BACKGROUND
The long-standing obligation of the Redevelopment Agency to fund the Iron
Horse Trail gap closure project between Hookston Rd. and Mayhew is to be undertaken in
this fiscal year. The project will occur'concurrently with the City of Concord's realignment
of the Hookston/Bancroft Rd. intersection. The construction of the Iron Horse Trait gap
closure will require the reconfiguration of the current private use, on the sites, and require
the relocation of one tenant (Tri-Cities Concrete), who is eligible for relocation benefits.
The recommended contract covers Public Works - Real property ',staff costs` and
consultant casts necessary to complete required appraisal, title, and relocation assistance
work. This contract does not cover the costs of any relocation benefits.

ACCEPT additional California Department of Aging funding for FY 2000/2001 in the amount
of$37,608 for a total amount of$738,408. (#29-092-1)

RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment and Human Services Director, or designee to ACCEPT
additionalCalifornia Department of Aging funding for FY 2000/2001 in the amount of$37,608 for a total
amount of$738,408. (#29-092-1)
FINANCIAL. IMPACT:
Per California Department of Aging Agreement(MS-001-27)for Multipurpose Senior Service Program
(MSSP),county to receive total funding in the amount of$738,408. No County match required.
BACKGROUND:
The California Department of Aging funding is yearly funding (FY 2000/2001 funding approved by the Board
May 16, 2000) accepted by the Department on behalf of the County to provide case management on a
countywide basis for Contra Costa County residents age 65 and over, Medi-Cal eligible, and certifiable for
skilled nursing facility care. The Multipurpose Senior Service Program provides case management for
assessment, care planning, service arrangement, and client monitoring through a team of health and social
service providers. The provision of case management services to this senior population allows individuals who,
may otherwise require'institutionalization, to remain safely in their own residences.

RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment & Human Services Director, or his designee, to APPLY for

Community Development Department in the amount of$75,000 for micro-enterprise development for the
period from July 1, 2001through June 30, 2002.
FINANCIAL IMPACT:
No County Costs. This federal grant will fund the Small Business Management Assistance Program through
the County Small Business Development Center for micro-enterprise development.
CHILDREN'S'IMPACT STATEMENT:
Activities under this grant will directly support the third community outcome: "Families that are
Economically Self-Sufficient."
BACKGROUND:
This application is for on-going funding for the Small Business Management Assistance Program through the
County Small Business Development Center. The purpose of this grant is to provide business assistance for
micro-enterprise development, with the goal of facilitating the creation and/or the retention of jobs that
directly benefit low-to-moderate income residents in the urban portions of the County.

Approve and authorize the Health Services Director, or his designee (Milt
Carnhi) , to execute on behalf of the County, Contract ##29-780 with Siskyou
Hospital. (dba Fairchild Medical Center) , for the period from June 12, 2000
through May 31, 2002, for Contra Costa Health Plan to provide Advice Nurse
services to Fairchild Medical Center Health ''Plan members, to be paid as
follows :
(1) Upon execution of this ;Agreement Agency shall pay County $3,500 to reimburse County
for the start-up costs actually incurred by County;
(2) For the period from June 12, 2000 through June 30, 2000 County shall pay Agency
$2,280 per month for up to 400 calls; and
(3) For the; period from July 1., 2000 through May 31 2002, County shall pay Agency in
accordance with the following rate schedule:

FISCAL 'IMPACT;
For a fee the Contra Costa Health Plan will provide Advice Nurse Services
to Contractor. The revenue generated by this Contract will be used to
offset the cost of Contra Costa 'Health Plan' s Advice Nurse services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
Under Contract 429-780,` the Contra Costa Health Flan' (Health' Plan) will
provide Contractor' s Health Plan members with ': telephone advice nurse
services including: information about how to access urgent care services;
authorization for emergency care; and clinical advice:
Approval of this Contract will allow. Contra Costa Health 'Plan to provide
services, through May 31, 2002 .

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMENDED ACTION:
Accept $30, 000 from the State Department of Mental Health, for the
period from July 1, 2000 through June 30, 2001 , for training of local
mental health boards and commissions in the Bay Area ''Region of the
California MentalHealth Directors' Association
FISCAL IMPACT'S
Acceptance of this grant award will result in $30, 000 for the training
of local mental health boards and commissions in the Bay Area ',',Region of
the California Mental Health Directors Association.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
Local mental health boards and commissions'' in the Bay Area 'Region of
the California Mental Health,' Directors Association are in need of
training and technical' assistance in order to perform the work that is
legislatively: mandated.. Approval of 'Award #29-515 from the State
Department ofMental Health will allow 'Contra Costa County to provide
necessary training for local mental health boards and commission,
through June 30, 2001 .
Three certified copies of the'' Board Order should be returned to the
Contracts and Grants Unit.
