HomeMy WebLinkAboutMINUTES - 10162001 - C.83 TO: BOARD OF SUPERVISORS FROM: William Walker, M.D. , Health Services Director By: Ginger Marieiro, Contracts Administrator �* Contra ot Costa DATE: fiber 3, 2001 �oUNo`° - C q3 County 0 SUBJECT: Approval of Contract Amendment Agreement #26-895-17 with ` Jason Appel, M.D. SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S) : Approve and authorize the Health Services Director, or his designee, (Frank Puglisi, Jr. ) , to execute on behalf of the County, Contract Amendment Agreement #26-895-17, effective October 1, 2001, with Jason Appel, M.D. , (Specialty: Orthopedics) , to amend Contract #26-895-16 , to extend the term of the Contract through December 31, 2002 , and to increase the total Contract Payment Limit by $87, 000 from $52 , 000 to a new total payment limit of $139, 000 . FISCAL IMPACT: Cost to the County depends upon utilization. As appropriate, patients and/or third party payors will be billed for services . BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : On January 19 , 2001, the Board of Supervisors approved Contract #26- 895-16 with Jason Appel, M.D. , for the provision of Orthopedic services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, including clinic coverage, consultation, training, and/or medical procedures, for the period from January 1, 2001 through December 31, 2001 . Approval of Contract Amendment Agreement #26-895-17 will allow Contractor to provide additional Orthopedic services through December 31, 2002 .
