t. Recommendation:
1. Approve and authorize the Health Services Director to execute and submit to the State
Department of Health Services the application for California Health Care for indigents
Program (CHIP)funds for fiscal year 2003/04.
2. Authorize the Health Services Director to execute and submit the Standard Agreement
for County Health Services.
3. Authorize the Health Services Director to execute and submit the Description of
Proposed Expenditure of C.H.I.P.
4. Authorize the Health Services Director to execute and submit any application, Standard
Agreement or proposed expenditure form amendments which may be required that
have no fiscal consequences.

III. Discussion Background:
The passage of the Tobacco Tax and Health Protection Act of 1988 (Proposition 99) and
its implementing legislation, Assembly Bili 75 and Senate Bill 960 and assembly bill 1154,
made available monies to reimburse hospitals, physicians, and other providers for
uncompensated care costs, for making capital improvements, and for implementing data
systems. These monies are available to the County for distribution both County operated
and Non-County operated entities, upon application to the State.
Contained within the application are assurances by the County regarding use of these
funds. These assurances apply to both the County operated services as well as the
privately operated services. The funding conditions are as follows:
1- Funds must be used to supplement existing levels of services provided and shall not
be used to fund existing levels of service.
2. Counties will be required to provide, or arrange and pay for, medically necessary
follow-up treatment, including prescription drugs for any condition detected as part
of a child health and disability prevention screen.
3. A county will have to maintain a level of financial support of county funds for health
services that is at least equal to its county match plus any overmatch of county
funds in the 1988/89 fiscal year.
4. Monies will be accounted for as revenue to the County health Services Plan Update
and Budget, Supplement and cannot be used as county matching funds for any
other program requiring a county match.
5. A county will not be able to impose more stringent eligibility standards for the receipt
of benefits under flection 17000 or to reduce the scope of benefits compared to
those which were in effect on November 8, 1988.
6. Funds will not able available for the support of health services provided to persons
detained in a county or city jail or other correctional facilities.
7. Receipt of funding shall not relieve a county of its obligation to provide indigent
health care as required by Section 17000,
8. Reports will be required on indigent health care program demographic, expenditure,
and utilization data in a manner that will provide an unduplicated count of users for
the fiscal year.
9. All interest earned on the Fund and on each account or sub-account will be accrued
to the benefit of the Fund, account, or sub-account, respectively.

1. Recommendation:
Approve and authorize the Health Services Director to execute and submit to the State
Department of Health Services the FY 2003-04 Standard Appropriation funds.
11. Financial Impact.
The Health Services Department estimates that the County will receive an allocation of State
revenues as follows:

111. Discussion f Background:
Senate Bill SB-2132, Chapter 826, Statues of 2000 appropriated $24,803,000 of Proposition
99 Tobacco Tax money from the Cigarette and Tobacco Produces Surtax Fund and
redirected it to reimburse physicians for uncompensated emergency services rendered by
physicians in hospital emergency departments. SB-2132 is separate from the California
Healthcare for the Indigents Program (CHIP) and is referred to as the "Emergency Medical
Services Appropriation (SMSA)". The State Department of Health Services administers
SMSA through a standard agreement that each County is required to execute in order to
receive its EMSA funding allocation. Contained within the standard agreement are
assurances by the County regarding the expenditure of these funds. Some of these
requirements are as follows:
1, These funds shall not be used to reimburse for physician services provided by
physicians employed by county hospitals.
2. These funds shall not be used to reimburse physicians who provide services
in a primary care clinic.
3. These funds may only be used to reimburse physicians for emergency
services rendered to indigents in fiscal year 2003-04.

accept on behalf of the County, Grant Award #28-637-4 (2-H76-HA-00618-04) with the U. S.
Department of Health and Human Services (Health Resources& Services Administration), to pay
County in an amount not to exceed $272,236, for the period from September 30, 2003 through
June 30, 2004, for the Ryan White Title III Early Intervention Planning Grant.
+ISCAL IMPACT:
Acceptance of this Grant Award will result a total of$272,236, from the U. S. Department of
Health and Human Services (Health Resources & Services Administration) to continue primary
care services to low income and uninsured individuals with HIV in West Centra Costa County. No
County funds are required.

