Fulford (dba Valley Audiology), a sole- proprietor,in an amount not to exceed$100,000,to provide audiological services to County-referred patients at Contra Costa Regional Medical Center and Health Centers,for the period from January 1, 2005 through December 3,1, 2005. FISCAL IMPACT: This Contract is 100% funded in the Health Services Department Enterprise I Budget. As appropriate, patients and/or third-party payors will be billed for services. BACKGROUND/REASON(S)TOR RECOMMENDATION(S): On December 22, 2003, the Board of Supervisors approved Contract 426-046-28 with Julie O. Fulford(dba Valley Audiology)for the period from January 1,2004 through December 31,2004,for the provision of audiological services to County-referred patients at Contra Costa Regional Medical Center including, but not limited to providing hearing aids and hearing evaluations. REASONS EOR RF+.CUMMEN rA IONS/RStKGROT1 : This Contractor, which specializes in placement of locum tenens physicians, recruits physicians for the County, which.the County then hires to work as temporary employees to provide vacation, sick leave, and extended leave relief for County-employed physicians. Physicians, placed by the Contractor, are required to be members of the County's Medical Staff and to be credentialed through existing medical staff processes. On August 12, 2003, the Board of Supervisors approved Contract #26-251-10 (as amended by Contract Amendment/Extension Agreement #26-251-11) with Interim. Physicians, Inc. dba Western Physicians Registry, for the period from September 1, 2002 through December 31, 2004, for the provision of recruitment services of locum tenens physicians for the Contra Costa Regional Medical Center. RFASONfS11 OR RFC ?.MMEND lTION(,S,)IBA1IND: The County has contracted for many years with registries to provide temporary help services to assist Contra Costa Regional Medical Center and Health Clinics during peak loads, temporary absences, and emergency situations. For example, given the uncertainty of the duration of Worker's compensation/leaves of absence, the use of registries allows the Department to obtain temporary registry help on short notice, and only until the employee is able to return to work. Other usage is to cover nights and weekend shifts in Admissions, which have historically been difficult to fill on a permanent basis, although efforts continue. Lander Contract #26-505 the Contractor will provide temporary admitting, registration, billing, and collection professionals for the Contra Costa Regional Medical Center,through December 31, 2005. Young, M.D., (Specialty: Radiology), a sell-employed individual, effective :December 1, 2004 to modify the Payment Provisions with no change in the original Payment Limit of $300,000 and no change in the term for the period from October 1, 2003 through September 30, 2005 to provide additional radiology services to patients at Contra Costa Regional Medical Center and Contra Costa Health Centers. ` A1,IMPACI: Done. Approval of this Amendment will modify the Payment Provisions to add a rate of$1.0 per each plain film, with no change in the original Payment Limit of$300,004. On October 14, 2003, the Board of Supervisors approved Contract 976-516 with Howard Young, M.D. for provision of radiology services for the Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from October 1,2003 through September 30,2005. It was the intent of the County and the Contractor that Dr. Young be reimbursed for reading plain films during the term of the Contract. However, due to the mutual mistake of the parties, the reimbursement amount was not included in the formal Contract. Approval of this Contract Amendment Agreement #76-516-1 will modify the Payment Provisions to remedy the mutualmistake of the parties so that Dr. Young can be properly reimbursed for reading plain films and allow him to continue providing radiology services,through September 30, 2005.
