RECOMMENDATIONS)
Approve and authorize the Health Services Director, or his designee (Donna
Wigand) , to execute on behalf of the County, Contract Amendment Agreement
#24-681-66 (2) with Adelaida Manalang (dba Delly' s Care' Home) , a self-
employed individual, effective September 1, 2003, to amend Contract #24--681-
66 (as amended by Contract Amendment Agreement #24-681-66 (1) , to modify the
Payment Provision to accurately reflect the intent of the parties, with no
change in the original Contract Payment Limit of $32,400 and the term of the
Contract .
FISCAL IMPACT:
This Contract is 100% Mental Health Realignment funded. This Amendment will
reflect an increase in the monthly payment from $1, 200 per month to a new
monthly payment amount of $2,700 .
BACKGROIM/RRE_ASON(S) FOR R'E'•CO=NDAT10N(S)
1n July 2003 , the County Administrator approved and the Purchasing Services
Manager executed Contract #24-681-66 (as amended by Contract Amendment
Agreement #24-681-66 (1) with Adelaida Manalang (dba Delly' s Care Home) , for
the period from July 1, 2003 through June 30, 2004, to provide augmented
board and care services for County-referred mentally disordered clients .
Due to an administrative oversight, the Payment Provisions set forth in
Contract #24-681-66 (as amended by Contract Amendment Agreement) did not
reflect the intent of the parties, of which the Contractor was to receive
an increase to the monthly payment from $1, 200 to $2, 700 . ,Approval of this
Contract Amendment Agreement #24-681-66 (2) will correct the error, allowing .
the Contractor to continue to provide services to County-referred mentally
disturbed clients, through June 30, 2004 .

REQQ=NPATION(a) .
Approve and authorize the Health Services Director, or his designee (Donna
Wigand) to execute on behalf of the County, Novation Contract ##74--115-4 with
Desarrollo Familiar, Inc . , a non-profit corporation, in an amount not to
exceed $119, 031, for the period from July 1, 2003 through June 30, 2004, to
provide mental health services for Spanish-speaking recipients of the
CalWORKs Program. This Contract includes a six-month automatic extension
through December 31, 2004, in an amount not to exceed $59, 516 .
1+"I59AL IMPACT:
This Contract is 100%v funded by the State CalWORKs through. the Employment
and Human Services Department .
In August 1997, the State of California Legislature passed Assembly Bill
1542 which brought major changes to the welfare programs previously operated
by the State. Among the changes was a provision which required treatment
of substance abuse and mental illnesses of Welfare-to-Work participants,
when these conditions interfere with participation in 'Welfare--to-Work
activities . Subsequently, the County' s Employment and Human Services
Department and Health Services Department signed an Interdepartmental
Services Agreement (#21-427) which allowed the Health Services Department
to provide substance abuse and mental health services to 'Welfare-to-Work
participants referred by the Employment and Human ServicesDepartment .
On August 6, 2002, The Board of Supervisors approved Contract ##74-115-3 with
Desarrollo Familiar, Inc. , for the period from July 1, 2002 through June 30,
2003 (which included a six-month automatic extension through December 31,
2003) for the provision of mental health services, including individual,
group and family collateral counseling, case management, and medication'
management services for Spanish-speaking CalWORKs participants to reduce
barriers to employment.
Approval. of Novation Contract ##74-1.15-4 replaces the six-month automatic
extension under the prior Contract and allows the Contractor to continue to
provide services through June 30, 2004 .

RECOMMENDATION (S):
APPROVE and AUTHORIZE the Community Services Director or designee
to execute contract#38-461-9 with the YWCA of Contra Costa, in the amount
of$52,353 for Head Start Program wrap around services for the period from
January 1, 2004 through June 30, 2004.
FINANCIAL IMPACT:
Funded by the U.S. Department of Health and Human Services Administration for
Children and Families (ACF). Funding for this contract is included in the Contra
Costa County Community Services Department's FY 2003-04 Budget. There is no
net County cost to this contract.
CHILDREN'S IMPACT STATEMENT:
The Community Services Department Head Start Program supports two of Contra
Costa County's community outcomes: "Children Ready for and Succeeding in
School" and "Families that are Safe, Stable and Nurturing." These outcomes are
achieved by offering comprehensive services, including high quality early
childhood education, nutrition, health, and social services, along with a strong
parent involvement focus, to low-income children throughout Contra Costa
County. The overall goal of the program is to bring a greater degree of social
competence in preschool children from low-income families. The Head Start
program allows the County to provide these services to program eligible children,
ages 3 to 5 years.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
On September 16, 2003, the Board approved and authorized submission of the
2004 Head Start Continuation Grant Application to the U.S. Department of Health
and Human Services, Administration for Children and Families to continue Head
Start program services to program eligible children and their families for the period
from January 1,2004 through December 31, 2004.
Approval of this contract with the YWCA of Contra Costa, will ,allow for
continuing Head. Start wrap around services to program eligible children ages three
(3)to five (5)years, in a full day childcare program.

