execute on behalf of the County, Standard Agreement, #28-602-6 (State #03-75516) with the ,State
Denar€:s ent of Health Services, in an amount not to exceed $1,710,45',', for the "On The Move With
Foodwise"Project,for the period from October 1, 2003 through September 30, 2005.
FISCAL INTACT:
Approval of this agreement will result in an amount not to exceed$1,710,457 of State funding for the
"On The Move With Foodwise"Project. No County funds are required.

Or. March 26, 2002, the Berard of Supervisors approved. Standard Agreement #28-602-5 (State
#01-15;65,6) with. State Department of Health Services, for the period from October 1, 2001 through
September 30, 2€303, to provide nutrition education to Contra Costa County, food assistance
recipients through classes and workshops, as well as through existing community channels, such as
after school programs and food distribution sites. The goal of the project is to improve nutrition and
physical well-being among a segment of Contra Costa County's population, when are at an increased
risk of chronic disease,
Standard Agreement#25-602-6 continues State funding through September 30, 2005.
Three sealed/certified copies of this Board Order should be returned to the Contracts and Grants Unit
for submission to the State.

Approve and authorize the Health Services Director, or his designee

Agreement #29-526 with Contra Costa County Superintendent of Schools
(Office o� Education,) , a public agency, to pay County $1, 167, 570,
.ta ,rovide mandated mental health services to eligible AE3632
students under the lndivmdualized Education Plan. (IEP) Program, for
the period from July 1, 2003 through June 30 , 2004 .
Approval of this Agreement will result in a. total payment to the
County of $1,167, 570 from the Contra Costa County Superintendent of
,Sch dols (Office of Education) during the term of the Agreement . No
County match is reguired.

Under this agreement- t#29-526, the County will provide mental health
services to ellgi le st=udents who are identified under the
Individualized Education Plan (IEP) Program. Services provided
under the IEP program,, in collaboration with the Individuals with
Disabilities Education Act (IDEA) Program, will include the
_c" lowing. Tnd vidua., ccunseying, colateral counseling assessment,
case management, medication services, crises intervention, group
counseling, and day treatment . Approval of Interagency Agreement.
Contra Costa Superintendent of Schools (Office of Education) will
pay County for the provision of professional :rental health services
through June 30 , 2004 .

BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
'his Bay Point Promotora ' s Folic ic Acid and -Nutrition Profect is a
collaboration between the March of Nimes, U. C. Cooperative
Extension., Contra Costa Public health staff, California Depart-went
of Maternal and Child Health, and the Center for Disease Control .
she goal of the project is to increase knowledge of Spanish speaking
women on the benefits of health nutrition and the use of mul.ts-
vit.arm-ins with folic acid to reduce birth defects . Studies has
revealed that a high incidence rate of neural tube birth defects
occur among Hispanic women.

1. RECOMMENDATION:
APPROVE and AUTHORIZE the Sheriff, or designee, to execute a contract on behalf of the
County with E Team, Incorporated, in an amount not to exceed $63,230, to develop and install
software for the Emergency Management System during the period of January 15, 2004
through December 31, 2004,
Il. FINANCIAL IMPACT:
DONE. The cost of this project is funded by the FY03 Governor's Office of Emergency Services
State Homeland Security Grant, Part ll.
III. BACKGROUND:
The Contractor will provide software for an Emergency Management System that will facilitate
communications and coordination with County departments, cities, special districts and industry.
This product will provide the County Office of Emergency Services staff and partner agencies
with a common set of tools they can use to manage both day-to-day and emergency operations
to include tracking of personnel, equipment and resource requests and expenditures during an
activation.

RECOMMENDATIONS:
Approve and authorize the ;District Attorney to amend the contract with.Accounternps for accounting support
for the District Attorney to increase t,-,e payrn�ent limit by 24,000 to $ 53,000 and extend the termination
date to Jude 30. 2004.
FINANCIAL IMPACT.
The cost of the contract extension is covered by savings from vacant budgeted accounting support positions.
REASONS FOR RECOMME D TI NIBA.CKGRC3UND:
Extension of the contract is needed or temporary accounting assistance to allow for adequate tracking,
accounting and claiming of revenue -fico grants and other sources of finding.
CONSEQUENCES OF NEGATIVECTION:
Failure to approve the request could jeopardize the receipt of grant and other revenge budgeted for 2.003/04
in the District Attorney's budget.
