Approval of Contract Amendment Agreement#26-294-11 with Staff Care,Inc. e 5 SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RFCQAMENDAT1 N4S): Approve and authorize the Health Services Director, or his designee (Jeff Smith, M.D.), to execute on behalf of the County, Contract Amendment Agreement #26-294-11 with Staff Care, Inc., a Corporation,effective November 1,2004,to amend Contract#26-294-8 (as amended by Amendment Agreements #26-294-9 and #26-294-10), to increase the total Contract Payment Limit by $425,000 from $550,000 to a new total Contract Payment Limit of$975,000 and to extend the term of the Contract through December 31, 2005. EISCAL IMPACT: Funding for this contract is included in the Health Services Department Enterprise I budget. As appropriate,patients and/or third-party payors will be billed for services. S): For many years the County has contracted with registries to provide temporary qualified personnel to assist the Department during peak workloads,temporary absences and emergency situations. On December 17, 2002, the Board of Supervisors approved Contract #26-294-8 (as amended by Contract Amendment Agreements #26-294-9 and #26-294-10) with Staff Care, Inc., for the period from January 1, 2003 through December 31, 2004, to provide locum tenens physicians to work as temporary employees to provide vacation, sick leave, and extended leave relief for County-employed physicians at Contra Costa Regional Medical Center and Contra Costa Health Centers, including the specialty of Radiology services. Approval of Contract Amendment Agreement #26-294-11 will allow the Contractor to provide additional services to include Vascular Surgery, Trauma Surgery, Occupational Medicine, Nephrology, Radiation Oncology, Pathology, Pulmonary and many other classifications of temporary physicians services,through December 31,2005. FISCALIMP.AI: This Contract is funded 100% by Enterprise I Funds. Cost to the County depends upon utilization. As appropriate,patients and/or third party payors will be billed for services. For a number of years the County has contracted with Medical and Dental Specialists to provide specialized professional services which are not otherwise available in its hospital and clinics. On December 2, 2003, the Board of Supervisors approved#76-515 with Larkin Breed, M.D., to provide part-time ancillary Radiology physician services, for Contra Costa Regional Medical Center and Contra Costa Health Centers including, but not limited to, consultation, on-call coverage, and clinic coverage services,for the December 1,2003 through December 31, 2004. OMMFND ` ION(S): On August 6, 2002, the Board of Supervisors approved Contract#26-936-4 (as amended by Amendment Agreement#26-936-5)with Kevin A. Beadles,M.D., for the period from August 1, 2002 through July 31, 2005, for the provision of Ophthalmology services, including consultation, clinic coverage, training, and medical and/or surgical procedures, for Contra Costa Regional Medical Center and Contra Costa Health Centers. The Department and the Contractor have mutually agreed to renegotiate the Contract Payment Provisions and the term,, to reflect the intent of the parties in which Dr. Beadles is to be paid for each clinic instead of a monthly rate. Therefore, in accordance with Paragraph 4. (Termination)of the Contract, the Department and Contractor have agreed to a mutual cancellation of Contract#26936-4 (as amended by Amendment Agreement #26-936-5). Approval of Cancellation Agreement #26-936-6 will accomplish this termination. Under Contract #26-936-7, Contractor will provide professional Ophthalmology services, through December 31,2007. This Contract includes a six-month automatic extension through December 31,2005, in an amount not to exceed$268,544. Fid,IMPACT: This Contract is funded by 84% State CalWORKs and 16%Federal Perinatal Set Aside Funds. In September 2004, the State Department of Social Services reduced funding for CalWORKs Substance Abuse/Mental Health Services Statewide for fiscal year 2004-2005. This Contract reflects a reduction in the County's Health Services Department operated services including indirect services with this Community Based Organization. RACKGRC) 1NDIREASONl FOR ill+COMMEMA'I'ION(S): This Contract meets the social needs of County's population in that it provides assessments and pre- treatment and pre-sobriety services for substance abusing and mentally ill participants in the CalWORKs program to help eliminate barriers to employment. On December 9, 2003 the Board of Supervisors approved Contract #24-948-10 with New Connections, for the period from July 1, 2003 through June 30, 2004 (which included a six-month automatic extension through December 31, 2004)to provide substance abuse treatment and recovery services, outpatient perinatal intensive day treatment services and substance abuse and mental health assessment services for participants in the CalWORKs Program.
