SPECIFIC REQUEST($)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director or his designee (Donna
Wigand) to execute on behalf of the County, Contract #74-191 with West
Contra Costa Unified School District, a non--profit corporation, in an amount
not to exceed $50, 000, to provide wraparound services to Severely
Emotionally Disturbed (SED) children, for the period from July 1, 2002
through June 30, 2003 .
FISCAL IMPACT:
This Contract is 50% funded by Federal Medi-Cal, 4896 funded by State Early
and Periodic, Screening, Diagnostic and, Treatment (EPSDT) Program, and 2%
funded by .the State AB3015 funds.
CHILDREN'S IMPACT STATEMENT:
This EPSDT program supports the following Board of Supervisors community
outcomes : Children ready for and succeeding in school ; Families that are
safe, stable, nurturing; and Communities that are safe and provide a high
quality of life for children and families . Expected outcomes include all
goals identified by Children' s Statewide System of Care guidelines including
an increase in positive social and emotional development as measured by the
Child and Adolescent Functional Assessment Scale (CAFAS) and decreased use
of acute care system.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of County' s population in that it
provides child-family team facilitators and other wraparound services to
families of children with serious emotional and behavioral disturbances;
facilitates multi-agency collaborative service delivery; and minimizes the
need for crisis services and involvement with the Juvenile Justice System.
Since 2001, West Contra Costa Unified School District has provided services
to Contra Costa. Youth and their families . Under Contract #74--191 Contractor
will provide wraparound services through June 30, 2003 .

RECOMMEMA.O Mil:
Approve and authorize the Health Services Director, or his designee(Wendel Brunner)to execute
on behalf of the County,Contract #23-312-3 with La Clinica de La Raza,a non-profit corporation,
in an amount not to exceed$58,333,for the period from September 1,2002 through August 31,2003,
for the provision of nutrition, dental, and case management services to County's low income,
uninsured patients with diabetes or other complex medical problems.
FISCAL IMPACT:
The Contract is 100%Banded by Health Resources and Services Administration.Community Access
Program(CAP)grant.

On June 2001, the Board of Supervisors approved Contract #23-312 (as amended by Contract
Extension Agreement#23-312-1 and Contract Amendment Agreement#23-312.-2) with La Clinica
de La Raza, for the provision of nutrition, dental and case management services to County's low
income, uninsured patients with diabetes or other complex medical problems, for the period from
May 1, 2001 through August 31, 2002.
Due to an administrative oversight by both County and Contractor, the Department inadvertently
overlooked processing a formal document consistent with the agreed upon oral agreement. Services
were requested and provided in good faith by the Contractor. Approval of Contract #23-312-3 will
allow the Contractor to continue to provide services,through August 31,2003.

Approve and authorize the Health Services Director, or his designee (Donna
Wigand) to execute on behalf of the County, Contract #24-602-20 with Seneca
Residential and Day Treatment Center, Inc. , a non-profit corporation, in an
amount not to exceed $36,750, to provide intensive day treatment for
County-referred minors, for the period from January 1, 2003 through June 30,
2003 . This Contract includes a six-month automatic extension through December
31, 2003 in an amount not to exceed $36,750.
FISCAL IMPACT:
This Contract is funded 40% by Federal Medi-Cal, 40% by State EPSD'T funds, and
20* by Mental Health Realignment.
CHILDREN'S IMPACT STATEMEM:
This Mental Health Program supports the following Board of Supervisors' community
outcomes: "Children Ready For and Succeeding in School"; "Families that are
Safe, Stable, and Nurturing"; and "Communities that are Safe and Provide a High
Quality of Life for Children and Families" . Expected program outcomes include
an increase in positive social and emotional development as measured by the Child
and Adolescent Functional Assessment Scale (CAFAS) .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) -
This Contract meets the social needs of County's population in that it provides
a multi-disciplinary treatment program as an alternative to hospitalization for
minors who need active psychiatric treatment for acute mental, emotional or
behavioral disorders.
On June 19, 2001, the Board of Supervisors approved Contract #24-602-18 (as
amended by Contract Amendment Agreement #24-602-19) with Seneca Residential and
Day 'Treatment Center, Inc. , for the period from July 1, 2001 through June 30,
2002, (with a six-month automatic extension through December 31, 2002) for the
provision of intensive day treatment, mental health, medication support, crisis
& case management services for County-referred minors.
Approval of Contract #24-602-20 will allow the Contractor to continue providing
services through June 30, 2002.

This contract meets the social needs of County's population in that it
provides mental health residential treatment services to 'SED adolescents
and ,latency age children as an alternative to hospitalization at Napa. State
Hospital .
On June 19, 2002, the Board of Supervisors approved Contract #24-682-17
with Victor Treatment Centers, Inc . , for the provision of subacute
residential treatment services for Seriously Emotionally Disturbed (SED)
adolescents and latency-age children, for the period from July 1, 2002
through June 30, 2003 .
Approval of Contract Amendment Agreement #24-682-18 will allow the
Contractor to provide additional residential treatment services for
County' s AB 3632 clients, through June 30, 2003 .

BACK GRC MMZREASON(S) FOR RECQ MENDAT10N(S) :
On. October 2, 2001, the Board of Supervisors approved Contract
#26-397-1 with Cardiomedix, Inc. , for the period from September 1,
2001 through August 31, 2003, for the provision of cardiac tele-
medicine monitoring services with regard to 24-hour Holter scanning,
analysis, and Trans-telephonic ECG Monitoring (TTM) for patients
referred by physicians of Contra Costa Regional Medical. Center.
Approval of Contract Amendment Agreement #264-397-2 will. allow
Contractor to provide additional cardiac monitoring services,
through August 31, 2003 .
