SUBJECT: LEASE AGREEMENT RENEWAL BETWEEN THE HOUSING AUTHORITY OF THE
COUNTY OF CONTRA COSTA AND THE HEALTH SERVICES DEPARTMENT OF
CONTRA COSTA COUNTY
& BACKGROUND AND JUSTIFICATION
SPECIFIC REQUEST(S) OR RECOMMENDATIONS)
I. RECOMMENDED ACTION:
Acting as the Board of Supervisors, APPROVE and AUTHORIZE the General Services Director,
and acting as the Board of Commissioners of the Housing Authority of the County of Contra Costa,
AUTHORIZE and APPROVE the Executive Director to exercise the first option, effective January 1, 2005,
to extend by one year the lease agreement for use of unit 602, 1744 4th Street, North Richmond,
California, for the Partners in Health Program at a monthly rate of$450.
11. FINANCIAL IMPACT:
The executed agreement will generate $450.00 per month, payable in advance on the first day of
each month during the term of this Lease to the Housing Authority Public Housing fund.
Ill. REASONS FOR RECOMMENDATION/BACKGROUND
The County of Contra Costa has been using 1744 4th Street, Unit 602, North Richmond California
to operate the "Partners In Health" program for the residents of Las Deltas, as well as the surrounding
community. The center provided counseling and drug treatment for their clients enabling them to become
drug free and put them on the road to self-sufficiency. This program has been operating at this location
since May, 2001. This is the second Lease Agreement and has been in effect since Januaryl, 2004.
There are three (3) 1-year renewals in the agreement. This is the first 1-year renewal to be effective
January 1, 2005 and end December 31, 2005 leaving two (2) 1-year renewals and all are subject to the
Board of Supervisors approval.
IV. CONSEQUENCES OF NEGATIVE ACTION:
Should the Boards elect not to authorize the General Services Director and the Executive Director
to exercise a one-year renewal of the Lease Agreement, the Partners In Health program would no longer
function and this would be a detriment to the development, as well as to the community.

RENEWAL OF LEASE
THIS SUPPLEMENTAL AGREEMENT made and entered into this 1St
day of January , 2005 , by and between the HOUSING AUTHORITY OF
THE COUNTY OF CONTRA COSTA, herinafter referred to as the "Authority",
and Partners In Health, Contra Costa County Health Services Department ,
herinafter to be referred to as the "Agency":
WITNESSETH:
WHEREAS, by lease agreement between the parties hereto, dated
January 1, 2004 ,the Authority leased to Agency the right
and privelege of occupying space in the Las Deltas Development
17444 th Street Unit 602 North Richmond
of the Authority; and
WHEREAS, the Agency desires and the Authority is willing that such
lease be renewed for the term of one (1) year.
NOW, THEREFORE, the Authority for and in consideration of the
promises and covenants in the aforesaid lease, and other valuable consideration,
the receipt of which is hereby acknowledged,hereby agrees that said lease shall
be, and it hereby is, renewed beginning January 1, 2005 thru December 31,
2005 .
The Agency hereby covenants and agrees that during the course of the
renewal period the Agency shall perform and be bound by all of the covenants
and conditions contained in said lease agreement of January 1, 2004 .
IN WITNESS WHEREOF, the parties have set their hands the day and
year first hereinabove set forth.
