to
provide supportive services for County residents with HIV/AIDS and their families.

and/or maintain their health status and promote stability in their lives.
On March 4, 2003, the Board of Supervisors approved. Contract #22-663-5 with Shelter, Inc., to
provide housing advocacy services for people with HIV Disease, for the period ffom March 1, 2003
through February 29,2004.
Approval of Contract #22--663--6 will allow the Contractor to continue providing services through
February 28, 2005.