Administration for Children and Families. First Baptist Church is responsible for the local,
non-cash, in-kind match of$464,070. Funding for the period from January 1, 2004 through.
June 30, 2004 is included in the Department's FY 2003-04 Budget, Funding for the period
from July 1, 2004 through December 31, 2004 will be included in the Department's

RECOMMENDATION (S):
APPROVE and AUTHORIZE the Community Services Department Director or designee to
execute contract amendment #38-787-3 with Eva Boyd to increase the payment limit by
$23,500 to a new payment limit of$47,000 and to extend the contract period from December
31, 2003 to June 30, 2004 for the provision of Administrative Support services,
FINANCIAL IMPACT:
This contract is funded through administrative portions of State and federal grants. Funding
for this contract is included in the Community Services Department's FY 2003-2004 Budget.
There is no net County cost to this contract.
CHILDREN'S IMPACT STATEMENT:
The Community Services Department Child Development Division supports two of Contra
Costa County's community outcomes: "Children Ready for and Succeeding in School"
and "Families that are Safe, Stable and Nurturing." These outcomes are achieved by
offering comprehensive services, including high quality early childhood education,
nutrition, and health to low-income children throughout Contra Costa County.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
In consideration of Contractor's agreement to provide continuing Administrative Support
services under the contract identified, County agrees to increase the Contract Payment
Limit and extend the contract Term. County and Contractor agree, therefore, to amend said
Contract as specified.
Approval of this request will allow for the provision of continued Administrative Support
services.

RECOMMENDATION N:
APPROVE and AUTHORIZE the Community Services Department Director or designee to
execute contract amendment #38-756-6 with Lenore Thompson in the amount of$14,150 for
a new contract payment limit of$28,825, and to extend the contract period from December
31, 2003 to June 30, 2004 for Disabilities/Mental Health Consultant services.
FINANCIAL IMPACT:
Funded by the U.S. Department of Health and Human Services Administration for Children
and Families (ACF). Funding for this contract is included in the Contra Costa County
Community Services Department's FY 2003-04 Budget. There is no net County cost to this
contract.

REASONS FOR.RECOMMENDATIONS/BACKGROUND:
Contractor shall provide screenings to identify infants and toddlers with, or at-risk of
disabilities. Contractor shall train staff on disabilities/mental health issues specific to
infants and toddlers. Contractor shall also assist in devising and implementing the
Disabilities/Mental Health Service Plan. Contractor shall also observe infants and toddlers
in classrooms, make outside referrals as appropriate, and provide crisis intervention.
Contractor shall assist in coordinating, monitoring, and evaluating service delivery and
follow-up on special education and mental health services.
Approval of this contract will allow contractor to provide Disabilities/Mental Health
services for the Community Services Department program eligible in is and toddlers
through June 30, 2004.

REASONS FOR RECOMMENDATIONS/BACKGROUND:
Contractor shall oversee and supervise the Community Services Department's Mental
Health Interns. Contractor shall participate in the annual review and development of
Mental Health Service plans, as well as keep appropriate records of Mental Health Interns
in meeting licensing requirements in accordance with applicable State law. Contractor shall
also observe Head Start and State Preschool children in classrooms, make outside referrals
as appropriate, provide crisis intervention, and supervise play therapy in accordance with
applicable State law.
Approval of this contract will allow contractor to provide Mental Health Intern Clinical
Supervisor services to the Community Services Department through 30, 2004.

Approve and authorize the Health Services Director or his designee (Donna
Wigand) to execute on behalf of the County, Novation Contract #74-058-5 with
Seneca Residential & Day Treatment Center, Inc . , a non-profit corporation,
in an amount not to exceed $543, 198, for the period from July 1, 2003
through June 30, 2004, to provide intensive crises stabilization services
to youth through the Mobile Crisis Response Teams. This Contract includes
a six month automatic extension through December 31, 2004, in an amount not
to exceed $271, 539 .
FISCAL IMPACT:
This Contract is funded 30% by Federal FFP, 30k by State EPSDT and 4001 by
Federal SAMHSA Funds.

This Contract meets the social needs of County' s population in that it
provides Intensive Crisis Stabilization Services to Seriously Emotionally
Disturbed (SED) children and adolescents (and their families) in high-risk
situations, to reduce the need for psychiatric hospitalization.
On December 17, 2002, the Board of Supervisors approved Novation Contract
#74- 058-3 (as amended by Contract Amendment Agreement #74-058-4) with Seneca
Residential & Day Treatment Center, Inc. to provide intensive crises
stabilization services to youth through the Mobile Crisis Response Teams,
for the period from July 1, 2002 through June 30, 2003, (which included a
a six-month automatic extension through December 31, 2003) .
Approval of Novation Contract #74-058-5 replaces the six-month automatic
extension under the prior Contract and allows the Contractor to continue
providing services through June 30, 2004 .
