#76-515 with Larkin Breed, M.D. , (Specialty: Radiology) , a self-
employed individual, for the period from December 1, 2003 through'
December 31, 2004, in an amount not to exceed $250, 000, for the
prevision of professional part-time Radiology physician services for
patients at Contra Costa Regional Medical Center and Contra Costa
Health Centers .
FISCAL IMPACT:
Cast to the County depends upon utilization. As appropriate, patients
and/or third party payors will be billed for services. '
BACXGRC MIP f REASON(S) FOR RECOMMENDATION(S)
For a number of years the County has contracted with Medical and Dental.
Specialists to provide specialized professional services which are not
otherwise available in its hospital and clinics.
Under Contract #76-515, Larkin Breed, M.D. , will provide part-time
ancillary Radiology physician services, for Contra Costa. Regional
Medical Center and Contra Costa Health Centers, including, but not
limited to, consultation, on-call coverage, and clinic coverage, '
through December 31, 2004 .

FISCAL IMPACT:
This Contract is funded by Enterprise I Budget . Cost to the County

party payors will be billed for services .
BACKGROUNDIREASON(S) FOR RECOMMENDATIONS:
On December 12, 2000, the Board of Supervisors approved Contract
#26-979 (as amended by Contract Amendment Agreement #26-979-1) with
Dr. William M. Barnard, to provide administrative and psychiatric
services and act as the Inpatient Psychiatric Clinical Director for
Contra Costa Regional Medical and Contra Costa Health Centers, for
the., period from January 1, 2002 through December 31, 2003 .
Approval of Contract #26-979-2 will allow Contractor to continue to
provide services, through December 31, 2006 .

For many years the County has contracted with registries to provide
temporary qualified personnel to assist the Department during peak
work loads, temporary absences and emergency situations.
On January 22, 2002, the Board of Supervisors approved Contract
#26-429 (as amended by subsequent Amendment Agreements #26-429-1
through #26-429-3) , with J & C Nationwide, for the recruitment of
temporary physician services to cover vacation, sick leave, and
extended leave relief for County-employed physicians at the Contra
Costa Regional Medical Center and Main Detention Facility through
December 31., 2003 .
Approval of Contract #26-429-4 will allow Contractor to continue to
provide services through December 31, 2005 .

REASONS FOR CQMMEN 1_0NS1BACKGRO
On August 6, 2002, the Board of Supervisors approved Contract #26-904-13 with
Stephen D. Weiss, M.D. , for the period from August 1, 20>02 through July 31,
2005, for the provision of General Surgery services .
Due to re--negotiation of the term and the payment provisions under the
Contract, the County and the Contractor have mutually agreed to terminate
Contract #26-904-13 by executing Cancellation Agreement #26-904 -14 and to
substitute Contract #26-904-15 in its place.
Under Contract #26-904-15 Dr. Stephen D. Weiss will provide General Surgery
services and act as Chief of Surgical Services. His services will include, but
may not be limited to, consultation, training, medical and/or surgical
procedures, and on-call coverage services, through December 31, 2006 .

The number of deliveries at Contra Costa Regional Medical Center
continues to increase, as does the percentage of high risk mothers.
This service provides additional support to staff involved in the
care of newborns and is consistent with the Department ' s efforts to
reduce risk to the County.
On December 2, 3003, the Board of Supervisors approved Contract
#26-355-3 with East Bay Neonatology, for the period from January 1,
2003 through December 31, 2003 , for the provision of neonatology
services at Contra Costa Regional Medical Center.
Approval of Contract #26-355-4 will allow Contractor to continue
providing Neonatology Patient Care services, including adminis-
trative, consultation, and teaching services for staff at Contra
Costa Regional Medical Center through December 31, 2007 .

behalf of the County, Contract #24-794-10 with Telecare Corporation, a corporation, in an amount
not to exceed$125,000,to provide inpatient psychiatric hospital services, for the period from July 1,
2003 through June 30, 2004,
FISCAL IMPACT:
This Contract is funded by acute psychiatric inpatient consolidation funds, Federal Medi-Cal
funds, and Mental Health Realignment.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S):
Chapter 633, Statutes of 1994, AB 757, authorized the transfer of state funding for Fee-For-
Service/Medi-Cal (FFS/MC) acute psychiatric inpatient hospital services from the Department of
Health Services to the Department of Mental Health (DMH). On January 1, 1995, the DMH
transferred these funds and the responsibility for authorization and funding of Medi-Cal acute
psychiatric inpatient hospital services to counties that chose to participate in this program.
Approval of Contract #24-794-10 will allow Telecare Corporation to provide inpatient psychiatric
hospital services to County-referred residents, ages eighteen and over,through June 30, 2004.

SUBJECT: REQUEST FOR RETROACTIVE APPROVAL FOR
CONTRACT FOR TELECARE SOLANO. CONTRACT 24-794-8
STATEMENT OF NEEEI3:
Each county is responsible for negotiating the reimbursement rates for all hospitals within
their jurisdiction. Given this, Contra Costa County cannot contract with out-of-county
facilities until the county-specific negotiation process is completed and the rates
published. This delays timely contract execution each year. Now that rates have been
established, it is imperative that a renewal of this contract be expeditiously approved.
This situation necessitates this request for retroactive approval of the contract for the
period 71II03-6/30104.
Therefore,retroactive approval of this request is to cover the service activities of this
contractor and to assure the continuation of all necessary inpatient psychiatric hospital
services to clients referred by county as requested by County. Approval of this request
for retroactive contract will result in no impact on payment limit.
BENEFIT TO THE COUNTY:
Approval of this retroactive contract will allow for contractor to continue providing all
necessary inpatient psychiatric hospital services to clients referred by Contra Costa
County.
AUTHORUATION OF SERVICES:
These services were requested/authorized by the Mental Health Division Care
Management Administrator, who is granted authority by the Mental Health Director,
pursuant to the California Welfare and Institutions Code, and Under,the Bronzon-
McCorquodale Act W&I Section 5608.

SQUAL NEEDS STATEMENT FOR ST. MELENA AND IT'S CA. SPECIALTY
HOSPITAL:
This contract meets the social needs of the County's population in that it provides
inpatient psychiatric treatment for severely emotionally disturbed clients from Contra
Costa County. In order to provide the best care possible for clients who need these
services, Contra Costa County has placed some clients in this facility. The provision of
acute, inpatient care in this facility assists in diverting clients from hospitalization at State
institutions.
