SPECIFIC REQUEST($)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand)to execute on behalf of
the County,Novation Contract#24--927---8 with Asian Pacific Psychological Services, a non-profit
corporation, in an amount not to exceed$383,450,to provide school and community based mental health
services and wraparound services for Asian children and their families for the period from July 1, 2004
through June 30,2005. This Contract includes a six-month automatic extension through December 31,2005,
in an amount not to exceed$191,725.
FISCAL IMPACT:
This Contract is funded 44%by Federal FFP Medi-Cal Funds,42%by State EPSUT Funds,and 13%by
Mental Health Realignment.
Funding under this Contract reflects a reduction in County subsidy/realignment funds and increases required
minimum Medi-Cal FFP/EPSDT funds in accordance with the Department's Board approved budget
balancing plan effective October 1, 2004.
CHILDREN'SIMPACT STATEMENT:
This EPSDT Program supports the following Board of Supervisors' community outcomes: "Children Ready
for and Succeeding in School"; "Families that are Safe, Stable, and Nurturing"; and"Communities that are
Safe and Provide a High Quality of Life for Children and Families". Expected program outcomes include an
increase in positive social and emotional development as measured by the Child and Adolescent Functional
Assessment Scale(CAFAS)
.BACKGROUND/REASON(S) FOR RECOMMENDATIONfS):
This Contract.meets the social needs of County's population in that it provides school and community based
mental health services,including: assessments; individual,group and family therapy;medication support;case
management;outreach; and crisis intervention services,to an underserved Asian population and will result in
greater home,community,and school success.
On August 5, 2403,the Board of Supervisors approved Novation Contract#24-927-7 with Asian Pacific
Psychological Services for the period from July 1, 2003 through June 30, 2004, (which included a six-month
automatic extension through December 31, 2004) for the provision of school and community based mental
health services to Asian children and families.
Approval of Novation Contract#24-927-8 replaces the automatic extension under the prior contract and
allows the Contractor to continue providing services through June 30, 2005.

SUBJECT: Approval of Novation Contract#24.928.8 with Fred Finch Youth Center
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATIONS):
Approve and authorize the Health Services Director or his designee(Donna Wigand)to execute on behalf of the
County,Novation Contract#24-928.8 with Fred Finch Youth Center, a non-profit corporation, in an amount
not to exceed $1,167,844, to provide school and community based mental health services for Seriously
Emotionally Disturbed students in the Mt. Diablo Unified School District and their families,for the period from
July 1,2004 through June 30,2005. This Contract includes a six-month automatic extension through December
31, 2005, in an amount not to exceed$583,922.
FISCAL IMPACT:
This Contract is funded 29%by Federal FFP Medi-Cal Funds and 29%by State EPSDT Funds and 42%Mt.
Diablo Unified School District. No County funds are required.
CHILDREN'S IMPACT STATEMENT:
This school-based collaborative program supports the following Board of Supervisors' community outcomes:
"Children Ready for and Succeeding in School", "Families that are Safe, Stable, and Nurturing'; and

through December 31, 2004) for the provision of school based mental health services.
Approval of Novation Contract#24.92&8 replaces the automatic extension under the prior contract and allows
the Contractor to continue providing services through June 30, 2005.

RECOMMENDATION{S):
Approve and authorize the Health Services Director, or his designee (Haven Feam) to execute on
behalf of the County, Contract #24-942-8 with Victor Kogler, a self-employed individual, in an
amount not to exceed. $98,440, to provide consultation, training, and technical assistance to the
Department with regard to the Division's System of Care and Management Information System for
the period from January 1,2005 through December 31,2005.
FISCAL IMPACT:
This Contract is funded 90% by Federal Prevention Set-Aside and SAPT Grant funds, and 10% by
StateFunds.
BA4CK GRQUNDIR.EASON(S)FOR RECOMMENDATION(S):
On January 27, 2004,the Board of Supervisors approved Contract#24-942--7 with Victor Kogler for
the period from January 1, 2044 through December 31, 2004, for the provision of consultation,
training, and technical assistance to the Department with regard to the Division's System of Care
and Management Information System.
Approval of Contract #24-942-8 will allow the Contractor to continue providing services through.
December 31,2005.

BACKGFt.OLTND/REASON{S) FOR ItECOIViMENDATION{S):
For a number of years the County has contracted with Medical,Dental and Mental Health Specialists to provide
specialized professional services, which are not otherwise available in its Hospital and Health Centers.
On March 23, 2004,the Board of Supervisors approved Contract#24.956.2 with Charles Windham,M.D.,to
provide outpatient Psychiatric services including Emergency Psychiatric Services,for the period from January 1,
2004 through December 31, 2004.
Approval of#24.956.3 will allow the Contractor to continue providing services,through December 31, 2005.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD) to
execute on behalf of the County, Contract #25-059 with Resources For Community Development,
a non-profit corporation, in an amount not to exceed $539,068, to provide supportive housing
services for homeless families in Central County, for the period from December 1, 2004 through
June 30, 2005.
FISCAL IMPACT:
This Contract is funded 100% by Federal McKinney-Vento Funds, through a Housing and Urban
Development(HUD)Supportive Housing Program Grant.
CHILDREN'S IMPACT STATEMENT:
This program supports the following Board of Supervisors' community outcome: "Communities
that are Safe and Provide a High (duality of Life for Children and Families". Expected program
outcomes include access to support and educational services for children in homeless families.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
This Contract meets the social needs of County's population in that it provides permanent supportive
housing and case management services for families with special needs. Families residing in the
apartments will receive case management services to help them access job training and mainstream
health and human services and to obtain long-term permanent housing.
Under Contract#25-059, Resources For Community Development will provide housing services for
12 families, Case Management services, and other supportive services for the homeless at the
Lakeside Apartments in Concord,through June 30, 2005.
