HomeMy WebLinkAboutMINUTES - 10162001 - C.85 R 4 TO: BOARD OF SUPERVISORS s FROM: William Walker, M.D. , Health Services Director Contra By: Ginger Marieiro, Contracts Administrator Costa DATE: October 3 , 2001 "a^re County SUBJECT: Cj�� Approval of Contract Amendment Agreement #22-7'69-3 with Cho Nai Cheung SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S) : Approve and authorize the Health Services Director or his designee (Lewis Pascalli) to execute on behalf of the County, Contract Amendment Agreement #22-769-3 to amend Contract #22-769-2 with Cho Nai Cheung, effective September 30, 2001, to extend the term from September 30, 2001 through December 31, 2001 and to increase the Contract Payment Limit by $32 , 636, from $32 , 636 to a new total Contract Payment Limit of $65, 272 . FISCAL IMPACT: This Contract is funded by the California Accidental Release Prevention (CAL/ARP) Program revenues included in the Health Services Department ' s Budget . BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : On June 26, 2001, the Board of Supervisors approved Contract #22-769-2 with Cho Nai Cheung for consultation and technical assistance to the Department ' s Hazardous Materials Division regarding risk management and prevention programs for the period from July 2 , 2001 through September 30 , 2001 . Approval of Contract Amendment Agreement #22-769-3 will allow the Contractor to continue providing services through December 31, 2001 .
