Approve and authorize the Health Services Director, or his designee (Adanna Henry) to execute on
behalf of the County,Novation Contract#24-429-32 with Ujima Family Recovery Services, a non-
profit corporation, in an amount not to exceed $1,335,666, to provide perinatal substance abuse
treatment services for the period from July 1, 2003 through June 30, 2004. This Contract includes a
six-month automatic extension through December 31,2004,in an amount not to exceed$667,833.
FISCAL IMPACT:
This Contract is 63.5%Federally funded,23.8% State funded,and 12.7%County funded.
CIM,DREN' IMPACT STATEMENT:
This Alcohol and Drug Abuse program supports the Board of Supervisors' "Families that are Safe,
Stable, and Nurturing" community outcome by providing parenting education, stability, and safety
for mothers (and their children) and pregnant women who are alcohol and drug dependent, while
they aro in substance abuse treatment. Expected outcomes include delivery of drug-free babies,
decreased use of alcohol, tobacco and other drugs, reduction in the number of relapses, and creation
of a sober social network.

This Contract meets the social needs of County's population in that it provides family-centered
alcohol and drug treatment services to pregnant and parenting women, in order to prevent perinatal
substance abuse and improve birth outcomes.
On October 15, 2002, the Board of Supervisors approved Novation Contract #2:4-429-30 (as
amended by Administrative Amendment Agreement #24-429-31) with Ujima 'Family Recovery
Services for the period from July 1, 2002 through June 30, 2003, (which included a six-month
automatic extension through .December 31, 2003) for the provision of perinatal intensive day
treatment,residential recovery program,and transitional housing for women.
Approval of Novation Contract #24--429-32 replaces the automatic extension ander the prior
Contractand allows the Contractor to continue providing services through June 30,2004.

Department with regard to custom healthcare software for the period from February 1, 2004

This Contract is ceded 100% by Third Party Revenue, and is included in the Department's
Enterprise I budget.
BACKGROUND/REASON(S)FOR RECOMMENDAT'IQIN S):
On February 11, 2003, the Board of Supervisors approved Contract 23-346 with Carole B.
Fitzgerald for the period from February 1, 2003 through January 31, 2004, to provide custom
healthcare software programming and support services. Approval of Contract #23-346-1 will allow
the Contractor to continue providing services through January 31,2005.

FISCAL IMPACT:
This Contract is 100%funded by Mental Health Inpatient Consolidation Funds.
BACKGROUNDIRFASON S?FOR RELOMMENDATION(S):
Chapter 633, Statutes of 1994, AB 757, authorized the transfer of state funding for Fee-For-
Service/Medi-Cal (FFS/M1C) acute psychiatric inpatient hospital services from the Department of
Health Services to the Department of Mental Health (DMH). Can January 1, 1395, the DMH
transferred these funds and the responsibility for authorization and funding of Medi-Cal acute
psychiatric inpatient hospital services to counties that chose to participate in this,program.
On June 17, 2443, the Board of Supervisors approved Contract #24-794--7(4) with St. Helena
Hospital and its California Specialty Hospital for the period from July 1, 2002 through June 30, 2443,
for the provision of inpatient psychiatric hospital services.
Approval of Contract 24-7947(5)will allow the Contractor to continue providing services through
June 30,2044.

STATEMENT OF NEED:
Each county is responsible for negotiating the reimbursement rates for all hospitals within
their jurisdiction. Given this, Contra Costa County cannot contract with out-of-county
facilities until the county-speck negotiation process is completed and the rates
published. This delays timely contract execution each year. Now that rates have been
established, it is imperative that a renewal of this contract be expeditiously approved.
This situation necessitates this request for retroactive approval of the contract for the
period'1/1/03-6/30/04.
Therefore, retroactive approval of this request is to cover the service activities of this
contractor and to assure the continuation of all necessary inpatient psychiatric hospital
services to clients referred by county as requested by County. Approval of this request
for retroactive contract will result in no impact on payment limit.
BENEFIT TO THE COUNTY:
Approval of this retroactive contract will allow for contractor to continue providing all
necessary inpatient psychiatric hospital services to clients referred by Contra Costa
County.

