contract with Chabot-Las Positas Community College District(CLPCCD) in the amount of$185,000 to provide
training for group home cage providers in Contra Costa County for the period from February 1,2004 through
December 31, 2004.. (#21.205-0)

$185,000 100%Federal Title 4-E funding. No County Funds.
(The total cost of the program.is$330,557. EHSD will act as a pass-through for funds to CLPCCD in the
amount of $185,000 with reimbursement from federal Title 4-E funds. CLPCCD will provide in-kind matchof
$145,357 to meet the total program costs of$330,557).
CHILDREN'S IMPACT S3:A 'FME T*
The training program will support two of the five community outcomes established in the Children's Report
Card,"2)Children and Youth Healthy and Preparing for Productive Adulthood"and"5)Communities that Safe
and Provide a High Quality of Life and Children and Families"by providing education and training to enable
group home providers to better serve children and youth as they move toward adulthood.
The education and training to be provided to group home caregivers will range from clinical training to topics
such as CPR., first aid, computer skills, and supervisory skills, all in an effort to expand the expertise of those
individuals providing services to children and youth in Contra Costa County group homes.
Chabot-Las Positas Community College District(CLPCCD)has a successful history of providing an array of
training and technical assistance to group homes and foster family agencies, as evidenced by partnerships with.
Alameda and Solano Counties. CLPCCD and EHSD outreach activities may include presentations,personal'
contact, and/or mailings to service providers announcing training information. The contact will result in
capacity-building in the Contra Costa County group home community and between counties with the same or
similar programs.

BACKGROUND!
The Employment and Human Services Department's Information Technology Division provides technical
support to the department. Mr. Navarro will complete the development of the Time Study Project, which was
initiated during his previous contract and will design and develop a revised Contracts Database.
Mr. Navarro was selected from a competitive procurement,Request for Interest (RFI)#i196,July 2003.

RACKGROTAD•
The California Department of Aging(CDA)has sent notification of changes in funding availability for Long'
Term Care Ombudsman services that have resulted in a net decrease of$13,622 in funds available for FY
2003/2004 for this contracted service.
Ombudsman Services of Contra Costa provides long-term care ombudsman services for adults including
mediation and conciliation Services, creation of Family Councils for support of families of long term care
residents,report investigation of physical abuse of all dependent adult and elder residents of long term care
facilities,education and training of senior's rights, benefits, and entitlements. Funding for this portion of the
contract received a 5.5%increase ($9,680)of One-Time-Only(OTO)Title VII A Older Americans Act(OAA)
funds.
Additionally, Ombudsman Services provides required volunteer recruitment,training,and certification for
volunteer placement in skilled nursing homes as specified by the California Department of Aging,which
addresses the Governor's Long Term Care Consumer Protection Initiative. Funding for this portion has been
decreased by 30%($23,302)due to elimination of Medi-Cal Reimbursement.F"' s

This contract directly supports three of the five community outcomes established in the Children's Report Card:
3)"Families that are Economically Self-Sufficient"; 4)"Families that are Safe Stable and Nurturing", and 5)
"Communities that are Safe and Provide a Nigh Quality of Life for Children and Families."
BACKGROUND,
Lao Family Community Development provides pre- and post-employment services and translation and
interpretation services in Vietnamese and various Laotian dialects to limited English proficient CaIWORKs
refugees residing in West Centra Costa County. Services include assisted job search,job placement,
employment disputes resolution for job retention, and other job related information. Eighty-seven percent
(87%)of'RESS funding under this contract is allocated for employment services and will increase the number of
CaIWORKs recipients served from twenty(20)to twenty-seven(27).
Thirteen percent (13%)of RESS funding under this contract is allocated for services promoting healthy
marriages including marriage building or relationship enhancement activities, such as marriage education,
relationship enhancement programs, divorce reduction activities, and/or other activities that promote and sustain
healthy marriages. /

BACKGROUND!
Health Services Department, Public Health Division administers and operates the County Senior Nutrition
Program providing congregate hot meals at twenty-one program sites in Contra Costa County, home delivered
meals, one-way client rides to and from congregate meal sites, and nutrition education to seniors.
The Older Americans Act(OAA) is allocating mid-year additional funding to the nutrition program to provide
additional supportive services that assist seniors to remain independent.

execute on behalf of the County, Contract Amendment Agreement #24-939-48(6) with Jewish
Family and Children's Services of the East Bay, a non-profit corporation, effective March 1, 2004,
to amend Contract #24-939-48(5), to increase the Contract Payment Limit by $25,000, from
$50,000 to a new total payment limit of$75,000.

BACKGROUND/REASON(S) FOR RECOMMENDATION(SI:
On January 14, 1997, the Board of Supervisors adopted Resolution#97/17, authorizing the Health
Services Director to contract with the State Department of Mental Health to assume responsibility
for Medi-Cal mental health specialty services. Responsibility for outpatient mental health specialty
services involves contracts with individual, group and organizational providers to deliver these
services.
On July 8,2003,the Board of Supervisors approved Contract#24-939-48(5)with Jewish Family and
ChildreWs Services of the East Bay, for the period from July 1, 2003 through June 30,2004.
Approval of Contra&Amendment Agreement#24-939-48(6) will allow the Contractor to provide

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
Approve and authorize the Health Services Director, or his designee (Jeff Smith„ M.D.) to execute
on behalf of the County, Contract Amendment Agreement #26-964-3 with V. Arek Keledjian,
M.D., a self-employed individual (Specialty: Internal Medicine), effective March 1, 2404, to
amend Contract#26-964-2, to increase the Contract Payment Limit by $10,000, from. $30,000 to
a new total payment limit of$40,000.
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients and/or third party payors
will be billed for services.
BACKGROUNDIREASON(S) FOR RECOMMENDATION(8,1:
For a number of years the County has contracted with Medical and Dental Specialists to provide
specialized professional services which are not otherwise available in its hospital and clinics.
On May 6, 2403, the Board of Supervisors approved Contract#26-964-2 with V. Arek Keledjian,
M.D., for the period from June 1, 2003 through May 31, 2446, to provide Internal Medicine
services, including on;-call and clinical coverage services for Contra Costa Regional Medical
Center and Contra Costa Health Centers.
Approval of Contract Amendment Agreement #26-964-3 will allow Contractor to provide
additional Internal Medicine services including,but not limited to, therapeutic procedures, through
May 31, 2006.
