SPECIFIC REQUEST(S)OR RECC3IVI'r+ ENDAT ON(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on behalf of the County, Novation Contract #24-933-16 with Crestwood Behavioral Health, Inc., a corporation, in an amount not to exceed $5,652,362, to provide subacute skilled nursing care for County's Seriously and Persistently Mentally III (SMPI) and Neurobehavioral clients, for the period from July 1, 20012 through June 30, 2003. This Contract includes a six-month automatic extension through December 31, 2003, in an amount not to exceed$2,826,181. FISCAL IMPACT: This Contract is funded 100%by Mental Health Realignment, offset by third party billing. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): Effective July 1, 1992, State Mental Health Realignment Ugislation shifted responsibility for payment to providers from the State to the Counties and required Counties to assume direct responsibility for contracting with sub-acute skilled nursing facilities. Crestwood Behavioral Health, Inc. has been providing services since 1998. On June 11, 2001,the Board of Supervisors approved Novation Contract#24-933-13 (as amended by Contract Amendment Agreements #24-933-14 and#24-933-15) with Crestwood Behavioral Health, Inc. for the period from July 1, 2001 through June 30, 2002, which included a six-month automatic extension through December 31, 2002, for the provision of subacute skilled nursing care for County's Seriously and.Persistently Mentally III (SMPI) and Neurobehavioral clients. Approval of Novation Contract #24-933-16 will replace the six-month automatic extensions under the prior Contract, and allows the Contractor to continue providing services through June 30, 2003. r NT€ ED ON ATTACHMENT. (dba Senior Outreach Services of Contra Costa) SPECIFIC REQUEST(S)OR RECOFAMENDAT€ON(S)&BACKGROUND AND JUSTIFICATION RECOMM ENDATION(S): Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.) to execute on behalf of the County, Novation Contract #22-137-31 with Family and Community Services, Inc. (dba Senior Outreach Services of Contra Costa), a non-profit corporation, in an amount not to exceed $260,000, to provide home-delivered meal services for the Senior Nutrition Program, for the period from July 1, 2002 through June 30, 2043. This Contract includes a four-month automatic extension through October 31, 2003, in an amount not to exceed$86,667. FISCAL IMPACT: This contract is 140%a federally funded under Title III-C(1) of the Older Americans Act of 1965. No County funding is required. BACKGROUND/REASON(S)FOR RECOIVIMENDATION(S): This Contract meets the social needs of County's population in that it provides home delivered meals on 250 serving days, to an average of 540 senior citizens, to ensure they receive at least 1/3 of their daily nutritional requirements. On January 22, 2002, the Board of Supervisors approved Contract #22-137-29 (as amended by Contract Amendment Agreement #22-137-30) with Family and Community Services, Inc. (dba Senior Outreach Services of Contra Costa) for the period from November 1, 2001 through June 30, 2002, which included a four-month automatic extension through October 31, 2002, for the provision of home delivered meals services for the Senior Nutrition Program. Approval of Novation Contract #22-137-31 will replace the four-month automatic extension under the prior agreement and allow the Contractor to continue providing services through June 30, 2003. FISCAL IMPACT: This Contract is funded 100% in County's Enterprise I budget. Approval of this Contract will result in a cumulative annual total in excess of $25,000; therefore, Board of Supervisors approval is required. BACKGROUND/REASON(S)FOR RECOMMENDATION(S}: In June 2402, the County Administrator approved, and the Purchasing Services Manager executed Contract#23-298-1 with Mark Clark for the period from June 1, 2402 through November 30, 2002, for the provision of consultation and technical assistance with regard to programming and administration for the Department's Information Systems Unit. Approval of Contract #23-298-2 will allow the Contractor to continue providing; services through November 30, 2003. James, MFT, a self-employed individual, for the period from December 1, 2002 through November 30, 2003, in an amount not to exceed $37, 440, for the provisions of clinical psychology services for the Mental Health Module at the County' s Detention Facility. FINANCIAL IMPACT: This Contract is funded by County General .Fund dollars in the Detention budget unit . REASONS FOR RECOMMENDATIONS/BACKGROUND: On December 18, 2001., the Hoard of Supervisors approved Contract #f:26- 428 with Penny S. James, MFT to provide temporary relief coverage with regard to clinical psychology services for patients at the County Detention Facility including, but not limited to, individual and group therapy, psychological testing, evaluations and diagnosis, for the period from December 1, 2001 through November 30, 2002 .
