Approval of Contract Amendment Agreement #24-905-7 with
Marc F. Bono, Psy.D.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) to execute on behalf of the County, Contract
Amendment Agreement #24-905-7 with Marc F. Bono, Psy.D. , effective
December 31, 2001, to amend Contract #24-905-5 (as amended by
Contract Amendment Agreement #24-905-6) , to extend the term of the
Contract through June 30, 2002 and to increase the total Contract
Payment Limit by $44 , 700, from $82 , 569 to a new total payment limit
of $127, 269 .
FISCAL IMPACT:
This Contract is funded by 75% State and 25% Mental Health
Realignment funds .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On December 19, 2000, the Board of Supervisors approved Contract
#24-905-5 (as amended by Contract Amendment Agreement #24-905-6)
with Marc F. Bono, Psy.D. , for the period from January 1, 2001
through December 31, 2001, for the provision of consultation and
technical assistance with regard to the Dual Diagnosis Project,
including the design of a Dual Diagnosis Treatment Program Plan,
assist with project evaluation and research, conduct training to
County-selected staff, and to perform a range of clinic program
activities, as requested by County.
Approval of Contract #24-905-7 will extend the term of the agreement
and allow the Contractor to provide additional hours of services
through June 30, 2002 .
