FISCAL IMPACT: No financial impact. BACKGROUND: This is the quarterly report on the services provided, management and security of the Crestwood Behavioral Health Center located in Pleasant Hill. Crestwood Healing Center opened October 10, 2003. They have recently celebrated their one-year anniversary. As of December 3, 2004 there are 44 residents in two distinct programs-- 18 in the Adult Residential Facility and 26 in the Mental Health Rehabilitation Center. Additionally, the sixteen bed Residential Treatment facility is scheduled to open in March, 2005. f CONTINUED ON ATTACHMENT: �4iS i. The pre-vocational program allows consumers to work side-by-side with staff learning the various skills needed for jobs in the Healing Center. This may be in the kitchen, landscaping, housekeeping, laundry or secretarial. • Nine consumers are currently working for a wage for Crestwood through vocational programs. • One consumer is currently employed 20 hours per week at local business. • Seven consumers are currently enrolled at outside schools (these include DVC, Loma Vista Adult Education and the ROP Program). • Six consumers are participating in Horticulture class with staff at IVC weekly. • One consumer is enrolled through ROP program in the Certified Nursing Assistant program. This consumer has passed her state boards for Certified Nursing Assistant and is continuing her classes at ROP for acute care training. • One consumer is enrolled at Loma Vista Adult Education in High school diploma program. • We last reported to the Board of Supervisors in September and since that time there have been no incidents and no walk-aways. In addition to quarterly reports to the Board of Supervisors, the Pleasant Hill Zoning Administrator has been providing quarterly reports to the Pleasant Hill Planning Commission. A copy of the October 26, 2004 report is attached as Exhibit A. This report concludes that the facility is operating in full compliance with their use permit. BACKGROUND On August 4, 2003, the City Council adopted resolution 66-03, granting the appeal of Crestwood and approving the use permit with conditions of approval. One of the conditions of approval is: Periodic Review. The Zoning Administrator shall report to the Planning Commission regarding applicant's compliance with these conditions of approval on a quarterly basis during the first year of operation of the facility. The first quarterly review shall be held three(3)months after the first consumer occupies the facility. j The Crestwood Healing Center opened on October 10, 2003 with three consumer admissions. The first three Zoning Administrator quarterly reviews were previously presented to the Planning Commission earlier this year. i 11. ANALYSIS OF CONDITIONS OF APPROVAL The following is a synopsis of the status of compliance by Crestwood Healing Center with the conditions of approval.A copy of City Council Resolution 66-03 with the conditions of use permit approval is attached. Building and Fire Code Compliance. Status: To date, the structure fully complies with all applicable state and local code regulations.The Healing Center will undergo its annual fire inspection by the Contra Costa Fire District in the next few weeks. Compliance with State and Federal Laws. Status: The facility is in compliance with state and federal laws. In March,the California Department of Mental Health conducted an annual survey and reissued a license to continue operation of the 40-bed Mental Health Rehabilitation Center(MHRC). On June 101h,the California Department of Social Services/Community Care Licensing issued a license to allow operation of the maximum 24-bed Adult Rehabilitation Facility(ARF).To date,Crestwood Behavioral Health has not obtained a license for the maximum 16-bed Residential Treatment Facility(RTF). I i i Status Report - Crestwood Healing Center Page 2 550 Patterson Boulevard UP 02-013 Contract with County. Status: Complete; no changes since last report. County Office Space. Status: Complete. Dedicated office space is available for County staff, which are on-site every day. A dedicated office space is also available for a mental health patients rights advocate, who is also a County employee, on-site at least four days a week. Exterior Lighting. Status: Complete. Generator. Status: Still Partially Complete. Crestwood installed a temporary rental generator in the summer of 2003, located on a portion of the site that had exclusive access through the abutting St. Teresa's property. The owners at St.Teresa subsequently erected a solid wooden fence to contain this generator and to eliminate access to it from their property. In effect, the temporary generator became "landlocked" and inaccessible. Crestwood continues to rent the generator and intends to leave it in its present location while attempts to mitigate a resolution with the owners of St. Teresa's continue. Impact on Police Services. Status: Complete. Crestwood is current with respect to its funding obligations for a police officer and police vehicle. The funding provided by Crestwood is being allocated to pay for police services to cover this beat. The police vehicle was purchased and is in use. Indemnification. Status: Complete. Landscaping. Status: Complete, Licensing. Status: In compliance. Crestwood is licensed to operate the MHRC from the California Department of Mental Health for a maximum of 40 consumers. Crestwood received a license from the California Department of Social Services/Community Care Licensing to allow operation of the 20-bed ARF.The use permit provides for a maximum of 24 consumers. However, the state licensed a maximum of 20 ARF consumers, based upon the numbers of showers at the facility (a 1:10 ratio is required). The application for the Residential Treatment Facility (RTF) will be submitted to the California Department of Mental Health and State of California Department of Social Services/Community Care Licensing (combined license) at some time in the future.The requested license for the RTF would be for 15 consumers, one less than is provided by the use permit. At total occupancy, Crestwood Healing Center would have a licensed maximum of 75 consumers, five less than provided in the use permit. Loading Area. Status: In compliance. Status: In compliance.The MHRC currently has 26 g g consumers and is allowed a maximum of 40. The ARF currently has 16 consumers and is allowed a maximum of 20 consumers.The request for a license to operate the RTF could be submitted in the near future to the Department of Mental Health and Community Care Licensing. Since the Center opened,69 consumers have been admitted.Forty-three have j been discharged. ' Neighborhood Committee. Status: Victoria Maat resigned from the committee during the Iast quarter. Staff has not been informed if this position has been filled.The first meeting of the committee was held with Crestwood and County Mental Health staff on April 8th. Another meeting was held on September 71h' Staff is not aware of other meetings scheduled by the committee. I Non-Consumer Screening. Status: In compliance. Other Facilities. Status: In compliance. Outdoor Equipment. Status: Complete. Parking. Status: In compliance. i Programs,Meetings,and Activities. Status: In compliance. Sixteen consumers currently participate in a pre-vocational program, which allows consumers to work for t wage,side-by-side with staff from the facility,learning a variety of work skills used at j the Healing Center. Work tasks include the kitchen,landscaping, housekeeping,laundry, or secretarial. One consumer currently works 20 hours per week at a local business. 4 Seven consumers are currently enrolled at outside schools, including Diablo Valley College (DVC),Loma Vista Adult Education and the ROP specific vocational program. One consumer is enrolled in an ROP certified nursing assistant program.Six consumers participate in a weekly in a horticulture class with staff at DVC.Upon admission,all consumers are provided voter registration cards. Most are registered voters. Several I 4 participate in a weekly class discussion of various current events,measures, and candidates. Weekly classes are also held on culture awareness,exploring traditions of various cultures. Different cultures are celebrated monthly, with individuals presenting j the dress, food, and traditions of their culture. All consumers complete a spirituality survey upon admission to the Healing Center. A weekly spirituality class is offered, including services in the community of the various faiths. Another weekly class in Our Daily Bread, held at the Center, where consumers bake pastries,muffins,pies, and cakes for meals at the center and various meetings held at the Center. Several off-site outings have been held since the Center opened, including fruit picking,the Berkeley kite festival, monthly barbeques at a local park, and concerts. Most recently, six bicycles were purchased for consumer use,encouraging health and wellness. i i r Status Report-Crestwood Healing Center Page 4 550 Patterson Boulevard UP 02-013 Security. Status: Complete. All required security systems and improvements have been continuously operational. Periodic Review. Status: This is the final report. Signage. Status: Not applicable. There are no regulated signs at the site. Smoking Areas. Status: In compliance. Special Event. Status: In compliance. No special events were held in the last quarter. Trash Enclosure. Status: Complete. Use Permit Description. Status: Complete. Visitors. Status: In compliance.From October 10, 2003 to September 23, 2004, County mental health staff made 635 visits to the Center.During that same period, family and visitors made 1, 260 visits. 111. INCIDENTS Since the July 20th Planning Commission meeting there has been one incident (on August 4"') where a consumer left a department store in the city while on a supervised visit. This consumer was located within one hour at her condo, which she owns.The consumer was admitted to a higher level of care and did not return to the Healing Center. After the August 4`h occurrence Crestwood staff met with Lieutenant Enea of the Police Department and with Victor Montoya and John Allen from Contra Costa County Mental Health. Additional systems were put in place should future similar incidents occur. Specifically, Contra Costa Mental Health staff will be provide after-hours notification for transporting consumers with assistance from Pleasant Hill Police Department. Additionally,clarification was made with the Police Department so that consumers, who are, according to the licensing regulations, already under conservatorship, are not also placed under State 5150 holds. Since it opened in October 2003,nine unauthorized leave incidents by Crestwood consumers have occurred. These incidents were described in previous status reports to the Planning Commission. Since the last report,Crestwood placed a staff person to monitor the lobby until 10:00 p.m. daily. ............................................... ............................................................................................ CONCLUSION � s The Crestwood Healing Center continues to operate m full compliance with the use permit conditions of approval. No action is required of the Planning Commission. Although this is the final status report, Planning staff will continue to maintain close communication with the staff at the Crestwood Healing Center. Staff will not hesitate to inform the Planning Commission if any significant violations of either the Use Permit Endings or conditions occur.
