SUBJECT: AUTHORIZATION TO AMEND CONTRACT FOR MENTAL HEALTH
INTERN SERVICES
Specific Request(S)or Recommendation(S) & Background and Justification
RECOMMENDATION (S):
APPROVE and AUTHORIZE the Community Services Department Director or designee to
execute contract amendment #3$661-I with Alicia Diaz to increase the payment limit by
$1500 to a new payment limit of$26,440 to provide continuing Mental Health Intern services
for the period from May 6, 2042 through May 5,2003.
FINANCIAL IMPACT:
Funded by the U.S. Department of Health. and Human Services Administration for Children
and Families {ACF}. Funding for this contract is included in the Contra Costa County
Community Services Department's FY 2002-03 Budget. There is no net County cost to this
contract.
CHILDREN'S IMPACT STATEMENT:
The Community Services Department supports two of Contra Costa County's community
outcomes: "Children Ready for and Succeeding in School" and "Families that are Safe,
Stable and Nurturing." These outcomes are achieved by offering comprehensive services,
including high duality early childhood education, nutrition, and health to low-income
children throughout Contra Costa County.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
Travel estimates for contractor to provide Mental Health Intern services has exceeded
expectations. Due to the increased travel necessary to provide Mental Health Intern
services,the Contract Payment Limit has been increased.
Approval of this amendment will allow contractor to provide continuing Mental Health
Intern services to the Community Services Department.

RECOMMENDATIONIS :
APPROVE and AUTHORIZE the Community Services Department Director or designee to
execute contract amendment 438-670-2 with Suraya Keating to increase the payment limit by
$2500 to a new payment limit of$25,500 to provide continuing Mental Health Intern services
for the period from September 16,2002 through September 15,2003.
FINANCIAL IMPACT:
Funded by the U.S. Department of Health and Human Services Administration for Children
and Families (ACF). Funding for the period from September 16, 2002 through.Tune 30, 2403
is included in the Community Services Department's FY 200203 Budget. Funding for the
period from July 1, 2403 through September 15, 2003 will be included in the Community
Services Department's proposed FY 2003-04 Budget. There is no net County cast to this
contract.

RESOURCE STAFF SERVICES

RECOMMENDATION 6):
APPROVE and AUTHORIZE the Community Services Department Director or designee to
execute contract amendment ##38-784-2 with OfficeTeam to increase the payment limit by
$34,000 to a new payment limit of$57,000 and to extend the contract period from December
31, 2002 to March 31,2003 for the provision of temporary human resource services.
FINANCIAL IMPACT:
This contract is funded through administration portions of State and federal grants. Funding
for this contract is included in the Community Services Department's FY 2002-03 .Budget.
There is no net County cost to this contract.

ASSESSMENT NEEDS SERVICES

RECOMMENDATION (S):
APPROVE and AUTHORIZE the Community Services Department Director or designee to
execute contract amendment #38-792-1 with Applied. Survey Research to increase the
payment limit by $18,480 to a new payment limit of $43,120 for continuing Community
Assessment Needs services for the period from January 1,2003 through March 31 2003.
FINANCIAL IMPACT:
Funded by the U.S. Department of Health and Human Services Administration for Children
and Families (ACF). Funding for this contract is included in the Contra Costa County
Community Services Department's FY 2002-03 Budget. There is no net County cost to this
contract.

REASONS FOR RECOMMENDATIONS/BACKGROUND:
A thorough Community Assessment will aid the Community Services Department in
program planning. The assessment will assist the Community Services Department in such
areas as grant writing, the Community Action Plan (CAP), and determination of future
child care facilities.
Applied Survey Research will provide research and assessment data on Community
Assessment Indicators including, but not limited to, employment, housing, birth rates,
health insurance, foster care, child abuse, and domestic violence.
Approval of this amendment will allow contractor to Prov' e continuing C mmunity
Assessment Needs services to the Community Services Deparrment.

Approve and authorize the Health Services Director', or his designee
(Kathryn Grazzini) to execute on behalf of the County, Contract
Amendment Agreement # 6-378-1 with Concord Yellow Cab, Inc. , a
corporation, effective August 1, 2002 , to amend Contract #26-378, to
increase the total Contract Payment Limit by $45, 000 from $27, 000,
to a new total payment limit of $72, 000 .
FISCAL IMPACT
This Contract is included in the Health Services Department' s
Enterprise I budget .

Taxicab services are utilized when a clinical condition could be
adversely affected by lack of transportation for needed medical
care, including but not limited to, physical disabilities which make
it unsafe for patient to travel on public transportation, patient
has a verifiable seizure disorder, or patient has received mediation
which has or could impair the mobility or coordination of the
patient .
On September 24, 2002, the .Hoard of Supervisors approved Contract
#26-378 with Concord Yellow Cab, Inc. , for the period from July 1.,
2002 through June 30, 2003 , to provide taxicab ' transportation
services to transport patients to and from Contra Costa Regional
Medical Center and Centra Costa. Health Centers and other health
facilities.
Approval of Contract Amendment Agreement ##26-378-1 will allow
Contractor to provide additional services through June 30, 2003 .
