Approve and authorize the Health Service Director or his Designee
(Kathryn Grazzini) , to execute on behalf of the County, Contract 426-
389-2 with Clinical One, a corporation, in an amount not to exceed
$300, 000, for the period from December 1, 2002 through November 30,
2003 , to provide nursing registry services at Contra Costa Regional
Medical Center.
FISCAL IMPACT:
This Contract is funded by the Health Services Department ' s Enterprise

BACKGROUND TREASON(S) FOR RECOMMENIDATION(S) :
For several years the County has contracted with nursing registries to
provide temporary licensed nursing personnel to assist Contra Costs.
Regional Medical. Center and Contra Costa Health Centers during peak
loads, temporary absences and emergency situations in critical areas.
There is a nationwide nursing shortage, and in spite of persistent
efforts to recruit nurses, the Department has experienced great
difficulty in filling any vacant positions. Therefore, the Department
has had to rely heavily on the registry services in 'order to provide
quality nursing care for patients .
Under Contract 426--385-2, Clinic One will provide nursing registry
services, through November 30, 2003 .

Early Childhood Mental Health Program
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee (Donna Wigand,
L.C.S.W. ) to execute on behalf of the County, Contract #24-308-27 with Early Childhood
Mental Health Program, a non--profit corporation, in an amount not to exceed $302,999, to
provide a therapeutic nursery school, an infant-family home visiting program, and family
and day care services in West Contra Costa County; for the period from January 1, 2003
through June 30, 2003 . This Contract includes a six-month automatic extension through
December 31, 2003, in an amount not to exceed $302,985.
FISCAL IMPACT:
This Contract is funded 37.3% by Federal Medi-Cal Revenue and 62.7% Mental Health
Realignment.
CHIWR.8N'S IMPACT STATEMENT:
This Mental Health program supports the following Board of Supervisors' community outcomes:
"Children Ready For and Succeeding in School"; "Families that are Safe, Stable, and
Nurturing"; and "Communities that are Safe and Provide a High Quality of Life for Children
and Families" . . Expected program outcomes include an increase in positive social and
emotional development as measured by the Child and Adolescent Functional Assessment Scale
(CAFAS) and a decrease in out-of-home placements as measured by mental health and social
services statistics.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
This Contract meets the social needs of County's population in that it provides mental
health services in West County to keep clients with their families in the community and
out of higher levels of care.
On December 12, 2002, the Board of Supervisors approved Novation Contract #24-308-26, for
the period from July 1, 2000 through June 30, 2002, with a six-month automatic extension
through December 31, 2002, to provide a therapeutic nursery school, an infant-family home
visiting program, and family and day care services in West Contra Costa County.
Approval of Contract #24-308-27 will allow the Contractor to continue providing services
through June 30, 2003 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to
execute on behalf of the County, Contract #24-826-9 with Early Childhood Mental Health

January 1, 2003 through June 30, 2003, to provide assessment and treatment services to
Meda.-Cal eligible children under the age of 6, and" their families, in West Contra Costa
County. This Contract includes a six-month automatic extension through December 31, 2003,
in an amount not to exceed $103,902.
FISCAL IMPACT:
This Contract is funded 51.5% by Federal FFP Medi-Cal Funds and 48.5% by State EPSDT Funds.

This EPSDT program supports "Children ready for and succeeding in school", "Children and
youth who are healthy and preparing for a productive adulthood", and "Families that are
safe, stably and nurturing" community outcomes. Expected program outcomes include
improvement in social-emotional. development, avoidance of out-of-home placements, reduction
of reports involving Children's Protective Services, increased family satisfaction, and
an increased use of public and community-base resources.
SACKGROUN LP-PASON(S) FOR RECOMM H7ATION(s) :
This Contract meets the social needs of County's population in that it provides Early
Childhood Assessment and Treatment Team (ECATT) services to children under the age of 6,
and their families, with a special focus on teen parents, in order to keep the children
out of higher levels of care, including State Hospitals.
On December 11, 2001, the Hoard of Supervisors approved. Contract #24-826-8 with Early
Childhood Mental Health Program, for the period from July 1, 2001 through June 30, 2002,
which included a six-month automatic extension through December 31, 2002, to provide early
childhood assessment and treatment services in West County.
Approval of Contract #24-826--9 will allow the Contractor to continue providing services
through June 30, 2003.

Wigand) , to execute on behalf of the County, Mental Health Specialist
Contract #24-974-5 with Joginder Singh, M.D. , a self--employed individual,
in an amount not to exceed $40, 950, for the period from: January 1, 2003
through December 31, 2003 , for the provision of professional psychiatric
services .
FISCAL IMPACT:
This Contract is funded by Mental Health Realignment, and offset by third
party billing.
BACKOROUNDIRRASON(S) FOR REC01 MENDATION(s) :
For a number of years the County has contracted with Medical., Dental. and
Mental Health Specialists to provide specialized professional services which
are not otherwise available in its Hospital and Health Centers .
Approval of Mental Health Specialist Contract #24-974-5 will allow the
Contractor to provide inpatient and outpatient psychiatric services at the
Psychiatric Emergency Services Outpatient Clinic and Contra Costa Regional
Medical Center, through December 31, 2003 .

execute, on behalf of the County, Contract#22-860 With Planned Parenthood.: Shasta Diablo, a non-
profit corporation, in an amount not to exceed $58,333, for the period from September 1, 2002
through August 31, 2003, to provide health education and management services for uninsured
women.
FISCAL, IMPACT:
Funding for this Contract is provided by Health Resources and. Services Administration (HRSA)
Community Access Program(CAP) Grant. No County funds are required'.
A G OUND S N S MR OMNI TI N S :
The Community Access Program provides healthcare assistance to uninsured low income residents
of Contra Costa County who are not eligible for other health coverage programs including Medi-Cal,
Health Families and Medicare.
Under Contract#22-860,the Contractor will provide health education and management services for
uninsured patients at its State-certified facilities located in Antioch and Concord,through August 31,
2003.
