SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee(Donna Wigand)to execute on behalf of the County, Contract Amendment Agreement #74--083--11 with La Cheim School, Inc., a nonprofit corporation, effective December 1, 2004, to amend Contract #74-083-10 to increase the total Contract Payment Limit by $140,000 from($200,000 to a new total payment of$340,000 and to increase the six-month automatic extension by$70,000 from$100,000 to$170,000. FISCAL IMPACT: This Contract is funded 50°1 by Federal Financial Participation, 46% by State EPSDT, and 4% Mental Health Realignment Funds. BACKGROUND/REASON(S)FOR RECONIMENUATION(S): The State Department of Mental Health has been working in collaboration with the County's Mental :Health Division to establish a mandated program to provide Therapeutic Behavioral Services(TBS)to children ages 0- 21 years of age, who participate in the Early and Periodic, Diagnosis, and Treatment (EPSDT) Supplemental Specialty Mental Health services. This program will provide services to eligible Medi-Cal beneficiaries under 21 years of age who meet the Mental Health Plan medical necessity criteria, who are members of the certified class and who meet the criteria for needing these services. On November 11, 2004, the Board of Supervisors approved Novation Contract #74-083-10 with La Cheim School, Inc. for the period from July 1, 2004 through June 30, 2005, (which included a six-month automatic extension through December 31, 2005) for the provision of Therapeutic Behavioral Services (TBS) to County referred clients ages 0-21 years of age. Approval of Contract Amendment Agreement t#74-083--11 will allow the Contractor to provide additional units of services through June 30, 2005. This Contract includes a six-month automatic extension through December 31,2005, in an amount not to exceed.$196,399. FISCAL IMPACT: This Contract is funded. 100%by State CaIWORKs funds. In September 2004, the State Department of Social Services reduced funding for CalWORKs Substance Abuse/Mental Health Services Statewide for Fiscal year 2004-2005. This Contract reflects a reduction in the County's Health Services Department operated services including indirect services with this Community Based Organization. CHILDREN'S IMPACT"STATEMENT: This Alcohol and Drug Abuse treatment program supports the Board of Supervisors' "Families that are Safe, Stable, and Nurturing" community outcome by providing parenting education, stability, and safety for mothers (and their children) and pregnant women who are alcohol and drug dependent, while they are in substance abuse treatment. Expected outcomes include delivery of drug-free babies, decreased use of alcohol, tobacco and other drugs,reduction in the number of relapses,and creation of a sober social network. BACKGROUNDfREASON(S)FOR Ct7►lV MENDATION(5): This Contract meets the social needs of County's population in that it provides family-centered alcohol and drug treatment services to pregnant and parenting women, in order to prevent perinatal substance abuse and improve birth Outcomes. On October 21, 2003, the Board of Supervisors approved Novation Contract #7419084-4 with Ujima Family Recovery Services for the period from July 1, 2003 through June 30, 2004, (which included a six-month automatic extension through December 31,2004)for the provision of perinatal residential treatment services for CaIWORKs participants at Contractor's facility known as The Rectory. Approval of Novation Contract #74--084---5 replaces the automatic extension under the prior contract and allows the Contractor to continue providing services through June 30,2005. SPECIFIC REQUEST($)OR RECOMMENDATION(S)&BACKGROUND ANIS JUSTIFICATION R.FC0MMENDA'T1CiN ): Approve and authorize the Health Services Director, or his designee (Ronna Wigand) to execute on behalf of the County, Novation Contract#24-315-35 with Young Men's Christian Association of the East Bay, Inc., a non.-profit corporation, in an amount not to exceed $300,897,to provide on- school-site counseling services for emotional and behavioral disturbed students in selected schools in West County for the period from July 1, 2004 through June 30, 2005. This Contract includes a six-month automatic extension through.December 31,2005, in an amount not to exceed$150,449. FISCAL IMPACT: This Contract is funded 44% by Federal Medi-Cal (FFP), 44% by State EPSDT Funding, and 12% by Mental Health Realignment. Funding under this contract reflects a reduction in County subsidy/realignment funds and increases required minimum Medi-Cal FFP/EPSDT funds in accordance with the Department's Board approved budget balancing plan effective October 1,2004. CHILDREN'S IMPACT STATEMENT: This EPSDT Program supports the following Board of Supervisors' community outcomes: "Children ready for and succeeding in school"; "Families that are safe, stable, and nurturing"; and "Communities that are safe and provide a high duality of life for children and families". BACKGROUND/REASON(S)FOR RECOMMENDATION(S): This Contract meets the social needs of County's population in that it provides counseling and therapy services for emotionally and behavioral disturbed students in selected junior high schools in the Nest Contra Costa Unified School District that improves their high school attendance records and decreases the use of acute Mental Health system of care services. On October 28, 2003, the Board of Supervisors approved Novation Contract #24-315-s34 with Young Men's Christian Association of the East Bay, Inc., for the period from July 1, 2003 through. June 30,2004(which included a six-month automatic extension through December 31,2004),for the provision of on-school-site counseling services for emotional and behavioral disturbed students. This Contract includes a sixmonth automatic extension through December 31, 2005, in an amount not to exceed$715,338. On July 6, 2004, the Board of Supervisors approved an amendment to Contract #24-925.10 with Lincoln Child Center, for the period from July 1, 2003 through June 30, 2004 (which included a six-month automatic extension through December 31, 2004), for the provision of SchoolBased Mental Health Services for Severely Emotionally Disturbed(SED)Students and their Families. Approval of Novation Contract #24-925-11 will replace the automatic extension under the prior contract, and will allow the Contractor to continue providing services through June 30, 2005.
