This Contract includes a six-month automatic extension through December 31, 2005, in an amount not to exceed $61, 250. FIS9AL ikmh= This Contract is 100° funded by the State CalWORKs through the Employment and Human Services Department. In September 2004, the State Department of Social Services reduced funding for CalWORKs Substance Abuse/Mental Health Services Statewide for fiscal year 2004-2005. This Contract reflects a reduction in the County' s Health Services Department operated services including indirect services with this Community Based Organization. MR REC99ZMMION(1) On December 9, 2003, the Board of Supervisors approved Contract #74-112--4 with Asian Community Mental Health Services to provide mental health services, including individual, group and family collateral counseling, case management, and medication management services for Asian-language speaking CalWORKs participants to reduce barriers to employment, for the period from July 1, 2003 through June 30, 2004 (which included a six--month automatic extension through December 31, 2004) . Approval of Novation Contract #74-112-5 replaces the automatic extension under the prior Contract, and allows Contractor to continue providing services through June 30, 2005. This Contract includes a six-month automatic extension through December 31, 2005, in an amount not to exceed$39,850. FISCAL IMPACT: This Contract' is 100% funded by the State CalWORKs through the Employment and Human Services Department. In September 2004, the State Department of Social Services reduced funding for CaIWORKs Substance Abuse/Mental Health Services Statewide for fiscal year 2004-2005. This Contract reflects a reduction in the County's Health Services',Department operated services including indirect services with this Community Based Organization. BACKGROUNDIREASON(S) FOR RECOMMENDATION(S): In August 1997,the State of California Legislature passed Assembly Bill 1542 which brought major changes to the welfare programs previously operated by the State. Among the changes was a provision which required treatment of substance abuse and mental illnesses of Welfare-to-Work participants, when these conditions interfere with participation in Welfare-to-Work activities. Subsequently, the County's Employment and Human Services Department and Health Services Department signed an Interdepartmental Services Agreement(#21.427)which allowed the Health Services Department to provide substance abuse and mental health services to Welfare-to-Work participants referred by the Employment and Human Services Department. On December 16, 2003,The Board of Supervisors approved Contract#74-115-4 with Desarrollo Familiar,Inc.,for the period from July 1, 2003 through June 30, 2004 (which included a six-month automatic extension through December 31, 2004) for the provision of mental health services, including individual,group and family collateral counseling,case management,and medication management services for Spanish-speaking CalWORKs participants to reduce barriers to employment. This contract includes a six-month automatic extension through December 31, 2005, in an amount not to exceed$117,654. FISCAL IMPACT: This Contract is funded 35% by Federal Medi-Cal (FFP) Revenue, 35% by State EPSDT funds, and 30% by Mental Health Realignment. Funning under this Contract reflects a reduction in County subsidy/realignment :funds and increases required minimum Medi-Cal FFP/EPSDT funds in accordance with the Department's Board approved budget balancing plan effective October 1,2004. CHILDREN'S IMPACT STATEMENT: This Mental Health Program supports the following Board of Supervisors' community outcomes: "Children Ready For and Succeeding in School"; "Families that are Safe, Stable, and Nurturing"; and"Communities that are Safe and Provide a Hight.Quality of Life for Children and Families". :Expected program outcomes include an increase in positive social and emotional development as measured by the Child and Adolescent.Functional Assessment Scale(CAFAS)and placement at discharge at a lower level of care. BACKGRQUND/REASQN S FOR RECQMMENDATIQN S : This Contract meets the social needs of County's population in that it provides Juvenile Court-mandated rehabilitative day treatment, mental health services, and medication support for SED children with emotional and behavioral problems to keep these children in the community and out of State hospitals. On August 12, 2003, the Board of Supervisors approved Contract #74-164-2 with Sunny Hills Children's Garden for the period from July 1, 2003 through June 30, 2004, which included a six-month Automatic Extension through December 31, 2004, for the provision of a Day Treatment program for Seriously Emotionally Disturbed(SED)children. This Contract includes a six-month automatic extension through December 31, 2005 in an amount not to exceed.$228,577. FISCAL IMPACT: This Contract is fended 64% by Federal Medi-Cal and "Spirit of Caring" funds, 33% by State EPSDT, and 3%by Mental Health Realignment(required County Match). Funding under this Contract reflects a reduction in County subsidy/realignment funds and increases required minimum Medi-Cal FFP/EPSDT funds in accordance With the Department's Board approved budget balancing plan effective October 1,2004. CHILDREN'S IMPACT STATEMENT: This Whole Circle System of Care program supports the following Board of Supervisors community outcomes: "Children ready for and succeeding in school"; "Families that are safe, stable, nurturing"; and "Communities that are safe and provide a high quality of life for children and families". Expected outcomes include all goals identified by Children's Statewide System of Care guidelines including an increase in positive social and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS); an increase in family satisfaction - as measured by the Parent Satisfaction Survey; and decreased used of acute care system. BACK,CROUND/REASON(S) FOR RECOMMENDATION(S): This Contract meets the social needs of County's population in that it provides respite and mentoring services to families of children With serious emotional and behavioral disturbances to reduce family stress, support family stability,prevent neglect and abuse,and minimize the need for out-of-home placements.
