RECOMMENDATION(S)_:
Approve and authorize the Health Service Director, or his designee
(Chris Grazzini) to execute on behalf of the County, Contract
Amendment Agreement #26-431-1 with Complete Systems Diagnostics,
effective February 25, 2002, to amend Contract, #26-431, to increase
the total Contract Payment Limit by $30, 000 from $25, 000 to a new
total payment limit of $55, 000 .
FISCAL IMPACT:
This Contract is funded by the Health Services Department ' s Enterprise

BACK.GROUNWREASON(S) FOR RECOMMENDATION(3) :
In December 2001, the County Administrator approved and the Purchasing
Services Manager executed Contract #26-431 with Complete Systems
Diagnostics, to provide temporary Ultrasound technologist to assist
Contra Costa Regional Medical Center and Contra Costa Health Centers
during peak loads, temporary absences and emergency situations in
critical areas, for the period from January 2, 2001 through January 2,
2003 .
Approval of Contract Amendment Agreement #26-431-1, will allow
Contractor to provide additional ultrasound staffing coverage services
for Contra Costa Regional Medical Center, through January 2, 2003 .

None . This Contract was 1001 State funded.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On May 22, 2001, the Board of Supervisors approved Contract #22-803
with Catherine Litwin-Weigle, to provide consultation and technical
assistance to the Department as the Provider Relations Consultant
for the Breast Cancer Partnership Program.
The purpose of this Board Order is to advise the Board that the
Department and the Contractor, have mutually agreed to terminate
Contract #22-803 with Catherine Litwin-Weigle, effective December
20, 2001 .

Dear Kim:
This letter is to inform you of my resignation from my current position as provider relations
consultant.
As you know, my husband has decided to take a job that will move us back to southern California
in the middle of January. My last official day of work will be December 18th.We are moving out of
our present home on January 10th and I will need time to pack and get organized.
It has been a privilege and an honor to work with such an amazing talented group of
professionals. It has been 10 years since I have enjoyed a job as much as this.Thank-you for the
opportunity to grow professionally and to work with a program that has assisted underserved
women in accessing early detection screening for breast cancer.

(Donna Wigand) , to execute on behalf of the County, Contract Amendment
Agreement #24-681-28 (10) with Lynda Walton (dba St . John' s Board &
Care Home) , to amend Contract #24-681-28 (9) , effective October 1,
2001, to increase the payment limit by $6, 000 from $48, 000 to a new
total Contract Payment Limit of $54, 000 .
FISCAL IMPACT:
Funding for this Contract is included in the Department ' s Fiscal Year
2001-2002 Budget and is 100% funded by Mental Health Realignment
funding.
BACKGROUND/RBASON(S) FOR RECOMMENDATION W :
On July 17, 2001, the Board of Supervisors approved Contract #24-681-
28(9) with Lynda Walton (dba St . John' s Board & Care Home) , for the
period from July 1, 2001 through June 30, 2002, for the provision of
augmented board and care services for County-referred mentally
disordered clients .
Approval of Contract Amendment Agreement #24-681-28 (10) will allow the
Contractor to provide additional services to County-referred mentally
disordered clients, through June 30, 2002 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Jeff Smith, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #26-958-3 with Muir Orthopaedic Specialists
(Formerly: Muir Orthopedics) , Specialty: Orthopedics, effective
December 1, 2001, to amend Contract #26-958-•2, to increase the total
Contract Payment Limit by $36, 500 from $82, 000 to a new total
payment limit of $118, 500 .
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and health center.
On April 3, 2001, the Board of Supervisors approved Contract #26-
958-2 with Muir Orthopaedic Specialists (Formerly: Muir
Orthopedics) , for the period from March 1, 2001 through February 28,
2002, to provide orthopedic services for Contra Costa Regional
Medical Center.
At the time of negotiations, payment limit was based upon target
levels of utilization, however, the utilization for Orthopedic
patient care services during the term of the agreement was actually
higher than originally anticipated.
Approval of Contract Amendment Agreement #26-958-3, will allow Muir
Orthopaedic Specialists (Formerly Muir Orthopedics) to provide
additional Orthopedic services through February 28, 2002 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, to execute on
behalf of the County, Contract #26-178-19, with Linda Nakell, Ph.D. ,
in an amount not to exceed $81,468, for the period from March 1,
2002 through February 28, 2003, to provide professional behavioral
science consultation services for the County' s Family Practice
Residency Program, Symptom Control Program, and AIDS Program.
FISCAL IMPACT:
Funding for this contract is included in the Department ' s Enterprise
I Budget, and as appropriate, patients and third-party payors will
be billed for contractor' s services.
BACKGROUND/REASON(S)FOR RECOMMENDATION(S) :
The Family Practice Residency Program requires behavioral science
faculty, and Dr. Nakell has provided professional behavioral science
consultation and training for the Department ' s Family Practice
Residency Program since 1988 . In addition, Dr. Nakell supervises
Residents, works with the Department ' s AIDS and Symptom Control
Programs, conducts a clinic, and consults with patients referred to
her by Residents.
On February 27, 2001, the Board of Supervisors approved Contract
#26-178-18, with Linda Nakell, Ph.D. , for the period from March 1,
2001 through February 28, 2002 .
Approval of Contract #26-178-19 will allow Dr. Nakell to continue
providing services through February 28, 2003 .