West Contra Costa County has been hard hit by the AIDS epidemic with 25 l of those livin g with
AIDS residing in the City of Richmond. Unfortunately, a large percentage of those living with
HIV/AIDS are of low income having to rely on Basic Adult Care (BAC)for their medical care,
or forced to pay for their medical care, because they do not qualify for BAC services.
The goal of this program is to allow the Health Services Department to expand outpatient medical
services to low income HIV positive recipients of medical services in West Contra Costa County
through June 30, 2004.

amount not to exceed $150,000, for the period from July 1, 2003 through June 30, 2004, for the
development and implementation of a Long Term Care Integration Program for disabled residents of
Contra Costa County.
FiTSCAL IMPAC T:
Approval of Grant Agreement will provide County with $150,000 of State funds for the Lung Term Care
Integration Program. No County funds are required.

The Contra Costa Health Plan in collaboration with the Employment and Human Services Department, is
working to provide residents of Contra Costa County with a Long Term Care Integration (LTCI)
program. The LTCI Program will integrate financial and administration of social and medical services for
aged, disabled and blind individuals. The goal of the LTCI Program is to reduce duplication, gaps and
inefficiencies in social and medical care for seniors and adults with disabilities.
Approval of this Grant Agreement #29-783 will allow the County to develop and implement the LTCI
Program, for the period from July 1, 2003 through June 30, 2004.
Four certified/sealed copies of this Board Order should be returned to the Contracts and Grants Unit for
submission to the State Department of Health Services.

July 31, 2004 , for the Mobilizing for Action through Planning and
Partnership (MAPP) Project . In accordance with the term of the agreement,

FISCAL IMPACT:
This Grant Agreement funded by Public Health Institute and its Partner-
ship for the Public ' s Health will result in an amount not to exceed
$25, 000 for the Mobilizing for Action through Planning and Partnership
(MAPP) Project . No County funds are required.
BACKGROUND/REASON(S) FOR RECOMMENDATIONW :
The MAPP project is part of a larger collaborative with the Partnership
for the Public' s Health (PPH) which is funded by the Public Health
Institute. The Family and Maternal and Child Health Program ( 'MCH) will
work closely with PPH partners in East, Central, and :West County, to
implement a community-wide strategic planning tool to help the FMCH
Program identify and prioritize public health issues, identify resources,
and take the necessary steps to improve these issues, by obtaining
community resident input .

Smith, M. D. ) to execute on behalf of the County, Coordinated Services
Agreement #29-654-8 with Planned Parenthood: Shasta-Diablo, for the period
from August 1, 2003 through July 31, 2006, for the provision of coordinated
obstetrical services for clients receiving prenatal services through
Planned Parenthood: Shasta--Diablo.
FISCAL IMPACT:
The County incurs no financial obligation to the Agency under this
Agreement . The County will bill Medi-Cal or other third-party payors for
inpatient obstetrical services provided to Planned Parenthood clients .
Planned Parenthood will pay the County a range between $82 . 00-$85 . 00 per
hour, for outpatient consultation and evaluation services provided to their
clients by a County Staff Physician and range between $28 . 00-$30. 00 per
hour, for services provided to their clients by a County Resident
Physician.
The purpose of this agreement is to increase access to early and consistent
prenatal care for low-income women in Contra Costa County. This agreement
describes the areas of cooperation between Planned Parenthood and the
County and specifies the responsibilities of each party in providing
coordinated prenatal, delivery, and post-partum services .
Planned Parenthood will provide routine prenatal care to clients through
the 35th week of pregnancy under the supervision of a Board certified
California-licensed obstetrician/gynecologist. High-risk clients will be
immediately transferred to County for prenatal care and will become
participants in the County' s Healthy Start Program. Contra Costa Regional
Medical Center will provide prenatal care beginning with the 36th week of
pregnancy and will provide labor and delivery obstetrical services .
Approval Contract #29-654-8, will allot the Department to be reimbursed for
continuous outpatient consultation and evaluation services requested by
Planned Parenthood through July 31, 2006.

BACRGR.f UND/REASON{S _ FOR RECOMMENDATION(S).:
The Health' Services Department maintains a TB Control Program which>'
serves all reported TB patients and their contacts in Contra Costa
County. Acceptance of this Award will allow the Department to: (1) `
continue to expand prevention and control activities, and (2) improve
completion of appropriate therapy which is essential to decrease TB
transmission, prevent the development of drug resistance, and cure TB'
patients, through June 30, 2004 .