SOCIAL NEEDS STATEMENT FOR ST. HELENA AND IT'S CA.'SPECIALTY
HOSPITAL:
This contract meets the social needs of the County's population in that it provides
inpatient psychiatric treatment for severely emotionally disturbed clients from Contra
Costa County. In order to provide the best care possible for clients who need these
services, Contra Costa County has placed some clients in this facility. The provision of
acute, inpatient care in this facility assists in diverting clients from hospitalization at State
institutions.

Under Contract #26-487, Contractor will provide local and locum
tenens staffing of rehabilitation and cardiopulmonary personnel for
patients at CCRMC through December 31, 2004 .

On January 14, 2003,the Board of Supervisors approved Contract 74--191 with Vest Contra Costa
Unified School District for the period from July 1,20102 through June 30,2003,for the provision of
wraparound services to Severely Emotionally Disturbed(SED)children.
Approval of Contract#74-191-1 allows the Contractor to continue providing services through June
30,2004.

STATEMENT OF NEED FOR RETROACTIVE APPROVAL
in November 2001 an agreement was made between West Contra Costa Unified
School District(WCCUSD) and Contra Costa Health Services for WCCUSD to provide
child and family wraparound services for the period July 1, 2001 through June 30, 2002,
Incoming funds from West Contra Geta USD are used to fund the wraparound position
in this agreement under Contract 29-507. The school district needed some extra time
to negotiate the funding for this contract, and this delay prevented the contract from
renewing on duly 1, 2002.

Approval of this retroactive contract will allow for WCCUSD to continue providing
wraparound services to eligible clients who are part of the certified class mandated to
receive them.

(Jeff Smith, M.D. ) to execute on behalf of the County, Contract #26-
958-5 with Muir Orthopaedic Specialists, a corporation, effective
December 1, 2003, to amend Contract #26--958-4, to increase the total
Contract Payment Limit by $20, 000, from $340, 000 to a new total
payment limit of $360, 000.
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed ;for services.

Approve and authorize the Health Services Director or his designee
(Jeff Smith, M.D. ) to execute on behalf of the County, Contract
Amendment Agreement #26--360-5 with DELIVEREX, a corporation, effective
January 1, 2004, to amend Contract #26-360-3 (as amended by Contract
Amendment Agreement ##26-360`4) , to increase the total Contract Payment
Limit by $108, 000 from $415, 000 to a new total payment limit. of
$523 , 000 .
FISCAL CTs
Funding for this contract is included in the Department ' s Enterprise I
Budget .
On December 18,, 2001, the Berard of Supervisors approved Contract #26-
360-3 (as amended by Contract Amendment Agreement ##26-360-4) with
DELIVEREX, for the period from November 1, 2001 through October31,
2004, to provide off -site medical records storage and retrieval
services for Centra Costa Regional Medical Center (CCRMC) and various
Divisions within the Health Services Department .
Approval of Contract Amendment Agreement #26-360-5 will allow the
Contractor to provide additional services through October 31, 2004, as
requested by the Department, due to a continuous increase' in
utilization during the term of the agreement which was actually higher
than originally anticipated.

Approve and authorize the Health Services Director;, or his Designee
(Steve Tremain, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #26-446-1 with JLG Medical Transcription Services,
a corporation, effective October 1, 2043 , to amend 'Contract #26-446,
to modify the Payment Provisions with no change'; in the original
Contract Payment Limit of $1, 620, 000 .
FLECAL IMPACT:
This contract is funded in the Health Services Department Enterprise
I Budget .
On July 9, 2002, the Board of Supervisors approved Contract #26--446,
with JLG Medical Transcription Services, for the period from July 1,
2402 through June 30, 2005, to provide dictation and medical
transcription services, 24-hours per day, seven days per week, upon
request, for patients at Contra Costa Regional Medical Center (CCRMC) .
Approval of Contract Amendment Agreement #26-446-1 will allow the
Contractor to provide an additional level of transcription services,
to include electronic signature or e-signature services through June
30, 2045 .
