RECOMMENDATION A. APPROVE a Fifth Amendment authorizing extra work under the Consulting Services Agreement dated May 25, 1999, with Interactive Resources, Inc. for Architectural Design Services for Remodeling and Office Landscape Partitions at Various Sites for Employment and Human Services Department. The Fifth Amendment increases the scope of services and increases the Payment Limit, as amended by the First, Second, Third and Fourth Amendments, from $636,115.00 to $733,795.00, an increase of$97,680.00. B. AUTHORIZE the Director of General Services to execute the Fifth Amendment to the Consulting Services Agreement, and to issue written authorizations for extra work, in addition to the authorizations for the First, Second, Third, Fourth and Fifth Amendments, provided that the cu nulative total extra cost for such additional authorizations shall not exceed $25,000.00 II. FINANCIAL IMPACT The architectural services necessary to design office landscape partitions (OLP) for various Employment and Human Services offices is sufficiently budgeted within the Employment and Human Services budget under professional services. REASONS FOR RECOMMENDATIONSBACKGROUND A. The Board of Supervisors approved and the Director of General Services executed a Consulting Services Agreement with Interactive Resources, Inc., effective May 25, 1999, for architectural services for Remodeling and Office Landscape Partitions (OLP) at Various Sites for Employment and Human Services Department. B. The First Amendment increased the scope and authorized the following additional architectural services. 1. At 4545 Delta Fair Blvd., Antioch, OLP scope increased from 170 to 226 workstations. Provided additional architectural services to accommodate increased number of workstations, minor mechanical modifications and reviewed existing electrical site conditions. 2. At 151 Linus Pauling, Hercules, OLP scope increased from 100 to 195 workstations. Provided revised architectural drawings to accommodate new floor layout. Changes included demolition of existing walls, modifications to existing mechanical and electrical systems, new doors and modifications to floors and ceilings. This amendment did not include redoing hardware in doors not directly affected by revised design. 3. At 201 East 18`h Street, Antioch, provided additional architectural, mechanical, electrical and furniture layout revisions, including new hardware throughout building. C. The Second Amendment increased the scope and authorized additional architectural services required to revise layout of workstations to place the Social Service and Child Welfare Divisions on opposite sides of the second floor at 151 Linus Pauling, Hercules. D. The Third Amendment increased the scope and authorized additional architectural services for design modifications, minor changes to the construction drawings and major changes to bill of materials required for splitting Phase 2 into two phases (2a and 2b) and changing the electrical from a 6-wire system to an 8-wire system at 4545 Delta Fair Blvd., Antioch. E. The Fourth Amendment increased the scope and authorizes additional architectural services. 1. At 151 Linus Pauling, Hercules, restarted the preliminary design phase and provided a new preliminary design to accommodate new project requirements. 2. At 151 Linus Pauling, Hercules, provided a structural analysis of the existing second floor framing system. F. The Fifth Amendment increases the scope and authorizes additional architectural services. I. At 30 and 40 Muir Road, Martinez, additional architectural services required to increase the project scope for remodeling to accommodate additional staff and increase space planning services. 2. At 30 and 40 Muir Road, Martinez, provide structural, civil and data voice work that was not part of original fee proposal. 3. At 30 and 40 Muir Road, Martinez, provide additional mechanical engineering services required to modernize the mechanical and electrical infrastructure. Effective Date and Parties: Effective November 6,2001, Interactive Resources, Inc. (herein called "Consultant"), a California Corporation, and the County of Contra Costa(herein called"County"),a political subdivision of the State of California,mutually agree as follows: 2. Purpose: On May 25, 1999,the parties entered into a contract entitled "Consulting Services Agreement," referred to as the "Agreement," which covers Architectural Design Services for Remodeling and Office Landscape Partitions at Various Sites for Employment and Human Services Department. The Agreement has previously been amended as follows: First Amendment dated March 7,2000, Second Amendment dated May 4, 2000, Third Amendment dated September 25, 2000, and Fourth Amendment dated October 16, 2001. The parties desire to further amend the Agreement to expand the scope of service and to increase the payment limit accordingly. 3. Amendments to Agreement: A. In the Agreement, Section 1(e), change the Payment Limit from $636,115.00 to $733,795.00, an increase of$97,680.00. This change shall apply to charges from and after October 2,2001. B. In accordance with Section 14 of the Agreement,provide the following services: I. At 30 and 40 Muir Road, Martinez, additional architectural services required to increase the project scope for the remodeling to accommodate additional staff and increase space planning services. 2. At 30 and 40 Muir Road,Martinez,provide structural,civil and data voice work that was not part of original fee proposal. 3. At 30 and 40 Muir Road, Martinez, provide additional mechanical engineering services required to modernize the mechanical and electrical infrastructure. 4. Effect: Subject to the revisions made by this and any prior amendments,the Agreement shall remain in full force and effect. 5.