Agreement (Amendment) #29-316-22 (State #00-91270) with the State: Department of Health
Services, effective June 29, 2003, to amend Standard Agreement#29-316-21, to add $503,772 for
a new total Payment Limit of $1,259,430 and extend the term through June 30, 2005, for the
County's High Risk Infant Follow-Up Project.
FISCAL IMPACT:
This amendment allocates State funding (via Federal Maternal Child Health Block Grant #93.994)
in the amount of$503,772 for fiscal years 2003-2004 and 2004-2005, for anew total payment limit
of$1,259,430. No County match is required.
BACKGROUND/REASON(5) FOR RECOMMENDATION(SI:
On March 20, 2001, the Board of Supervisors approved Standard Agreement #29-316-21 with the
State Department of Health Services for the High Risk Infant Follow-Up Project for the period from
July 1, 2000 through June 30, 2003. This amendment extends the term of the agreement through
June 30, 2005 and provides State funding for the fourth and fifth year of the agreement.
Three sealed and certified Board Orders should be returned to the Contracts and Grants Unit for
submission to the State Department of Health Services.

continuation of the Supplemental Food Program for Women, Infants
and Children (WIC) .
FISCAL XXPACT:
This agreement encumbers $785, 974 of Federal funding through the
State for the second year of this three year agreement . No County
funds are required.

For over eighteen years the County has participated in the WIC
Program with the State. This is a mandated program under the
Community Health Services Division of the State Department of
Health Services. Approval of Standard Agreement (Amendment) #29-
203-82 will provide federal funding through the State for
continuation of the program during the next federal fiscal year
2003-2004 .
WIC is a nutrition education, counseling and food supplement
program for low-income, pregnant, postpartum and breast-feeding
women, infants and children at nutritional risk. Approximately
16, 675 clients are served by this program.

transmit an application for funding in the amount of$60,000 along with the necessary
certification and assurances to the Federal Emergency Management Agency(FEMA) for the
operation of the Emergency Shelter Program for Adults, Emergency Shelter Program for
Families, and Calli House:Youth Shelter.
B. Authorize the Director of Health Services or his designee to accept up to $60,000 in
FEMA funding and enter into a contract with FEMA to perform all responsibilities in
relationship to receipt of the funding and contracted provisions for CY 2004.
FISCAL IMPACT:
The funds requested of the FEMA Phase 22 is essential to providing adequate emergency
housing and supportive services to individuals and families year-round. If funded at the full
request, FEMA will help'to support 983 bed nights at the year-round adult shelter,
approximately 736 bed nights for parents and their children at the family shelter sites, and over
75 bed nights at the youth shelter. No additional County funds are required.

Emergency Shelter for Adults
There is continuing concern about the lack of emergency shelter services for homeless men
and women. The Contra Costa Crisis Center, the agency that operates the County's Homeless
Hotlines and maintains the waiting list for the shelters, reported that during the FY 42-03 they
received an average of 1600 inquiries per month by single adults throughout the Bay Area
looking for shelter—92% are Contra Costa residents. There is a current waiting listof 2414
adults for the Adult Shelters.
Emergency Shelter for Families
It is reprehensible to think that even one child has to sleep in a car or on the streets, yet it is believed that
9740 parents and their children experience this horror each year in Contra Costa County. Currently, our
homeless hotlines are being flooded with from parents looking for shelter space for themselves and their
children. As of September 30, 2043, there were 310 families on the waiting list to enter the County's
Emergency Shelter for Families program. Unfortunately, there are only limited resources throughout the
County and the demand for shelter far exceeds the County's current capacity to assist families in need.
Calli House: Shelter for Youth
The County Homeless Program opened the first homeless shelter for youth of its kind in April 2002.
Since that time, youth shelter program has served 137 youth ages 12 to 21. Youth over the age of 18
reside at the West County Emergency Shelter and receive case management and other services during the
day at Calli House. Calli House provides a youth-friendly environment with age-specific case
management and other supportive services aimed at assisting the youth to exit the streets. Seventy-two
percent of all youth served have successfully exited the streets (90% of youth under 18 have returned
home).
