Wigand, LCSW) , to execute on behalf of the County, Contract #24-887-5 with Willie Hardaway (dba Hardaway Family Care Home) , for the period from March 1, 2042 through February 28,•, 2443, in an amount not to exceed $36, 444, for the provision of augmented board and care services for County-referred mentally disordered clients. FISCAL IMPACT. This Contract is 100% funded by Mental Health Realignment. BACKGR S N S OR RE d A IONS The Department ' s residential support program, called Supported Adult Group Enterprises (S.A.G.E. ) provides Augmented Board and Care Services designed to meet the needs of the seriously persistently mentally ill (SPMI) adults, who currently reside in locked facilities and state hospitals. Hardaway Family Care Home is one of the few board and care facilities that was able to meet the S.A.G.E. Program requirements under the Department ' s Request For Proposal . FISCAL IMPACT: This Contract was funded 100% by Federal Medi-Cal and State EPSDT Funds . BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : On August 14, 2001, the Board of Supervisors approved Contract #74- 130 with Nanette Murphy, for the period from July 1, 2001 through June 30, 2002, to provide consultation and technical assistance to the Children' : Mental Health Program with regard to parent support services . The purpose of this Board Order is to advise the Board of Supervisors that the Department and the Contractor, have agreed to terminate Contract #74-130, effective October 10, 2001 . The primary reasons were stated in our conversation of 10 October 2001. I appreciate the opportunity to have participated in Parent Partners. Unfortunately, I feel my part was to loosely defined. This led to difficulty in my assuming any responsibility, particularly in the area of notifying parents of their rights, and the staff of their responsibility regarding Special Education. I am a strong advocate in this, and any less effort is a personal violation of my ethics in this matter. I also realize the job was eventually going to be more than this, but when, it was hard to say. My frustration level was acutely high. I thank you, and Kathy .Davison, for the opportunity to interview, and be selected for this position. I am truly saddened that with all the effort put forth by all parties that it didn't work out. SPECIFIC REQUEST(S)OR RECOMMENDATIONS?&BACKGROUND AND JUSTIFICATION RLCgZ-=NPATIQX(S) : Approve and authorize the Health Services Director or his designee {Donna Wigand} to execute on behalf of the County, Contract #74-161 with Phoenix, Programs, Inc. , in an amount not to exceed $406, 986, to provide integrated housing services to the homeless mentally ill residents enrolled in facilities of the Street Relief Continuum, for the period from October 1, 2001 through June 30, 2002 . FISCAL IMPACT: This Contract is 100 Stated funded by the California Department of Mental Health. BACKGROUND/REASON S FOR R$CQXXE1I7ATZrON{S)_: The California Department of Mental Health has allocated funding to the County' s Mental Health Division Adult Systems of Care, pursuant to Section 5814 . 5 of the Welfare Institutions Code and Sections 5800 et seq. County' s Mental Health Division will utilize this one-time only AB2034 grant to provide the Street Relief Continuum Care services to homeless mentally ill residents who are not currently connected to the Mental Health System of Care Program. Phoenix. Programs, Inc. , operates the Antioch Homeless Shelter, Nevin House, and Neireka House. These facilities are located in all three regions of the County, and serve a diversity of consumers within the target population. Under Contract #74-161, Phoenix Programs, Inc. , will provide integrated housing services to the homeless mentally ill residents, including emergency and immediate housing, transitional housing, and permanent housing services at facilities in the Street Relief Continuum, through June 30, 2002 . SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION LS) : Approve and authorize the Health Services Director, or his designee (Chuck Deutschman) to execute on behalf of the County, Contract #74-162 with Rubicon Programs, Inc. , in an amount not to exceed $68, 187, for the period from February 1, 2002 through June 30 2002, for the provision dual diagnosis treatment services in East, Central and West Contra Costa County. This Contract includes a automatic six-month extension through December 31, 2002, in an amount not to exceed $81, 824 . FISCAL IMPACT This Contract is funded 100 by State Substance Abuse and Crime Prevention Act (SACPA) - Prop 36 funds . No County funds are required. BACKGROUND/REASON(S) FOR RECOMMENDATYON(S) : The Substance Abuse and Crime Prevention Act of 2000 (Prep 36) has mandated and funded Counties to establish Alcohol and Other Drug MOD) treatment programs for non-violent drug offenders in lieu of incarceration. On February 6, 2001, the Board of Supervisors designated the County' s Health Services Department, through its Community Substance Abuse Services Division, as the lead agency in Contra Costa County responsible for the implementation of Prop 36 treatment efforts. Under Contract #74-162, the Contractor will provide dual diagnosis treatment services in East, Central and West Contra Costa County, through June 30, 2002 .
