SUBJECT: Long Term Care Integration Project

RECOMMENDATION,
ACCEPT oral presentation from the Lang Term Care Integration Task Force on the pilot program,
completion of planning phase, and establishment of Long Term Care Advisory committee.

BACKGROUND:
The Health Services and Employment and Human Services Departments have been awarded a
$150,000 grant from the California Department of Health Services, Offices of Long Term Gare, to
continue a unique departmental collaboration in developing a long term care integration program for
aged and disabled Med-Cal recipients. The grant supports the two agencies in their on-going work
with community activists, advocates, providers, and consumers to develop a single comprehensive,
coordinated, and consumer-friendly program of medical, social, and supportive services that will help
some community members remain independent and still get the care they need. Since 1099, an "up
from the grassroots" Long Term Care Integration Task Force has met to determine why it's so
complicated to get the services needed for the fastest growing population in the country. "One of our
highest priorities is to teach the community about what's involved when we say'long term care
integration',"said Dr. A.C. Hollister, chair of the current Task Force. "[wring all of the years of
planning, a major goal has been to include consumers. This year we'll develop a long-range plan to
assure that there is on-going consumer participation."
The current system of helping the frail elderly and disabled remain independent involves multiple
agencies and a maze of bewildering choices. "Each of us wants to be independent. However, if we
have significant medical and social risks, the choice may become problematic. The integration of
long term care services such as help with bathing and cooking, getting to the doctor, and monitoring
medicines gives even the frailest members of our community the option of being as independent as
possible," said Carol Freels, Chief of California's Office of Long Term Gare.
