Approval of Contract Amendment Agreement #22-767-3 with Toni Guy
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Contract
Amendment Agreement #22-767-3 with Toni Guy, effective November 1,
2001, to amend Contract #22-767-1 (as amended by Contract Amendment
Agreement #22-767-2) , to increase the Contract Payment Limit by
$2 , 971 from $64 , 629 to a new payment limit total of $67, 600 . This
Contractor provides consultation and technical assistance with
regard to the countywide comprehensive violence prevention plan.
FISCAL IMPACT•
This Contract is 15 . 5% State funded and 84 . 5% County funded.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On November 14 , 2000, the Board of Supervisors approved Contract
#22-767-1 (as amended by Contract Amendment #22-767-2) with Toni Guy
to provide consultation and technical assistance with regard to
development of a countywide comprehensive violence prevention plan
including, but not limited to, acting as a Violence Prevention
Planning and Resource Development consultant; implementing an action
plan and conducting violence prevention planning and resource
development, through December 31, 2001 .
Approval of Contract Amendment Agreement #22-767-3 will allow
Contractor to provide additional services, through December 31 ,
2001 .

AMENDMENT SPECIFICATIONS

In consideration for Contractor' s agreement to provide
additional hours of services under the Contract identified
herein (as amended by Contract Amendment Agreement #22-767-2) ,
County agrees to increase the Contract Payment Limit . County
and Contractor agree therefore to amend said Contract as
specified below while all other parts of the Contract remain
unchanged and in full force and effect .
1 . Payment Limit Increase. The payment limit set forth in
Contract Amendment Agreement #22-767-2 , Amendment
Specifications Paragraph 2 . (Payment Limit Increase) , is
hereby increased by $2, 971 from $64 , 629 to a new total
Contract Payment Limit of $67, 600 .
2 . Modification of Payment Provisions . Contract Amendment
Agreement #22-767-2, Amendment Specifications, Paragraph 3 .
(Modification of Payment Provisions) is hereby deleted and
replaced with a new paragraph to read as follows :

Provisions Paragraph 1 . (Payment Amounts) subparagraph
d. , is hereby deleted and replaced with a new
subparagraph d. to read as follows :
[X] d. (1) $30 . 90 per hour of service, not to

(2) In addition, County will reimburse
Contractor an amount not to exceed
$2 , 500 as follows : a) for allowable
miscellaneous expenses actually
incurred in the provision of services
hereunder during the term of this
Contract; b) for mileage for travel
in her/his personal vehicle as
required by service activities under
this contract . Such reimbursement
shall be paid at the same rate and in
the same manner as if Contractor were
a County employee and c) for
conference registration and travel on
behalf of the County, approved in
advance by the Public Health
Director. "
