SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on behalf of the County, Novation Contract #24-315.34 with Young.Men's Christian Association of the East Bay, Inc., a non-profit corporation, in an,amount not to exceed $300,897, to provide on- school-site counseling services for emotional and behavioral disturbed students in selected schools in West County for the period from July 1, 2003 through June 30, 2004. This Contract includes a six-month automatic extension through December 31, 2004,in an amount not to exceed$150,449. FISCAL DAPACT: This Contract is funded 40% by Federal Medi-Cal (FF'), 24% by State EPSDT Funding, and 36% by Mental Health Realignment. CHILDREN'S IMPACT STATEMENT: This EPSDT Program supports the following Board of Supervisors' community outcomes: "Children ready for and succeeding in school"; "Families that are safe, stable, and nurturing"; and "Communities that are safe and provide a high quality of life for children and families". Expected program outcomes include an increase in positive social and emotional development as measured by the Child and Adolescent Functional Assessment Scale(CAFAS). BACKGROUND/REASON(S)FOR RECOMMENDATION(S): This Contract meets the social needs of County's population in that it provides counseling and therapy services for emotionally and behavioral disturbed students in selected junior high schools in the West Contra Costa Unified School District that improves their high school attendance records and decreases the use of acute Mental health system of care services. On December 17, 2002, the Board of Supervisors approved Novation Contract #24-315-32 (as amended. by Administrative Amendment Agreement #24-315--•33) with Young Men's Christian Association of the East Bay, Inc., for the period from July 1, 2002 through June 30 2003 (which included a six-month automatic extension through December 31, 2003), for the provision of on- school-site counseling services for emotional and behavioral disturbed students. Approval of Novation Contract #24--315-34 replaces the Automatic Extension under the prior agreement and allows the Contractor to continue providing services through June 30, 2004. BACKCR.OUTNL`/REASON(S) FOR RECOMMENDATION(S) : For a number of years the County has contracted with Medical and Dental Specialist to provide specialized professional services which are not otherwise available in its hospital and health centers . On October 15, 2002 the Board of Supervisors approved Contact #26-977-- 2 with Pacific Urology, for the period from November 1, 2002 through October 31, 2003, to provide urology services to patients at Contra Costa Regional Medical Center including, but not limited to, clinic coverage, consultation, training, on-call, ultra sound, evaluation and management services, and medical and/or surgical procedures . Approval of Contract ##26-977-3 will allow the Contractor to continue to provide services through October 31, 2005 . FZ1MXC AL IMPACT The Agreement is funded in the Health Services Department ' s Enterprise I Budget . Under the term of the agreement, VANCHCS is paid a fee for service in accordance with the fee schedule which is incorporated in the Contract. The services provided for the County' s patients under this Contract are billable to patients and third party payors . REASQNS FOR RECOMMENDA` IONS/EACKGROM For many years the County and U.S. Department of Veterans Affairs has maintained a mutual sharing agreement which has made available to the County specialized medical services not otherwise available due to lack of resources, equipment, and personnel . These services included specialized laboratory testing, radiology services, nuclear medicine studies, CT scans, MRI ' s, dermatology, gastroenterology, urology, audiology and speech, and ophthalmology services . The County provided Emergency Room treatment and inpatient care, including certain ancillary services for VA--referred patients'. Approval of Contract #26-343-1.0, will allow Contractor to continue providing specialized ophthalmology medical services to County' s patients referred for treatment by Contra Costa Regional Medical Center physicians through September 30, 2004 . BACKGROUND/REASON(S) FOR RECOMMENDATION(S): The Health Plan has an obligation to provide Health Plan members with professional health care services. The UCSF Medical Center is exemplary in' the broad spectrum of care it can provide. Approval of this Contract will allow the Health Plan to utilize this facility and its services to enhance our County medical staff and facilities in continuing to provide excellent care to the community. Under Contract#27-563, the Contractor will provide health care services to Health Plan members, through September 30, 2004. CMLDREN'S IMPACT STATEMENT: This Runaway and Homeless Youth Program supports County's "Children and Youth Healthy and Preparing for Productive Adulthood" and"Communities that are Safe and Provide a High Quality of Life for Children and Families" community outcomes by providing temporary shelter and services to homeless youth in crisis, to stabilize their situation and possibly reunite the youth with their families.. BACKGRCDtJI'+U EASON(S)FOR RECOMNMNDATION{S): This Contract meets the social needs of County's population in that it provides shelter and services at a drop-in center, including food, clothing, counseling, and other support services to Contra Costa youth between the ages of 14 and 17. On November 12, 2002 the Board of Supervisors approved Contract #25-039--1 (as amended by Contract Amendment Agreements #25-0392 and #25---039-3) with Opportunity West for the period from September 30, 2002 through September 29, 2003, for the provision of temporary shelter and support services to homeless youths in crisis. On January 7, 2003,the Board of Supervisors approved Contract#25-051 with Neighborhood House of North Richmond, for the period from December 1, 2002 throw September 29, 2003 for the provision of intensive day treatment services, residential alcohol and drug abuse treatment services, and sober living services to homeless clients in County's,Synergy Program. Approval of Contract #25-051v--1 will allow the Contractor to continue providing services through September 29,2004.
